Background. The military conflict that takes place in the Eastern Ukraine, a significant increase in the number of terrorist acts pose a large number of questions to surgeons on the organization and provision of medical care to the victims of firearms and mine explosive injuries at the medical care stages. VAC (vacuum-assisted closure) therapy is a modern and up-to-date wound treatment technique that significantly improves the course of all stages of the wound process: reduces local edema, improves local blood circulation, reduces microbial contamination, leads to a relatively rapid reduction of the wound cavity decreasing the duration of healing. The purpose of our study was to determine the clinical efficacy of VAC-therapy in the treatment of patients with gunshot injuries due to anti-terrorist operation in the East of Ukraine. Materials and methods. To conduct our study, we analyzed the data of 74 injured people with combat trauma who were treated at the Military Medical Clinical Center of the Central Region in 2015–2016. All the victims participated in our study were males. Their average age was 41.1 ± 2.2 years. The total array of research was divided into 2 groups. The main group included 45 patients with gunshot wounds and purulent complications, in the comprehensive treatment of which VAC-therapy was used. Control group consisted of 29 injured people with gunshot injuries and purulent complications, in the comprehensive treatment of which we didn't use VAC-therapy. Results. The principle of the VAC-therapy method is based on the use of a closed drainage system that maintains controlled negative pressure in the area of the wound. The criterion for the satisfactory outcome of wound treatment in patients with gunshot wounds was the purification of purulent wounds, the beginning of the granulation growth and transition to the second phase of the wound process. In our study, we determined the time of obtaining satisfactory result in victims with local infectious complications of gunshot wounds. In the main array, the patients prevailed whose positive result of treatment for purulent complications of gunshot wounds was noted on day 4–6. In contrast to the main group, in controls the positive result of the wound process was most often observed on day 10–12 of treatment. The above data clearly indicate a significant acceleration of the satisfactory result of treatment using VAC-therapy, and the technique itself is an auxiliary method for controlling the infection in patients with purulent complications of gunshot wounds. In all patients treated by us, the application of this method of treatment allowed further closure of the wound defect by superimposing secondary surgical sutures or using autodermoplasty with positive effects. Conclusions. Use of VAC-therapy is possible in patients with local infectious complications of gunshot wounds. Application of VAC-therapy greatly improves treatment outcomes in patients with gunshot wounds complicated by purulent infection. When using VAC-therapy, the second phase of the wound process occurs on day 4–6, which is twice as fast compared with integrated therapy without its applications, and allows the recommendation of VAC-therapy for clinical use. ; VAC-therapy — современная новейшая методика лечения ран, которая значительно улучшает течение всех стадий раневого процесса: уменьшает локальный отек, способствует усилению местного кровообращения, снижает уровень микробного обсеменения, приводит к относительно быстрому уменьшению раневой полости, сокращая срок заживления. Целью нашего исследования было определить клиническую эффективность использования VAC-therapy в лечении пострадавших с огнестрельными ранениями. Для выполнения нашего исследования мы проанализировали лечение 74 пострадавших с огнестрельными ранениями, полученными в результате боевых действий, которые лечились в Военно-медицинском клиническом центре Центрального региона в период 2015–2016 годов. Проведенное исследование указало, что использование VAC-therapy возможно у пострадавших с местными инфекционными осложнениями огнестрельных ранений, значительно улучшает результаты лечения раневого процесса у пострадавших с огнестрельными ранениями, осложненными гнойной инфекцией, что позволяет рекомендовать VAC-therapy для клинического использования. ; VAC-therapy — сучасна новітня методика лікування ран, що значно поліпшує перебіг усіх стадій ранового процесу: зменшує локальний набряк, сприяє посиленню місцевого кровообігу, знижує рівень мікробного обсіменіння, призводить до відносно швидкого зменшення ранової порожнини, скорочуючи термін загоєння. Ціллю нашого дослідження було визначити клінічну ефективність використання VAC-therapy у лікуванні постраждалих із вогнепальними пораненнями. Для виконання нашого дослідження ми проаналізували лікування 74 постраждалих із вогнепальними пораненнями, що були отримані в результаті бойових дій, які лікувались у Військово-медичному клінічному центрі Центрального регіону в період 2015–2016 років. Проведене дослідження показало, що використання VAC-therapy є можливим у постраждалих із місцевими інфекційними ускладненнями вогнепальних поранень, значно покращує результати лікування ранового процесу у постраждалих із вогнепальними пораненнями, ускладненими гнійною інфекцією, що дозволяє рекомендувати VAC-therapy у клінічне використання.
