Jacobs argued that grand planning schemes intending to redevelop large swaths of a city according to a central theoretical framework fail because planners do not understand that healthy cities are organic, spontaneous, messy, complex systems that result from evolutionary processes. She argued that a gradual pace of redevelopment would facilitate maintenance of existing interpersonal ties. This paper operationalises the concept of pace of development within a cross-sectional framework as the 'age diversity of housing'. Analysis of a population-based multilevel community survey of Chicago linked with census housing data predicts individual perceptions of neighbourhood social relations (cohesion, control, intergenerational closure and reciprocal exchange). A gradual pace of redevelopment resulting in historical diversity of housing significantly predicts social relations, lending support to Jacobs's claims.

Lifestyle and informal sports have been recognised by policy makers as offering opportunities to increase participation in physical activity, particularly amongst hard to reach groups. Lifestyle sports are, however, double edged in their potential to achieve these goals. Their playful and non-traditional features may attract new participants less interested in traditional sports but the very liquidity of these activities may mean that the engagement of participants is fragmented and not sustained beyond a particular period in their lives. This article presents the perspective of mountain biking user-managers; those involved in the delivery, clubs and communities of mountain bikers across the United Kingdom. Findings suggest that whilst lifestyle sport communities are dependent on the work of formalised clubs to gain access to the funding and resources they need to sustain their activities, core participants will not always want to have to liaise or become involved formally within a club structure. In addition, clubs will not succeed in delivering sustained activities in line with sport policy to increase and maintain participation by relying on individual grants and without the support of an active informal user community. Young people have been identified as a key target group for whom participation in sport and physical activity could have important benefits to health and well-being and consequently have been the focus of several government policies to increase participation in the UK. Lifestyle sports represent one such strategy for encouraging and sustaining new engagements in sport and physical activity in youth groups, however, there is at present a lack of understanding of the use of these activities within policy contexts. This paper presents findings from a government initiative which sought to increase participation in sport for young people through provision of facilities for mountain biking in a forest in south-east England. Findings from qualitative research with 40 young people who participated in mountain biking at the case study location highlight the importance of non-traditional sports as a means to experience the natural environments through forms of consumption which are healthy, active and appeal to their identities. In addition, however, the paper raises questions over the accessibility of schemes for some individuals and social groups, and the ability to incorporate sports which are inherently participant-led into state-managed schemes. AbstractMost of what is known about the association between children's executive function (EF) and school readiness skills is derived from research conducted in Western countries. We tested whether these associations were evident in a middle‐income country context. Participants were 1,480 children, aged 4–7 years old, who participated in an endline assessment of the Tayari program, an early childhood education (ECE) model that is being delivered by the Kenyan education system. High rates of task completion, low rates of floor effects, and high rates of assessor quality ratings supported the feasibility of large‐scale direct assessments of EF with young children. Assessor ratings of children's attention‐related behaviors during testing were positively associated with their performance on EF tasks (rs = 0.12–0.27). An EF composite score was not related to demographic factors or to children's exposure to the Tayari program. However, the EF composite score was uniquely associated with performance‐based measures of early literacy (β = 0.18, 95% confidence interval [CI] = 0.05, 0.31), early numeracy (β = 0.16, 95% CI = 0.07, 26), and social‐emotional competencies (β = 0.12, 95% CI = 0.03, 0.20), even after adjustment for multiple covariates. These results are discussed with respect to the ways in which EF skills inform ongoing efforts to invest in ECE in low‐ and middle‐income countries.

AbstractIntroductionApproval of the first long‐acting injectable antiretroviral therapy (LAI ART) medication heralded a new era of HIV treatment. However, the years since approval have been marked by implementation challenges. The "Accelerating Implementation of Multilevel Strategies to Advance Long‐Acting Injectable for Underserved Populations (ALAI UP Project)" aims to accelerate the systematic and equitable delivery of LAI ART.MethodsWe coded and analysed implementation barriers according to the Consolidated Framework for Implementation Research (CFIR) domains, desired resources and programme goals from questionnaire short‐answer responses by clinics across the United States responding to ALAI UP's solicitation to participate in the project between November 2022 and January 2023.ResultsThirty‐eight clinics responded to ALAI UP's solicitation. The characteristics of LAI ART as an innovation (cost, complexity of procurement, dosing interval, limited eligibility) precipitated and interacted with barriers in other CFIR domains. Barriers included obtaining coverage for the cost of medication (27/38 clinics) (outer setting); need for new workflows and staffing (12/38) and/or systems to support injection scheduling/coordination (16/38), transportation and expanded clinic hours (13/38) (inner setting); and patient (10/38) and provider (7/38) education (individuals). To support implementation, applicants sought: technical assistance to develop protocols and workflows (18/38), specifically strategies to address payor challenges (8/38); additional staff for care coordination and benefits navigation (17/38); opportunities to share experiences with other implementing clinics (12/38); patient‐facing materials to educate and increase demand (7/38); and support engaging communities (6/38). Clinics' LAI ART programme goals varied. Most prioritized delivering LAI ART to their most marginalized patients struggling to achieve viral suppression on oral therapy, despite awareness that current US Food and Drug Administration approval is only for virally suppressed patients. The goal for LAI ART reach after 1 year of implementation ranged from ≤10% of patients with HIV on LAI ART (17/38) to ≥50% of patients (2/38).ConclusionsDiverse clinic types are interested in offering LAI ART and most aspire to use LAI ART to support their most vulnerable patients sustain viral suppression. Dedicated resources centred on equity and relevant to context and population are needed to support implementation. Otherwise, the introduction of LAI ART risks exacerbating, not ameliorating, health disparities.
