Background: Reducing inequities in access to healthcare is one of the most important goals for all health systems. Financial barriers play a fundamental role here. People with disability (PWD) experience further financial barriers in access to their needed healthcare services. This study aimed to explore the causes of barriers in access to health services for PWD in Tehran, Iran. Methods: In this qualitative study, we used semi-structured in-depth interviews to collect data and selected participants through purposeful sampling with maximum variation. We conducted 56 individual interviews with people with disability, healthcare providers and policymakers from Sep 2015 until May 2016, at different locations in Tehran, Iran. Results: We identified four categories and eight subcategories of financial barriers affecting access to healthcare services among PWD. Four categories were related to health insurance (i.e. lack of insurance coverage for services like dentistry, occupational therapy and speech therapy), affordability (low income for PWD and their family), financial supports (e.g. low levels of pensions for people with disabilities) and transportation costs (high cost of transportation to reach healthcare facilities for PWD). Conclusion: Financial problems can lead to poor access to health care services. To achieve universal health coverage, government should reduce health insurance barriers and increase job opportunities and sufficient financial support for PWD. © 2019, Iranian Journal of Public Health. Objectives: In most countries around the world, sex work is an illegal activity. Female sex workers (FSWs) in Iran hide their identities, and they are known to be a hard-to-reach population. Despite free access to HIV testing, fewer than half of FSWs receive HIV testing. The purpose of this study was to characterize the reasons for which FSWs do not seek testing at drop-in centers (DICs) and voluntary counseling and testing (VCT) centers in Iran. Methods: A qualitative study was conducted in 2016. The participants were 24 FSWs who received services at VCT centers and DICs for vulnerable females in the north of Iran and 9 males who were the clients of FSWs. In this study, we made use of purposive sampling and carried out a thematic analysis. Results: We found 4 major and 6 minor themes. The major themes were: fear of being infected (with HIV), stigma, indifference, and knowledge. Conclusions: Despite the significant efforts made by the government of Iran to establish and expand DICs for vulnerable females, the number of FSWs receiving services at these centers has not been very considerable. Consequently, by introducing and implementing training programs for peer groups, it may be possible to take steps toward establishing strategic programs for the control and prevention of HIV/AIDS. Background: Number of Iranian articles published in ISI journals has increased significantly in recent years. Despite the quantitative progress, studies performed in Iran represent low collaboration in research; therefore, we decided to evaluate collaboration in Golestan University of Medical Sciences (GOUMS) research projects. Methods: In this cross-sectional study, all GOUMS research projects that had got grants from the university between 2005-2007 were studied. Among 107 research projects included in our study, 102 projects were evaluated and checklists were completed. The researcher's questionnaire was sent to the principle investigators (n= 46) of the projects and eventually 40 questionnaires were collected. Results: The review of 102 research proposals shows that 10 projects (9.8) have been performed in collaboration with other organizations. Scientific outputs in these projects have been more than projects which were confined to the university (98 compare to 68; p= 0.04). The total cost of the projects under study was a little more than 300,000 US. In just 12 projects (11.8) a part of the cost had been provided by organizations outside the university. About 50 of researchers declared that they had chosen their research topic based on their "personal interest". Only 1 project was performed by the demand of nongovernmental organizations and 12 researchers reported no collaboration in their activities. Conclusion: This study shows that collaboration in GOUMS research projects is low. Moreover, collaborations with governmental and nongovernmental organizations are trivial. The scientific outputs in collaborative research projects are much more than other projects.

Being the second-largest country in the Middle East, Iran has a long history of civilisation during which several dynasties have been overthrown and established and health-related structures have been reorganised. Iran has had the replacement of traditional practices with modern medical treatments, emergence of multiple pioneer scientists and physicians with great contributions to the advancement of science, environmental and ecological changes in addition to large-scale natural disasters, epidemics of multiple communicable diseases, and the shift towards non-communicable diseases in recent decades. Given the lessons learnt from political instabilities in the past centuries and the approaches undertaken to overcome health challenges at the time, Iran has emerged as it is today. Iran is now a country with a population exceeding 80 million, mainly inhabiting urban regions, and has an increasing burden of non-communicable diseases, including cardiovascular diseases, hypertension, diabetes, malignancies, mental disorders, substance abuse, and road injuries. © 2019 Elsevier Ltd
