Sixteen states have adopted school report card accountability systems that assign A-F letter grades to schools. Other states are now engaged in deliberation about whether they, too, should adopt such systems. This paper examines A-F accountability systems with respect to three kinds of validity. First, it examines whether or not these accountability systems are valid as a measure, that is, do these systems validly measure school quality? Second, it examines whether or not they are valid as a policy instrument. or, how far do A-F accountability systems fulfill the stated aims of their proponents—empowering parents, providing "simple" and "common sense" measures of educational quality, and so on? Finally, it examines whether or not A-F systems are valid as a democratic framework:, how well do these systems align with the broader goals of educating students for democratic citizenship and of incorporating parents and community members in democratic deliberation about policies for their public schools? The paper concludes that A-F accountability systems are invalid along each of these lines, and provides recommendations for democratically developing and implementing criteria for school assessment. ; Dieciséis estados han adoptado sistemas de rendición de cuentas de tarjetas de calificaciones escolares que asignan calificaciones de letra A-F a las escuelas. ABSTRACTObjectivesTo evaluate how the size and characteristics of potential palliative care populations vary depending on which administrative data sources are used to identify them and how inclusion criteria are applied.ApproachPopulation-based studies in palliative care have used a range of administrative databases including death certificate data and linked hospital admissions data to identify people who received or could have benefited from palliative care. We conducted a retrospective cohort study using linked hospital admissions and mortality data from the Western Australian Data Linkage System to evaluate how the size and distribution of the potential palliative care populations varied according to the data sources, whether linked data were used and how inclusion criteria were applied. Our cohort consisted of 23,852 people aged 20 years and over who died in Western Australia between 1 January 2009 and 31 December 2010 after excluding those whose deaths related to pregnancy or trauma. Within this cohort, the number, proportion and characteristics of people who died from one or more of 10 medical conditions considered amenable to palliative care were identified using different information sources.ResultsOf the 23,852 people who died, a total of 10,445 (43%) people had a condition potentially amenable to palliative care recorded as the underlying cause of death on their death certificate. This increased to 15,064 (63%) people when including one of these conditions listed anywhere on their death certificate. When hospital admission records from the last year of life and death records were used to identify people who might potentially have benefited from palliative care, there were 17,384 (73%) people identified with at least one of these conditions. The median age at death varied by up to 12 years between data sources depending on condition. The sex distribution of the decedents, which was measured in terms of the proportion of males dying from the condition, showed variations of up to 30% depending on the data source.ConclusionsThe use of different data sources to identify conditions that might benefit from palliative care will result in differing frequency of conditions and age and sex distribution of the population. This is a result of many people having multiple conditions and the method in which these are recorded in different data sources. It is important to be aware of the characteristics of the individual data sources when the data are used for health service planning.

Risk of a major adverse cardiovascular event (MACE) following first-ever hospitalisation for acute gout: a Western Australian population-level linked data study

ObjectivesCardiovascular disease is the largest contributor of increased mortality in patients with gout. Acute inflammation as seen with gout attacks may have a mechanistic role in developing Major Adverse Cardiovascular Events (MACE). We examined the temporal relationship between admission to hospital with acute gout and MACE.
ApproachLinked inpatient and mortality data from the Western Australian Rheumatic Disease Epidemiology Registry were used. We identified patients with an incident acute gout (index) hospitalisation and admission or death records due to MACE (composite of acute coronary syndrome, stroke, heart failure, cardiovascular death). The risk of MACE during the index post-discharge period (1-30 days after index admission) and control period (365 days prior to index admission and 365 days post-discharge) was determined using a self-controlled case-series (SCCS) design. Conditional fixed-effects Poisson regression was used to obtain incidence rate ratios (IRR). Sensitivity analyses were performed excluding deaths and 180-day events.
ResultsWe identified 962 patients (mean age=76.2 years [SD=12.2]; 66.8% male) with incident acute gout admission and documented MACE during the control and/or index post-discharge periods. 917 (95.3%) patients experienced MACE during the control period and 114 (11.9%) during the index post-discharge period. The rate of MACE during the control and post-discharge periods were 0.84 and 1.44 events per person-year, respectively, with an IRR=1.67 (95% CI: 1.38-2.02) for the post-discharge period compared with the control period from regression analysis. Sensitivity analyses excluding deaths and 180-day events were IRR=1.68 (95% CI=1.29-2.20) and IRR=1.66 (95% CI=1.34-2.07) respectively.
ConclusionOur self-controlled case-series study using linked administrative data found an increased risk of MACE during the 30 days after discharge for index gout hospitalisation. This suggests a temporal association between acute inflammation and MACE.
