Background: On 23rd March 2020 the UK government released self-isolation guidance to reduce the risk of transmission of SARS-Cov-2. The influence such guidance has on sexual activity is not known. Aim: To investigate levels and correlates of sexual activity during COVID-19 self-isolation in a sample of the UK public. Methods: This paper presents pre-planned interim analyses of data from a cross-sectional epidemiological study, administered through an online survey. Outcomes: Sexual activity was measured using the following question: "On average after self-isolating how many times have you engaged in sexual activity weekly?" Demographic and clinical data was collected, including sex, age, marital status, employment, annual household income, region, current smoking status, current alcohol consumption, number of chronic physical conditions, number of chronic psychiatric conditions, any physical symptom experienced during self-isolation, and number of days of self-isolation. The association between several factors (independent variables) and sexual activity (dependent variable) was studied using a multivariable logistic regression model. Results: 868 individuals were included in this study. There were 63.1% of women, and 21.8% of adults who were aged between 25 and 34 years. During self-isolation, 39.9% of the population reported engaging in sexual activity at least once per week. Variables significantly associated with sexual activity (dependent variable) were being male, a younger age, being married or in a domestic partnership, consuming alcohol, and a higher number of days of self-isolation/social distancing. Clinical Implications: In this sample of 868 UK adults self-isolating owing to the COVID-19 pandemic the prevalence of sexual activity was lower than 40%. Those reporting particularly low levels of sexual activity included females, older adults, those not married, and those who abstain from alcohol consumption. Strength and Limitations: This is the first study to investigate sexual activity during the UK COVID-19 self-isolation/social distancing. Participants were asked to self-report their sexual activity potentially introducing self-reporting bias into the findings. Second, analyses were cross-sectional and thus it is not possible to determine trajectories of sexual activity during the current pandemic. Conclusion: Interventions to promote health and wellbeing during the COVID-19 pandemic should consider positive sexual health messages in mitigating the detrimental health consequences in relation to self-isolation and should target those with the lowest levels of sexual activity.

Purpose: We investigated the association between food insecurity and fall-related injury among older adults from six low- and middle-income countries (LMICs), and the extent to which this association is mediated by mental health. Methods: Cross-sectional, community-based, nationally representative data from the WHO Study on global AGEing and adult health (SAGE) were analyzed. Past 12-month food insecurity was assessed with two questions on frequency of eating less and hunger due to lack of food. Fall-related injury referred to those that occurred in the past 12 months. Multivariable logistic regression analysis and mediation analysis were conducted to assess associations. Results: Data on 14,585 adults aged ≥65 years [mean (SD) age 72.5 (11.5) years; 54.9% females] were analyzed. After adjustment for potential confounders, severe food insecurity (versus no food insecurity) was associated with 1.95 (95%CI = 1.11–3.41) times higher odds for fall-related injury. Moderate food insecurity was not significantly associated with fall-related injury (OR = 1.34; 95%CI = 0.81–2.25). The mediation analysis showed that 37.3%, 21.8%, 17.7%, and 14.0% of the association between severe food insecurity and fall-related injury was explained by anxiety, sleep problems, depression, and cognition, respectively. Conclusion: Severe food insecurity was associated with higher odds for injurious falls among older adults in LMICs, and a large proportion of this association may be explained by mental health complications. Interventions to improve mental health among those who are food insecure and a strong focus on societal and government efforts to reduce food insecurity may contribute to a decrease in injurious falls.

Food insecurity has been shown to be associated with fast-food consumption. However, to date, studies on this specific topic are scarce. Therefore, the aim of the present study was to investigate the association between food insecurity and fast-food consumption in adolescents aged 12-15 years from 68 countries (7 low-income, 27 lower middle-income, 20 upper middle-income, 14 high-income countries). Cross-sectional, school-based data from the Global School-based Student Health Survey were analyzed. Data on past 30-day food insecurity (hunger) and fast-food consumption in the past 7 days were collected. Multivariable logistic regression and meta-analysis were conducted to assess associations. Models were adjusted for age, sex, and body mass index. There were 180,164 adolescents aged 12-15 years [mean (SD) age 13.8 (1.0) years; 50.8% boys] included in the analysis. Overall, severe food insecurity (i.e., hungry because there was not enough food in home most of the time or always) was associated with 1.17 (95%CI=1.08-1.26) times higher odds for fast-food consumption. The estimates pooled by country-income levels were significant in low-income countries (adjusted odds ratio [aOR]=1.30; 95%CI=1.05-1.60), lower middle-income countries (aOR=1.15; 95%CI=1.02-1.29), and upper middle-income countries (aOR=1.26; 95%CI=1.07-1.49), but not in high-income countries (aOR=1.04; 95%CI=0.88-1.23). The mere co-occurrence of food insecurity and fast-food consumption is of public health importance. To tackle this issue, a strong governmental and societal approach is required to utilize effective methods as demonstrated in some high-income countries such as the implementation of food banks and the adoption of free school meals.

Abstract

Objectives
Mild cognitive impairment (MCI) is a unique indicator of underlying distress that may be strongly associated with suicide risk. Despite this, to date, no study has examined the association between MCI and suicidal ideation. Therefore, the present study aimed to examine the association between MCI and suicidal ideation among adults aged ≥65 years from 6 low- and middle-income countries (LMICs; China, Ghana, India, Mexico, Russia, and South Africa).


Methods
Cross-sectional, nationally representative data from the World Health Organization's Study on Global Ageing and Adult Health were analyzed. MCI was defined using the National Institute on Aging-Alzheimer's Association criteria. Self-reported information on past 12-month suicidal ideation was collected. Multivariable logistic regression and meta-analysis were conducted to assess associations.


Results
Data on 13,623 individuals aged ≥65 years were analyzed. The prevalence of suicidal ideation ranged from 0.5% in China to 6.0% in India, whereas the range of the prevalence of MCI was 9.7% (Ghana) to 26.4% (China). After adjustment for potential confounders, MCI was significantly associated with 1.66 (95% confidence interval [95% CI] = 1.12–2.46) times higher odds for suicidal ideation.


Discussion
Mild cognitive impairment was significantly associated with higher odds for suicidal ideation among older adults in LMICs. Future longitudinal studies from LMICs are necessary to assess whether MCI is a risk factor for suicidal ideation.

Dear editor, COVID-19 was declared a global pandemic in March 2020. To prevent transmission of the SARS-CoV-2 virus on 23rd March 2020 the UK government issued guidance that all citizens should stay at home, and only leave their house for essential medical care, shopping for food and medicine and for one period of exercise (National Health Service, 2020). The impact of following the guidance on health-related behaviours is largely unknown. With people spending large periods of time indoors, screen time (e.g. watching TV, using computers, tablets etc) is likely to increase. Excessive screen time use has been shown to be associated with a range of negative mental health outcomes (Allen et al., 2019; Huang et al., 2020; Teychenne et al., 2015). Therefore, we aimed to investigate levels and correlates of screen time during COVID-19 self-isolation in a sample of the UK public, and its association with mental health.
