The common Filipino needing proper medical treatment can not afford the expensive medical tests, medicines , hospital fees and professional fees which characterize the well-equipped private hospitals concentrated in the national capital region. On the other hand, the government hospitals mostly with limited medical equipment and staffed with underpaid but overworked salaried doctors and nurses can only do so much. The continuing exodus of well-trained and highly-skilled medical professionals like doctors and nurses for better compensation in other countries aggravates the problem. Thus, the need to provide accessible quality health care to meet the medical needs of all Filipinos, especially the poor, through an alternative health delivery system characterized by the establishment of NGO-managed cost-effective regional medical centers using public financial support together with comprehensive medical scholarship and continuing career development program.

Background: Although the Chinese government put a lot of effort into promoting the community patient's life satisfaction, there still lacked the holistic and systematic approaches to promote the community patient's life satisfaction in various regions of China. On the basis of the literature, it was found that both the community patient's assessment of community medical service and trust in community health delivery system were important considerations when the community patient comprehensively evaluated community medical service to generate life satisfaction. So this study was set up to test whether and to what extent the community patient's assessments of various major aspects of community medical service/various major aspects of the community patient's trust in community health delivery system influenced life satisfaction in whole China/in various regions of China. Methods: In order to explore the situation of China's community health delivery system before 2009 and provide a reference for China's community health delivery system reform, the data that could comprehensively and accurately reflect the community patient's life satisfaction, assessment of community medical service, and trust in community health delivery system in various regions of China was needed, so this study collaborated with the National Bureau of Statistics of China to carry out a large-scale 2008 national community resident household survey (N = 3,306) for the first time in China. And the specified ordered probit models were established to analyze the dataset from this household survey. Results: Among major aspects of community medical service, the medical cost (particularly in developed regions), the doctor-patient communication (particularly in developed regions), the medical facility and hospital environment (particularly in developed regions), and the medical treatment process (particularly in underdeveloped regions) were all key considerations (p<0.05 for t statistics) in generating the community patient's life satisfaction. Among major aspects of the community patient's trust in community health delivery system, trust in doctor (particularly in underdeveloped regions), trust in prescription (particularly in underdeveloped regions), and trust in recommended medical examination (particularly in underdeveloped regions) were all important considerations (p<0.10 for t statistics) in generating the community patient's life satisfaction. Conclusion: The reduction of medical cost (particularly in developed regions), the improvement of doctor-patient communication (particularly in developed regions), the promotion of medical facility and hospital environment (particularly in developed regions), the improvement of medical treatment process (particularly in underdeveloped regions), the promotion of trust in doctor (particularly in underdeveloped regions), the promotion of trust in prescription (particularly in underdeveloped regions), and the promotion of trust in recommended medical examination (particularly in underdeveloped regions) could help promote the community patient's life satisfaction.

The purpose of this paper is to determine the gender responsiveness of the present mental health delivery system in the Philippines. According to the 1995 Census of the Population, 4.2 percent of the disabled population are mentally ill. In numerical terms, this is about 38,675 of 919,332 persons with disabilities. Of these, 52.64 percent are female while 47.36 percent are male. Most of those afflicted are in the 25-39 age group. The study is based on interviews with selected mental health care givers in an NGO clinic and in a government hospital and on observations of interactions between patients and care givers. At present, interest in mental illness among women is focused on domestic workers or overseas Filipino workers in foreign countries. While studies on psychiatric morbidity may be useful, these do not reflect the need to understand the social construction of gender in society. What is needed are studies that will constribute to changes in institutional treatment and policy development geared towards gender-responsive practices in mental health care settings.

"Die deutschen Lehrbücher für Gesundheitsökonomie basieren mehr oder weniger auf dem Menschenbild des homo oeconomicus. Dieses grundlegende Paradigma der neoklassischen Ökonomie ist entweder trivial in dem Sinn, dass die Menschen immer versuchen, das Beste aus einer jeden Situation zu machen, oder sie hat nur sehr begrenzte empirische Evidenz, wie man anhand Mark Paulys 'moral hazard'-Postulat zeigen kann. Es besagt, dass die öffentliche Finanzierung von Gesundheitsdiensten falsche Anreize setzt, weil die Nutzer versuchen würden, mehr Leistungen als erforderlich zu bekommen. Diese systematische Überkonsumtion medizinischer Leistungen könne nur durch Zuzahlungen bzw. Franchise-Systeme beschränkt werden. Eigentlich unterstellt Pauly, dass medizinische Behandlungen ein reines Vergnügen sind, von dem man gar nicht genug haben kann - eine äußerst unrealistische Annahme. Zuzahlungen haben nur dann eine rationale Wirkung auf die Inanspruchnahme von medizinischen Leistungen, wenn die Patienten eine wirkliche Wahl haben, etwa im Festbetragssystem für Arzneimittel. Aber wenn es so wenige Belege für die Rationalität von Zuzahlungen gibt, weshalb war dann dieses Instrument in jedem Kostendämpfungsgesetz der letzten 30 Jahre in Deutschland enthalten? Die Gesundheits- und Sozialpolitik wird in Deutschland von einer hoch ideologischen Debatte über Lohnkosten und deren Wirkung auf Deutschlands Stellung im globalen Wettbewerb dominiert. Krankenkassenbeiträge werden als Lohnnebenkosten definiert und für die hohe Arbeitslosigkeit verantwortlich gemacht. Leistungskürzungen in der Gesundheitsversorgung sollen ein Impuls für wirtschaftliches Wachstum und Beschäftigung sein. Es gibt keine belastbaren Belege für diese Behauptung, aber sie beherrscht die veröffentlichte Meinung. Ein anderes Paradigma der neoklassischen Ökonomie ist die umfassende Nützlichkeit des Wettbewerbs. Jedoch gibt es zwei verschiedenen Denkschulen, deren Protagonisten Walter Eucken und F.A. von Hayek sind. Während Eucken der freien Marktwirtschaft eine suizidale Tendenz zum Monopolismus unterstellt und deshalb für einen regulierten Wettbewerb plädiert, nimmt Hayek eine dogmatische Haltung zum freien Markt ein und weist jede politische Einflussnahme ab. Diese unterschiedlichen Grundsätze bestimmen auch heute noch die deutsche Debatte über den Wettbewerb in der gesetzlichen Krankenversicherung. Es ist evident, dass ein solches System nur mit einem Risikostrukturausgleich vernünftig funktionieren kann. Andernfalls entstünde ein total verzerrter Wettbewerb zu Lasten jener Krankenkassen, die chronisch Kranke und sozial Schwache versichern." (Autorenreferat)

Der Verfasser grenzt zunächst das Netzwerkkonzept gegen das eng mit diesem verwandte Konzept 'soziale Unterstützung' ab. Er charakterisiert im folgenden soziale Netzwerke als Vermittlungs- und Beratungsinstanzen. In diesem Sinne stehen sie als soziale Ressourcen vermittelnd zwischen den persönlichen (Persönlichkeitsmerkmale, Fähigkeiten) und den institutionellen Ressourcen (Einrichtungen des Gesundheitsversorgungssystems). Soziale Netzwerke können Ansatzpunkte für direkte und indirekte Interventionen zur Stärkung und Mobilisierung vorhandener oder zum Aufbau neuer Unterstützungspotentiale sein. Der Verfasser stellt exemplarisch Vernetzungsformen bei der Förderung der Selbsthilfe und der Selbstorganisation gegenseitiger Hilfe sowie im professionellen und informellen Gesundheitssystem zur Optimierung der gesundheitlichen Versorgung vor.
