This article examines the illness and recovery experiences and perceptions of physically active middle aged and older adults participating in hiking groups. These perceptions are examined within the local milieu of their group and the larger social context of biomedical norms of healthy older bodies. Discourse on the body was viewed through the lens of medical anthropology and data were analyzed using embodied ethnography. There were 15 participants (53 percent female) and all were of European descent. The hiking group provided participants with meaningful spaces and places where they could explore all aspects of their health with the support of others who had undergone similar life experiences. The physical activities they engaged in as a group were therapeutic and transformational for several members. Their group activities created a deep sense of community and aided in their healing processes. Holistic health programs such as hiking groups could provide an alternative or ancillary treatment options. However, cost, location, opportunities for socialization, and the physical abilities of potential participants should be seriously considered before adopting a hiking program for this demographic.

BACKGROUND: Beliefs about causes and responsibility for chronic diseases can affect personal behaviour and support for healthy policies. In this research we examined relationships between socio-demographics (sex, age, education, employment, political alignment, perceived health, household income, household size) and perceptions of causes and responsibility for health behaviour, chronic disease correlates, and attitudes about cancer prevention and causes. METHODS: Using data from the 2016 Chronic Disease Prevention survey in which participants (N = 1200) from Alberta, Canada responded to items regarding how much they believed personal health behaviours, prevention beliefs, and environmental factors (i.e., healthy eating, physical activity, alcohol, smoking, and where a person lives or works) are linked to getting cancer. Participants also responded to questions about causes and responsibility for obesity, alcohol, and tobacco (i.e., individual or societal). Relationships were examined using multinomial logistic regression on socio-demographics and survey items of interest. RESULTS: Men (compared to women) were less likely to link regular exercise, or drinking excessive alcohol, to reducing or increasing cancer risk. Similarly, men were less likely to link environmental factors to cancer risk, and more likely to agree that cancer was not preventable, and that treatment is more important than prevention. Finally, men were more likely to believe that alcohol problems are an individual's fault. Left and central voters were more likely to believe that society was responsible for addressing alcohol, tobacco, and obesity problems compared to right voters. Those with less than post-secondary education were less likely to believe that regular exercise, maintaining a healthy body weight, or eating sufficient fruits and vegetables were linked to cancer - or that society should address obesity - compared to those with more education. Purpose.To identify perceptions of how sociocultural environment enabled and hindered physical activity (PA) participation.Design.Community-based participatory research.Setting.Two semirural and two urban communities located in Alberta, Canada.Participants.Thirty-five people (74.3% females, 71.4% aged 25–64 years) across the four communities.Method.PhotoVoice activities occurred over 3 months during the spring of 2009. Participants were asked to document perceived environmental attributes that might foster or inhibit PA in their community. Photographs and narratives were shared in one-on-one interviews. Line-by-line coding of the transcripts was independently conducted by two researchers using an inductive approach. Codes were arranged into themes and subthemes, which were then organized into the Analysis Grid for Environments Linked to Obesity (ANGELO) framework.Results.Six main themes (accompanied by subthemes) emerged: sociocultural aesthetics, safety, social involvement, PA motivation, cultural ideas of recreation, and car culture. Representative quotes and photographs illustrate enablers and obstacles identified by participants.Conclusion.This PhotoVoice study revealed how aspects of participants' sociocultural environments shaped their decisions to be physically active. Providing more PA resources is only one step in the promotion of supportive environments. Strategies should also account for the beautification and maintenance of communities, increasing feelings of safety, enhancement of social support among community members, popularization of PA, and mitigating car culture, among others.

Walkability is a critical component of built environments, yet there is still diverse conceptualization and measurement of the construct. The Walk Score® metric is one measure of walkability, which is widely used in scholarly, industry, and public domains. With increased interest in the use of Walk Score® as a research tool, it is necessary to examine the operationalization and scope of the measure. This scoping review examined how researchers utilized Walk Score® in walkability research, with specific attention to identifying limitations related to health outcomes as well as the use of the metric in non-health research contexts. Findings from the review provide a novel and critically nuanced understanding of how the assumptions and limitations of Walk Score® are addressed relative to socio-ecological aspects of walkability.

In April 2011, a conference with invited experts from research, policy and practice was held to build consensus around policy levers to address environmental determinants of obesity. The gap between existing policy tools and what can promote health through community design is a major policy opportunity. This commentary represents a consensus of next actions towards creating built environments that support healthy active living. The policy environment and Canadian evidence are reviewed. Issues and challenges to policy change are discussed. Recommendations to create supportive built environments that encourage healthy active living in communities include the following: 1) empower planning authorities to change bylaws that impede healthy active living, protect and increase access to green space, introduce zoning to increase high density, mixed land use, and influence the location and distribution of food stores; 2) establish stable funding for infrastructure promoting active transportation and opportunities for recreation; 3) evaluate the effectiveness of programs to improve the built environment so that successful interventions can be identified and disseminated; 4) mandate health impact assessment of planning, development and transportation policies to ensure that legislative changes promote health and safety; 5) frame issues to dispel myths and to promote protection from obesity risk factors.

Children are exposed to food environments that make nutrient-poor, energy-dense food cheap, readily available and heavily marketed; all conditions with potential negative impacts on diet and health. While the need for programmes and policies that improve the status of food environments is clear, greater public support is needed for governments to act. The purpose of this qualitative collective case study was to examine if community engagement in the Local Environment Action on Food (LEAF) project, a community-based food environment intervention in Alberta, Canada, could build public support and create action to promote healthy food environments. Semi-structured interviews with a purposeful sample of 26 stakeholders from 7 communities explored LEAF's impact and stakeholder experiences creating change. Data collection and analysis were iterative, following Charmaz's constant comparative analysis strategy. Participants reported environmental and community impacts from LEAF. Notably, LEAF created a context-specific tool, a Mini Nutrition Report Card, that communities used to promote and support food environment action. Further, analysis outlined perceived barriers and facilitators to creating community-level food environment action, including level of engagement in LEAF, perceived controllability, community priorities, policy enforcement and resources. Findings from this study suggest that community-based interventions, such as LEAF, can help build community capacity and reduce existing barriers to community-level food environment action. Thus, they can provide an effective method to build public awareness, demand and action for healthier food environments.

BACKGROUND: Point of Purchase (PoP) promotional and advertising activities are a sophisticated tobacco marketing strategy. This study describes tobacco PoP activities in school neighbourhoods and compares PoP activities in retail stores between schools with high and low smoking prevalence. METHODS: A cross-sectional study was conducted in 81 randomly selected schools across five provinces. Students in grades 10–11 completed a questionnaire on smoking. Observations were made in all retail stores located within a one-kilometre radius around the school. ANOVA tests were used to detect differences on PoP variables between high (>20.6%) and low (≤20.6%) smoking prevalence schools, defined as percentage of students reporting at least a few puffs on >2 days in the last 30 days. RESULTS: Approximately half of retail stores in each school neighbourhood exhibited tobacco PoP activities. Average school smoking prevalence was 20.99%. There were significant main effects on PoP variables between schools with high and low smoking prevalence, Wilk's λ=0.81, F (6,74)=2.89, p<0.01, η(2)=0.19. Stores near schools with high smoking prevalence had significantly lower prices per cigarette (F (1,79)=15.34, p<0.01, η(2)=0.16), more in-store promotions (F (1,79)=6.73, p<0.01, η(2) =0.08), and fewer government-sponsored health warnings (F (1,79)= 6.26, p<0.01, η(2)=0.07) compared to schools with low smoking prevalence. CONCLUSION: Higher levels of PoP activities in stores located in the school neighbourhood are related to school smoking prevalence. Schools with low smoking prevalence had more stores that posted government health warning signs and higher cigarette prices. Legislation regulating PoP activities and health warnings in school neighbourhoods should be considered.

Greater availability of low nutritional quality foods and decreased consumption of nutrient-dense foods have negatively impacted the nutrient profile of the Canadian diet. Poor diet is now the leading risk factor for chronic disease and premature death in Canada. To help consumers choose healthful foods, nutrition labelling is one policy tool for communicating relevant nutrition information. However, there are notable shortcomings with current nutrition labelling systems, which make it difficult for Canadians to navigate the complex food environment. Government action on nutrition labelling systems, including front-of-package (FOP), shelf, and menu labelling, is required. In May 2016, we hosted a consensus conference with experts from research, policy and practice to review available evidence, share experiences and come to consensus regarding the next best steps for action on nutrition labelling in Canada. In this paper, we examine the evidence, opportunities and challenges surrounding FOP, shelf, and menu labelling. We outline recommendations, emphasizing FOP, shelf, and menu labelling as part of a standardized, coordinated and multi-pronged strategy supported by a robust, evidence-based nutrition profiling system. Recommendations for monitoring adherence to regulations and participation of stakeholders to avoid conflict of interest in policy development, implementation and evaluation are included. Within a comprehensive strategy, these recommendations can help to improve the nutrition information environment for Canadians.
