Zusammenfassung. Insgesamt N = 933 Personen beiderlei Geschlechts im Alter von 40 bis 65 Jahren schätzten zu jedem von acht vorgegebenen Risikofaktoren psychosozialer Provenienz deren Bedeutung für die Entstehung von Krebs- (Ca) und Herzkreislauferkrankungen (HK) ein. Die hypothetischen Risikofaktoren waren im Hinblick auf die Theorie von Grossarth-Maticek und Eysenck (1990) formuliert worden. Neun Jahre nach der Ersterhebung (t1) bearbeiteten dieselben Personen in einer Follow-Up-Erhebung (t2) dieselben Items erneut. Zu t1 waren nCaalt = 45 Probanden an Ca vorerkrankt; nCaneu = 31 kamen bei t2 dazu. Für HK lauteten die entsprechenden Zahlen nHKalt = 34 und nHKneu = 7. In t1 maßen die Probanden (Pbn) den meisten der psychosozialen Risikofaktoren für Ca eine größere Bedeutung zu als für HK. Das galt insbesondere für "Anhaltende emotionale Unsicherheit" und "Andauernde Unterdrückung eigener Bedürfnisse". Zu t2 kam es zu gegensinnigen Unterschieden; generell war die Bedeutung der Risikofaktoren für Ca zurückgegangen, während diejenige für HK zugenommen hatte. Zwischen gesunden und kranken Pbn bestanden keine systematischen Unterschiede. Gesonderte Analysen zeigten, dass für die von t1 zu t2 aufgetretenen Unterschiede nicht das angestiegene Alter der Pbn, sondern die vergangene Zeit verantwortlich war. Dahinter mag sich die angemessene Rezeption aktueller Forschungsergebnisse durch die Bevölkerung und eine zugenommene Skepsis gegenüber psychosozialen Faktoren als möglichen Risikofaktoren für die Entstehung von Krebs verbergen.

Abstract. Ten years ago a sample of N = 5.133 male and female subjects (age 28-74) responded to questionnaires including scales for personality, life style, work stress as well as questions on prevalent disease. We now report on the follow-up regarding self-reported incidence of cardiovascular disease and cancer. During a mean follow-up of 10 years, 257 participants had died. Of those alive, N = 4.010 (82%) participated in the follow-up. Of these, 120 and 180 persons reported incident cardiovascular disease and cancer, respectively. The incidence of cardiovascular disease could be significantly predicted by the personality factors "Emotional Lability", "Behavioral Control" and "Type-A-Behavior" as well as by the "Rationality/Antemotionality"-scale according to Grossarth-Maticek. After controlling for age, gender and smoking behavior only the significant effect of "Emotional Lability" remained and the predictors according to Grossarth-Maticek had no incremental validity. Cancer could not be predicted by any personality factors.

This study assessed whether permanent supportive housing (PSH) participation is associated with health service use among a population of adults with disabilities, including people transitioning into PSH from community and institutional settings. Our primary data sources were 2014 to 2018 secondary data from a PSH program in North Carolina linked to Medicaid claims. We used propensity score weighting to estimate the average treatment effect on the treated of PSH participation. All models were stratified by whether individuals were in institutional or community settings prior to PSH. In weighted analyses, among individuals who were institutionalized prior to PSH, PSH participation was associated with greater hospitalizations and emergency department (ED) visits and fewer primary care visits during the follow-up period, compared with similar individuals who largely remained institutionalized. Individuals who entered PSH from community settings did not have significantly different health service use from similar comparison group members during the 12-month follow-up period.

Abstract

Background
Frailty is a dynamic aging-related syndrome, but measuring frailty transitions is challenging. The Faurot frailty index is a validated Medicare claims-based frailty proxy based on demographic and billing information. We evaluated whether three-year changes in the Faurot frailty index were consistent with concurrent changes in the frailty phenotype in a cohort of older adults.


Methods
We used longitudinal data from the National Health and Aging Trends (NHATS) study with Medicare claims linkage (2010-2018). We identified older adults (66+ years) in the 2011 and 2015 NHATS cohorts with at least one year of Medicare fee-for-service continuous enrollment (N=6,951). We described annual changes in mean claims-based frailty for up to three-years, based on concurrent transitions in the frailty phenotype.


Results
At baseline, 32% were robust, 48% prefrail, and 19% frail based on the frailty phenotype. Mean claims-based frailty for older adults who were robust at baseline and worsened to frail increased over three-years (0.09-0.25). Similarly, those who worsened from prefrail to frail experienced an increase in mean claims-based frailty (0.14-0.26). Improvements in the frailty phenotype did not correspond to decreases in claims-based frailty. Older adults whose frailty phenotype improved over time had a lower baseline claims-based frailty score than those who experienced stable or worsening frailty.


Conclusions
Older adults who experienced a frailty phenotype worsening over three years experienced concurrent increases in the Faurot frailty index. Our results suggest that claims data may be used to identify clinically meaningful worsening in frailty.

IntroductionIncreasingly in pharmacoepidemiology, linking is required to enrich analytic data to more accurately define study populations, enable adjustment for confounding, and improve capture of health outcomes. When creating such novel linked datasets, researchers should consider their suitability to meet research objectives, assess source data completeness and population coverage, and ensure well-defined data governance standards and protections exist. Additionally, while the RECORD-PE guidelines assist in the reporting of studies using observational health data specific to pharmacoepidemiology, they do not address the unique requirements for transparent evaluation and reporting of the data linkage process.
Objectives and ApproachWe aimed to 1) provide guidance on data linkage appropriateness and feasibility to plan purposeful and sustainable new linkages that advance pharmacoepidemiological research and 2) generate a checklist with specific recommendations to assist researchers in providing clear and transparent assessment of the linkage process. To develop these guidelines, a working group comprised of members of the International Society of harmacoepidemiology was formed. Recommendations were open for comment by Society members and endorsed by the Society.
ResultsGuidance for feasibility assessment was categorized into five domains: (1) research objectives and justification; (2) data quality and completeness; (3) the linkage process; (4) data ownership and governance; and (5) overall value added by linkage. A checklist for evaluation and reporting of data-linkage processes covered five domains including; (1) data sources; (2) linkage variables; (3) linkage methods; (4) linkage results; and (5) linkage evaluation, including validation and verification of the resulting linked data.
Conclusion/ImplicationsOur guidelines for data linkage feasibility assessment and reporting can be used to inform the design of sustainable linked data resources and for transparent communication of linkage processes. Together, these guidelines will help various stakeholders to critically assess the potential for bias in research based on linked data and help generate actionable evidence.

Abstract

Background
Many older adults with cancer have ≥2 impairments on geriatric assessment, which affects present and future frailty status, treatment tolerability, and outcomes. Our objective was to identify and describe distinct geriatric assessment impairment classes using latent class analysis (LCA) in older patients with gastrointestinal malignancies and assess 1-year mortality.


Methods
We used the Cancer & Aging Resilience Evaluation (CARE) Study, a registry of older adults (≥60 years) at University of Alabama at Birmingham. The analytic cohort included patients with gastrointestinal malignancies who completed a self-administered geriatric assessment (CARE tool) before chemotherapy and had ≥1 geriatric assessment impairment. Thirteen geriatric assessment impairments were used as indicators in LCA. Resultant classes were described, mortality was estimated, and risk contrasts (differences and hazard ratios) were calculated with 95% confidence intervals. For comparison, estimates were provided for frailty categories (robust, prefrail, and frail) determined from 44 items in the CARE tool. Stratified analyses included high-risk (pancreatic, hepatobiliary, and esophageal) versus low-risk gastrointestinal cancers, and stage (IV vs I–III).


Results
Six geriatric assessment impairment classes were identified: Mild impairment (LC1); Social support impairment (LC2); Weight loss alone (LC3); Impaired, low anxiety/depression (LC4); Impaired with anxiety/depression (LC5); and Global impairment (LC6). One-year mortality was 14%, 22%, 29%, 34%, 50%, and 50% for LC1–LC6, respectively. For frailty categories, estimates ranged from 18% (robust) to 40% (frail). In stratified analyses, LC4–LC6 consistently had higher mortality estimates compared to LC1.


Conclusions
The 6 geriatric assessment impairment classes showed a wider spread of mortality estimates compared to frailty categories and could be used to identify vulnerable patients and to plan interventions.
