Self-Tracking as a Threat to Solidary Healthcare? A Power-Critical Approach Towards Solidarity in Digitalized Health Care

"Smart technologies enable an increasing personalization of health care services. This becomes salient in the phenomenon of self-tracking: Health- and fitness-tracking devices are used to control and optimize individual health behavior and to facilitate individually tailored medical prevention and care. While the insurance industry takes an interest in self-tracking data and promises better tariffs and bonus points for those with a healthy lifestyle, consumer protectors warn against personalized risks, discrimination and eroding solidarity. However, the concept of solidarity involved in these debates is often still less than clear, especially with regard to its normative implications and associated power-relations. Against this backdrop, I propose and discuss a power-critical perspective on solidarity in digitalized health care. In an exploratory approach, I first map the relevant power-relations in conjunction with self-tracking data in the insurance industry. As I will argue, these power-relations need critical reflection as they challenge common understandings of solidarity and thus the normative foundations of the welfare state. To this purpose, I provide an outline of a power-critical approach towards solidarity focusing especially on aspects of reciprocity and shared values as fundamental components of solidarity that are closely linked to power relations. Such an approach can help to re-conceptualize solidarity in the context of digitalized health care and thus provide a basis for critical analyses in smart bioethics. "
Методологические основы преодоления диспропорций развития предпринимательского сектора как составляющей устойчивого развития экономики Украины
В статье изложены методологические основы преодоления диспропорций развития предпринимательского сектора как составляющей устойчивого развития экономики Украины. Эти подходы являются базой для разработки методики преодоления диспропорций предпринимательского сектора, в которой должно быть предусмотрено нивелирование действия диспропорциональных факторов: правовых несовершенная и нестабильная нормативная правовая база; политических недостатки действующей социально-экономической политики, ориентированной преимущественно на аграрный характер реальной экономики; экономических высокие тарифы и цены на продукцию и услуги естественных и локальных монопольных структур; нестабильность и фискальный характер системы налогообложения, наличие налога на внутреннее потребление (НДС), что делает невыгодным технологическое производство в промышленности, стимулируя экспорт сырья и полуфабрикатов; рост тарифов за аренду земли, транспортные услуги и т.д.; финансовых практическое отсутствие системы самофинансирования, (кредитные союзы, общества взаимного страхования); практическое отсутствие кредитования банками реальной экономики, в т.ч. жесткие требования к малым предпринимательским структурам по предоставлению гарантий возврата кредита (из-за отсутствия длительной кредитной истории и повышенных рисков); процентные ставки по кредитам, значительно превышающие уровень рентабельности производственных предприятий; отсутствие доступа к иностранным инвестиционным, кредитно-финансовым ресурсам; социальных отсутствие справедливой системы трудовых отношений и социальной защиты предпринимателей; обезличенность страховых фондов (взносы поступают не на персонифицированные счета плательщика, а в общий фонд); организационных недостаточно эффективная институциональная инфраструктура государственной и региональной поддержки (неэффективность и необеспеченность финансовыми ресурсами программ поддержки малого и среднего предпринимательства, низкая роль предпринимателей в их разработке); наличие административных барьеров (трудности при регистрации, закрытии, сертификации, лицензировании, получении других разрешительных документов); многочисленные проверки различных контролирующих органов. ; The article describes the methodological basis to overcome the imbalances of the business sector as a pillar of sustainable economic development of Ukraine. These approaches are the basis for the development of techniques to overcome the imbalances of the business sector, which should provide the leveling action disproportionate factors: legal imperfect and unstable legal and regulatory framework; political the current shortcomings of the socio-economic policy focused mainly on the agricultural character of the real economy; economic high tariffs and prices for the products and services of natural monopolies, and local agencies; instability and fiscal nature of the taxation system, the presence of domestic consumption taxes (value added tax), which makes unprofitable production technology in the industry, stimulating the export of raw materials and semi-finished products; the rising cost of land rent, transportation, etc.; financial the virtual absence of self-financing (credit unions, mutual insurance companies); the virtual absence of lending by banks the real economy, including stringent requirements for small entrepreneurial structures for providing guarantees repayment of the loan (due to lack of long-term credit history and increased risk); interest rates on loans is much higher than the level of profitability of industrial enterprises; lack of access to foreign investment, credit and financial resources; social lack of a fair system of labor relations and social protection of entrepreneurs; impersonality insurance funds (contributions come not personalized the payer's account and the general fund); organizational the lack of effective institutional infrastructure of the state and regional support (inefficiency and insecurity of financial resources programs to support small and medium enterprises, low role of entrepreneurs in their development); administrative barriers (difficulties with registration, closing, certification, licensing, obtaining other permits); numerous inspections of various regulatory bodies.
