Background Cancer rates in Africa are projected to double by 2030 due to aging and increased exposure to cancer risk factors, including modifiable risk factors. We assessed adherence to 5 modifiable cancer risk factors across 18 African countries. Methods Data on adults 18 years and older were obtained from the 2002–2004 World Health Survey. Adherence to current World Cancer Research Fund guidelines on smoking, alcohol, body weight, physical activity, and nutrition was assessed. Adherence scores ranged from 0 (no guideline met) to 5 (all guidelines met). Determinants of adherence were assessed using multivariable linear regression adjusted for individual and country level characteristics. Results Across all countries, adherence to the guidelines among adults was high for smoking (72%–99%) and alcohol (85%–100%), but low for body weight (1.8%–78%), physical activity (3.4%–84%) and nutrition (1.4%–61%). Overall adherence score ranged from 2.32 in Mali to 3.72 in Comoros. In multivariable models, residing in low versus high SES households was associated with reduced adherence by 0.24 and 0.21 points for men and women respectively after adjusting for age, gender, education, and marital status (p<0.001). Every % increase in GDP spent on health was associated with increased adherence by 0.03 in men and 0.09 in women (p<0.001). Conclusions The wide variation in adherence to cancer prevention guidelines observed across countries and between population sub-groups suggests the need for targeted public health efforts to improve behaviors related to body weight, physical activity and nutrition.

Cancer prevention has been associated with decreased rates of cancer incidence and increased survival. Cancer prevention, however, can have a greater impact if barriers to implementing cancer prevention into practice are removed and opportunities are both fostered and seized. The purpose of this article is to identify barriers and opportunities to cancer prevention in clinical practice and provide recommendations for the future. A multidisciplinary team participated in "The Future Directions Cancer Prevention and Control: Workforce Implications for Training, Practice and Policy" workshop on October 17-18, 2009 at The University of Texas MD Anderson Cancer Center in Houston, TX. During the meeting, the team discussed barriers and opportunities for the implementation of cancer prevention into clinical practice. Further data were collected from peer-reviewed journals and published government and cancer agencies reports. Several issues were identified: 1) The funding allocated to basic cancer prevention research and application is not optimal and less than that for cancer treatment; 2) Participation in cancer prevention behaviors and screening practices are lower than desired, especially among the uninsured; 3) A shortage in healthcare professionals is a major challenge in meeting the future needs of cancer prevention; 4) Demands on medical schools to balance increased enrollment, incorporate cancer prevention in an already crowded curriculum, and develop faculty are daunting; and 5) Healthcare reforms in 2010 provide both opportunities and additional challenges for cancer prevention. Based on the current state of cancer prevention, we formed six recommendations: 1) Additional funding for cancer prevention research with a focus on implementation into practice; 2) Improved tracking of cancer prevention research funding and the outcomes associated with it; 3) Continued monitoring of cancer prevention services participation with emphasis on closing the gap in health disparities; 4) Financial and technical assistance to ...

Strategies for cancer prevention generally come from observational epidemiology and must include monitoring for the effects of the actions. The measurement-iterative loop allows us to refine our approach to cancer prevention. When available, clinical trials can also provide strategies for control. Exposure-specific strategies are described; these are such things as health promotion and behavior modification, legislative approaches, treatment for addiction, changes in the food supply, chemoprevention, occupational and environmental regulation, immunization, identification of persons with enhanced genetic susceptibility, and improved surveillance systems. For some exposures such as tobacco, zero exposure is the goal. For others, prudent avoidance or exposures as low as reasonably achievable are appropriate approaches. Research on how to impact deeply ingrained lifestyle and cultural factors has high priority.

Research on occupational cancer epidemiology has been an important area of occupational health in Europe since the early studies were conducted in the United Kingdom in the 1950s and 1960s. During the last decade, occupational cancer research in Europe has gained an international dimension and become increasingly interdisciplinary in nature. At present, occupational exposures might be responsible for 13 to 18% of lung cancers, 2 to 10% of bladder cancers, and 2 to 8% of laryngeal cancers in European men; among women these figures are 1 to 5%, 0 to 5%, and 0 to 1%, respectively. A notable aspect of current occupational cancer research in Europe is the decreasing importance of traditional circumstances of high exposure to recognized occupational carcinogens and the increasing importance of new industries, mainly in the service sector where possible cancer hazards are poorly known. In addition, the political changes in Central and Eastern Europe open new possibilities for the investigation of high-exposure circumstances and occupational cancer in women.

Cancer prevention is a major component of cancer control, which also comprises screening, treatment, rehabilitation and palliative care. Preventive approaches need to be congruent with those adopted for other chronic diseases, with a major impact in reduction of incidence and mortality of many common cancers to be expected from smoking control and dietary modification. Increasing interest is now being paid to other environmental causes of cancer, and to gene‐environment interactions. However, one of the major research needs remains the evaluation of better ways to convince people to make the necessary changes in their lifestyle that will reduce their risk of cancer.

Cancer prevention research has produced profound scientific knowledge that has led to the development of several evidence‐based prevention strategies. But do these research outcomes lead to preventive action in real life? Many factors contribute to the so‐called 'implementation gap' between prevention recommendations and their application and adherence, including individual actions and behaviour, health service structures and political actions. This article discusses factors underlying the implementation gap in both clinical‐ and population‐based prevention. Understanding how these factors contribute to the implementation gap is important for planning successful cancer prevention strategies, as well as generally achieving disease prevention.
