Despite significant improvements in many health outcomes over the past sixty years, many chronic problems in Indonesia's health system including financial sustainability, governance and inequities in accessing health care have long been apparent, even before the COVID-19 pandemic. The epidemiological transition associated with demographic and socioeconomic change in recent decades makes Indonesia one of many countries that still struggle to address the issues of communicable, maternal and nutritional diseases while facing an increasing burden of non-communicable diseases. The contributors to In Sickness and In Health: Diagnosing Indonesia investigate challenges and opportunities facing the Indonesian health system and assess hurdles that Indonesians have to navigate in their quest to achieve a longer and better quality of life. Politics shaping recent health policy reforms in Indonesia, barriers to the supply of specialist doctors and quality medicines, availability of accurate health and population data, and the financial toll of the COVID-19 pandemic are among the topics discussed in this book. Accessing essential health services for mothers and children and for those living with disability, discrimination and mental illness, as well as an innovative trial to control dengue, are also examined.

Do local institutions influence the nature of political clientelist exchange? We find a positive answer in the context of a village institution prevalent in Java since the Dutch colonial rule, where democratically elected village heads receive usufruct rights over a piece of communal village land (bengkok land) as a compensation for their service in lieu of salary. To formulate how limited-term private ownership of bengkok land promotes clientelism, we model a timely delegation of agricultural tenancy contracts to villagers-cum-voters as an incumbent re-election strategy. Based on a household survey fielded in 2018 across 130 villages in Java, Indonesia, we find that the chances of a bengkok plot being rented out increase by 6 percentage points as the time of the next election becomes closer by one year, and sharecropping is preferred to a fixed-rental contract as the election approaches. The empirical results are statistically significant and remain largely unchanged against a series of robustness checks. We also find suggestive evidence of short-term efficiency loss from clientelist politics over bengkok land.

Tanah bengkok (bengkok land) in Java, Indonesia boasts a unique institution where elected village leaders receive usufruct rights to a parcel of land owned by the village, in lieu of salary. Despite its relevance to the political economy of land distribution in Java, unavailability of systematic data has so far constrained in-depth empirical research on bengkok land. In 2018, we conducted a survey covering 130 villages and more than 1,800 households in Java. We found substantial heterogeneity in the incidence and use patterns of bengkok land across villages. Fixed rental tenancy appeared more prevalent than sharecropping on bengkok land and bengkok landlords seldom got involved in tenants' farming decisions, which made bengkok land management look more 'business-like'. Finally, evidence is consistent with political cycles as the village heads with reelection motives offered sharecropping contracts to non-relatives to garner a larger pool of supporters.
