Cognitive behavior therapy (CBT) is an evidence-based intervention in contemporary clinical health care services. CBT was developed as a psychotherapy to overcome the deficits of psychoanalysis and behaviorism by Aron T. Beck. However, after 63 years, CBT has transformed from its early framework. In comparison to other psychotherapies, cognitive behavior therapy has rapidly changed and developed as an evidence-based clinical intervention. Therefore, this study explored the evolution of cognitive behavior therapy by focusing on its origin, development, limitations, clinical application, third waves, and future directions. Early CBT therapeutic framework has developed as a cognitive model of depression and developed as an intervention to address the negative core belief of individuals and rationalize maladaptive behaviors, thoughts, and emotions. However, clinicians have experienced several limitations and scholars have recognized theoretical limitations in CBT. Later, scholars on CBT have overcome these deficits and developed disorder-specific CBT therapies. Then, clinicians empirically evaluate those therapeutic frameworks and transform CBT into an evidence-based clinical intervention. Further, CBT has strength in integrating third-wave therapies. In the future, CBT will therapeutically further transform with technology, integrating advanced theories of pathology, and clinical demand of the present health care services. Therefore, as a psychotherapeutic intervention, cognitive behavior therapy has transformed from its early framework, and at present, it has developed empirically. Further, in the future, it will transform into a more effective psychotherapeutic intervention.

Upaniṣad literature can be called the most important group of Vedic literary works in Eastern philosophy, and one of its most important Upaniṣads called Kaṭhopaniṣad, is an Upaniṣad book of outstanding value literary and philosophically. This work, which belongs to the Kriṣna Yajurveda, discusses the facts in the form of a dialogue between Nachiketas and Yama. The primary purpose of this research is to investigate to what extent the philosophical importance of the teachings in the Kaṭhopaniṣad, and to what extent the teachings contained in the Kaṭhopaniṣad are similar to other Indian philosophical teachings and whether it covers all the basic study areas of the philosophy. It is expected to conduct a comprehensive study in this work. As this research is mainly used the book research method, it is primarily based on the study of primary and secondary sources. In addition to this, interviews with experts, journal articles and internet sources are used to gather information. Several philosophical methodologies are used to analyze the research problems including descriptive analysis, critical method, comparative method and analytical method. According to the analysis of this research, the final conclusion can be implied that there can be many similarities between Kaṭhopaniṣad and Buddhism not only in terms of the ideas of the teachings of it but also in the way of expression in its technical terms. In addition to this, another point in the conclusion is that philosophically Kaṭhopaniṣad is an extremely important literary work that includes a certain kind of metaphysics, cosmology, epistemology, psychology as well as ethics.

With the increasing population and accelerated urbanization, demands for water are rising for different sectors around the world, including in South Asia. Integrated water resource management (IWRM) offers a promising potential to address multifaceted water demands. This study therefore aimed to address this issue by (i) reviewing key issues related to water, land, and food in South Asian countries, (ii) exploring the prevalent irrigation management strategies in those countries, and (iii) examining the IWRM situation based on a Nepalese case study, and it proposes some options to support effective implementation of IWRM. South Asia, the home to 24% of the world's population with only 15% and 7% of the world's arable and permanent crop land and water resources, respectively, is the worst-affected region in the world from undernourishment. Surface irrigation is the dominant irrigation application method in the region, which incurs high water losses due to the lack of flexible water control structures in canal networks. The Nepalese case study revealed a lack of clear institutional arrangements to implement IWRM and disparate and conflicting views about IWRM. Creation and strengthening of basin-level water user organizations, technological improvements, and awareness-raising activities are some potential ways forward to implement IWRM.

Introduction The UK government stockpiles co-amoxiclav to treat bacterial complications during influenza pandemics. This pragmatic trial examines whether early co-amoxiclav use reduces re-consultation due to clinical deterioration in 'at risk' children presenting with influenza-like illness (ILI) in primary or ambulatory care. Methods 'At risk' children aged 6 months to 12 years presenting within five days of ILI onset were randomly assigned to oral co-amoxiclav 400/57 or placebo twice daily for five days (dosing based on age +/- weight). 'At risk' groups included children with respiratory, cardiac, and neurological conditions. Randomisation was stratified by region and used a non-deterministic minimisation algorithm to balance age and current seasonal influenza vaccination status. Our target sample size was 650 children, which would have allowed us to detect a reduction in the proportion of children re-consulting due to clinical deterioration from 40% to 26% with 90% power and 5% two-tailed alpha error, including allowance for 25% loss to follow-up and an inflation factor of 1.041. Participants, caregivers and investigators were blinded to treatment allocation. Intention-to-treat analysis included all randomised participants with primary outcome data on re-consultation due to clinical deterioration within 28 days. Safety analysis included all randomised participants. Trial registration: ISRCTN 70714783. EudraCT 2013-002822-21. Results We recruited 271 children between 11 February 2015 and 20 April 2018. Primary outcome data were available for 265 children. Only 61/265 children (23.0%) re-consulted due to clinical deterioration. No evidence of a treatment effect was observed for re-consultation due to clinical deterioration (co-amoxiclav 33/133 (24.8%), placebo 28/132 (21.2%), adjusted risk ratio [RR] 1.16, 95% confidence interval [CI] 0.75 to 1.80). Background: During the COVID-19 pandemic in 2020, the UK government began a mass SARS-CoV-2 testing programme. This study aimed to determine the feasibility and acceptability of organised regular self-testing for SARS-CoV-2. Methods: This was a mixed methods observational cohort study in asymptomatic students and staff at University of Oxford, who performed SARS-CoV-2 antigen lateral flow self-testing. Data on uptake and adherence, acceptability, and test interpretation were collected via a smartphone app, an online survey, and qualitative interviews. Findings: Across three main sites, 551 participants (25% of those invited) performed 2728 tests during a follow-up of 5.6 weeks. 447 participants (81%) completed at least two, and 340 (62%) completed at least four tests. The survey, completed by 214 participants (39%), found that 98% of people were confident to self-test and believed self-testing to be beneficial. Acceptability of self-testing was high, with 91% of ratings being acceptable or very acceptable. 2711 (99.4%) test results were negative, nine were positive and eight were inconclusive. Results from eighteen qualitative interviews with students and staff revealed that participants valued regular testing, but there were concerns about test accuracy that impacted uptake and adherence. Interpretation: This is the first study to assess feasibility and acceptability of regular SARS-CoV-2 self-testing. It provides evidence to inform recruitment, adherence to, and acceptability of regular SARS-CoV-2 self-testing programmes for asymptomatic individuals using lateral flow tests. We found that self-testing is acceptable and people were able to interpret results accurately.
