Owing to the low % of returns & relatively incomplete responses mailed questionnaires are rarely used in survey res. But the success experienced in obtaining 100% returns of mailed questionnaires requiring from 1-1.5 hours & from 20-30 minutes to complete suggest that the researcher often may eliminate this difficulty & capitalize on their economy & practicality. Though R's in this case were the enrollment & executive directors of 85 Blue Cross Plans in the US, Canada, & Puerto Rico, the suggestions also may apply to more diverse groups of R's. The process of development & use of a mailed questionnaire may be divided into 3 phases: (1) R preparation & involvement in which a relationship is developed between the investigator & the R's & they become increasingly involved & committed to the study; (2) questionnaire design & construction in which case studies may be used to suggest relevant variables & careful attention is given to questionnaire appearance, wording & logical arrangement of questions, ease of response, & questionnaire pretesting; &, (3) follow-up procedures in which the use of a personal letter may increase the feeling of obligation to respond & the use of air & special delivery mails may impress R's with the importance of the study. In the 2nd or 3rd follow-up a telephone call or telegraph message may help to obtain the returns of reluctant R's. C. M. Coughenour.
