BACKGROUND: Guidelines based on patient preferences differ from those developed solely by clinicians and may promote patient adherence to guideline recommendations. There is scant evidence on how to develop patient-informed guidelines. This study aimed to describe how guideline developers identify, incorporate and report patient preferences. METHODS: We employed a descriptive cross-sectional survey design. Eligible organizations were non-profit agencies who developed at least one guideline in the past five years and had considered patient preferences in guideline development. We identified developers through the Guidelines International Network and publicly-available guideline repositories, administered the survey online, and used summary statistics to report results. RESULTS: The response rate was 18.3% (52/284). Respondents included professional societies, and government, academic, charitable and healthcare delivery organizations from 18 countries with at least 1 to ≥6 years of experience generating patient-informed guidelines. Organizations most frequently identified preferences through patient panelists (86.5%) and published research (84.6%). Most organizations (48, 92.3%) used multiple approaches to identify preferences (median 3, range 1 to 5). Most often, organizations used preferences to generate recommendations (82.7%) or establish guideline questions (73.1%). Few organizations explicitly reported preferences; instead, they implicitly embedded preferences in guideline recommendations (82.7%), questions (73.1%), or point-of-care communication tools (61.5%). Most developers had little capacity to generate patient-informed guidelines. Few offered training to patients (30.8%), or had dedicated funding (28.9%), managers (9.6%) or staff (9.6%). Respondents identified numerous barriers to identifying preferences. Reduced pesticide use is one of the reasons given by Europeans for accepting new genetic engineering techniques. According to the advocates of these techniques, consumers are likely to embrace the application of cisgenesis to apple trees. In order to verify the acceptability of these techniques, we estimate a Bayesian multilevel structural equation model, which takes into account the multidimensional nature of acceptability and individual, national, and European effects, using data from the Eurobarometer 2010 73.1 on science. The results underline the persistence of clear differences between European countries and whilst showing considerable defiance, a relatively wider acceptability of vertical gene transfer as a means of reducing phytosanitary treatments, compared to horizontal transfer.

During a time of rapid economic transformation in Vietnam, we examined two possibilities for elderly support: living together with children and receiving remittances. Our analysis used four household surveys conducted in Vietnam between 1992 and 2004. With the highly detailed 1997–1998 survey, we found that 73.1 percent of Vietnamese elderly are living with children and 34.8 percent were either receiving remittances directly or married to a recipient. From our logistic regression analysis, we determined further that living with children and remittances both serve continuing roles for elderly support, and our findings suggest that expanding the pension system in Vietnam can potentially play an important role in reducing elderly poverty without crowding out these other support mechanisms.

AbstractThis study examined whether relationship involvement and partner gender were associated with between‐person differences and within‐person fluctuation in positive and negative affect and alcohol use among people who are attracted to more than one gender or regardless of gender (e.g., bisexual, pansexual, queer; bi+). One hundred and ninety‐eight bi+ individuals (M age 26.97; 73.1% non‐Latinx White) completed a 28‐day diary study. Multilevel regression analyses modeled between‐person differences in daily affect and alcohol use as a function of relationship status and partner gender. ANCOVAs were conducted to examine differences in within‐person fluctuation. Results partially support past findings that relationship involvement may not confer benefits for bi+ people and that mixed‐gender partnerships could be particularly challenging. Future research directions are discussed.

AbstractChildren's attributions during challenge have roots in toddlerhood with the onset of self‐evaluative emotions and may reflect sex differences in early socialization practices. This study examined toddler sex as moderating associations between mothers' (n = 104, 73.1% White, 75% with a college degree or more) characteristics (low emotional awareness, emotion coaching beliefs) and toddlers' (Mage = 26.4 mos.; 51 girls) self‐evaluative negative attributions of difficulty during a challenging lab task. Negative attributions reflected self‐directed comments such as "I can't do it." Girls made more negative self‐attributions in the context of mothers' lower emotional awareness whereas boys made more negative self‐attributions in the context of mothers' greater emotion coaching (medium effects). Results demonstrate early sex differences in toddlers' negative self‐attributions to challenge.

Abstract Background International guidelines recommend ethical and scientific quality standards for managing and reporting adverse events occurring during clinical trials to competent research ethics committees and regulatory authorities. The purpose of this study was to determine whether clinical trial protocols in Cameroon are developed in line with national requirements and international guidelines as far as detecting, reporting and investigating of adverse events is concerned. Methods It was a documentary review of all approved clinical trial protocols that were submitted at the Cameroon National Ethics Committee for evaluation from 1997 through 2012. Data were extracted using a preconceived and validated grid. Protocol review process targeted the title, abstract, objectives, methodology, resources, and the chapter on safety. Results In total, 106 (4.9 %) clinical trial protocols were identified from 2173 protocols seen in the archive and 104 (4.8 %) included for review. Seventy six (73.1 %) trials did not include the surveillance of adverse events as part of their objective. A total of 91 (87.5 %) protocols did not budget for adverse event surveillance, 76 (73.1 %) did not have a data safety management board (DSMB), 11(10.6 %) included insurance for participants, 47 (45.2 %) did not include a case definition for serious adverse events, 33 (31.7 %) described procedures to detect adverse events, 33 (31.7 %) described procedure for reporting and 22 (21.2 %) described procedure for investigating adverse events. Discussions Most clinical trial protocols in Cameroon are developed to focus on benefits and pay little attention to harms. The development of national guidelines can improve the surveillance of adverse events in clinical trial research conducted in Cameroon. Adverse events surveillance tools and a budget are critical for an adequate planning for adverse event surveillance when developing trial protocols. Conclusion Clinical trial protocols submitted in the Cameroon National Ethics Committee do not adequately plan to assess adverse events in clinical trial protocols. In order to improve on the safety of participants and marketed drug, there is a need to develop national guidelines for clinical trials by the government, and to improve evaluation procedures and monitoring of ongoing trials by the ethics committee.

Several investigations explain that under physiologic conditions the cervical rachis influences the Temporo Mandibular Junction dynamics; being deduced that in pathological situation, pathology of Tempano Mandibular Junction would take place. According to this concept many authors determine that patients with Temporo Mandibular Disorders (TMD) show a higher prevalence of cervical dysfunctions, while others disagree this point. The investigation tries to clarify this alternative; being carried out a study of traverse and descriptive type; being selected 51 subjects of the staff of the department dentistry of the Hospital Military Central. Being used the index of Krogh Poulsen to evaluate the presence of TTD and the technique of Penning; to evaluate the alterations of the cervical posture; the data were processed through the test of CHI squared (x2). Being a prevalence of 50.9% of TTD and 75.5% of prevalence of abnormal cervical posture. The frequency of abnormal cervical posture in subjects with TTD was of 73.1%; while in the subjects without TTD with it was of 56%. not being significant to the test of Chi squared. ; Muchas investigaciones explican que en condiciones fisiológicas el raquis cervical condiciona la dinámica de la articulación temporo mandibular; deduciéndose que en situación patológica se produciría patología de ésta. De acuerdo con éste concepto muchos autores determinan que individuos con transtornos temporomandibulares (TTM) presentan una mayor prevalencia de disfunciones cervicales mientras que otros contradicen esta opinión; ésta investigación pretende aclarar esta disyuntiva; realizándose un estudio de tipo transversal y descriptivo. Se seleccionaron 51 sujetos perteneciente al personal asistencial del Departamento de Odontología del Hospital Militar Central, Lima, Perú. Se usó el índice de Krogh Poulsen para evaluar la presencia de TTM y la técnica de Penning para evaluar las alteraciones de la postura cervical. Los datos hallados fueron probados con x2. Encontrándose una prevalencia del 50.9% de TTM y 75.5% de prevalencia de postura cervical anormal. La frecuencia de postura cervical anormal en sujetos con TTM fue de 73.1%; mientras que en los sujetos sin TTM fue de 56%. No siendo significativo a la prueba de Chi cuadrado.

Abstract Maintaining and fulfilling the needs of specialist doctors in government public hospitals is still a challenge. Government general hospitals have not been able to meet the minimum standard for specialist doctors. The distribution of personnel and the incentives received by specialist doctors varies between regions. This study aims to provide recommendations related to policies to meet the needs of specialist doctors at government general hospitals. This cross-sectional research study was conducted in May - December 2017 in 19 hospitals in 18 districts/cities. A total of 134 specialist doctors and dental specialists were respondents in this study. The fulfillment of non-financial incentives for specialist doctors, 28.4% lived in official homes, 41.8% received official vehicles and 73.1% were facilitated to attend seminars/education and training. Fulfillment of financial incentives shows that 73.1% of their income is sufficient for the family's needs for 1 month and 63.5% can save. Further east, the incentives received by specialist doctors are greater. The combined capacity of central and local budgets can be optimized to provide adequate and equitable incentives to retain specialist doctors in government hospitals, especially in less desirable areas and with limited budgets. Masih banyak rumah sakit yang belum memenuhi standar minimal dokter spesialis di rumah sakit dan masih adanya disparitas distribusi tenaga dan insentif dokter spesialis di tiap daerah. Penelitian ini bertujuan untuk menyediakan rekomendasi kebijakan terkait pemenuhan kebutuhan dokter spesialis dari aspek penyediaan insentif bagi dokter. Penelitian ini dilakukan secara cross-sectional dan dilaksanakan pada bulan Mei - Desember 2017 di 19 RS di 18 kabupaten / kota. Sebanyak 134 dokter spesialis dan dokter gigi spesialis yang menjadi responden penelitian ini. Sebanyak 28,4% tinggal di rumah layanan yang disediakan, 73,1% dari pendapatannya mencukupi kebutuhan keluarga selama 1 bulan, 63,5% dapat menabung, 41,8% dapat kendaraan, 73,1% telah difasilitasi untuk mengikuti seminar / pendidikan dan pelatihan. Semakin ke timur, insentif yang diterima dokter spesialis semakin besar. Kombinasi kemampuan anggaran pusat dan daerah dapat dioptimalkan untuk memberikan insentif yang memadai dan merata untuk mempertahankan dokter spesialis di rumah sakit pemerintah, terutama di daerah yang tidak diminati dan dengan kendala anggaran. The Russian ancien régime presents a paradox. Like other anciens régimes, it exhibited many signs of decay. The government had been unsuccessful in its major military enterprises ever since the Crimean War, its diplomatic defeats ranging from the Congress of Berlin to the Bosnian annexation crisis of 1909. Failure abroad was accompanied by signs of weakness within. The crisis of 1879-82 had shown the strength of internal opposition. In 1905, ignominious defeat at the hands of Japan proved to be the prelude to a revolution that shook tsarism to its foundations and clearly displayed its structural weaknesses. The last two "Autocrats of All Russia" were men of mediocre talent. The tsarist court, especially under Nicholas II, was ridden with scandals typical of a declining, unsure, and, indeed, frightened regime. Meanwhile, Russia's ruling class, the hereditary landed nobility, improvident and debt-ridden, its landholdings reduced from 73.1 million desiatins in 1877 to a mere 43.2 million by 1911, was in headlong decline.

By 2050, the world population will reach 9.2 billion, increasing the food demand by twice. Lowering loss due to pests is still challenging, where pesticides play an important role, but its indiscriminate use causes inadequate residual amounts to be present in foods. This study aims to monitor the organochlorine and organophosphorus pesticide residuality in Allium fistulosum cultivated in Risaralda, Colombia using gas chromatography - mass spectrometry (GC-MS). This method presented a highly sensitive (LOD: 0.11-7.15 µg kg-1), acceptable precision (RSD: 0.83-1.35%) and recoveries percentages between 46.32% - 118.67%. A greater presence of organochlorine pesticides banned or severely restricted by the Rotterdam agreement, such as 4,4'-DDT, was reported in samples of Allium fistulosum, with concentrations up to 221.22 ?g kg-1, while endrin with a concentration of 469.23 ?g kg-1 and its degradation products which exceed the maximum residue limite (MRL) for plant samples, reported by the Codex Alimentarius. According to this MRL, it was found that 73.1% of the samples have residual exceeding the allowed limit of organochlorine pesticides by more than forty times, posing a risk to human health and the ecosystem. Continuous monitoring and strict governmental control are required to reduce the exposure of humans and other living beings. ; Em 2050 a população mundial chegará a 9.2 bilhões, aumentando a demanda por alimentos em duas vezes. Diminuir as perdas devido a pragas ainda é um desafio, onde os pesticidas desempenham um papel importante, mas seu uso indiscriminado faz com que quantidades residuais inadequadas estejam presentes nos alimentos. Este estudo tem como objetivo monitorar a concentração residual de pesticidas organoclorados e organofosforados em cebola junca (Allium fistulosum) cultivada na Colômbia, como ou uso de GC-MS. O método usado tem alta sensibilidade (LOD: 0.11-7.15 µg kg-1), precisão aceitável precision (RSD: 0.83-1.35%) e percentagens de recuperações entre 46.32% - 118.67%. Pesticidas clorados proibidos pela Convenção de Roterdã foram encontrados nas amostras com concentrações acima de 221.22 ?g kg-1, enquanto endrin com uma concentração de 469.23 ?g kg-1 e seus produtos de degradação que excedem o limite máximo de resíduos (LMR) para amostras de plantas. De acordo com o LMR do Codex Alimentarius, constatou-se que 73.1% das amostras têm residual superior ao limite permitido de pesticidas organoclorados em mais de 40 vezes, o que representa um risco para a saúde humana e ecossistema. Monitoramento contínuo e rigoroso controle governamental são necessários para reduzir a exposição de seres humanos e outros seres vivos.

The Colombian riverine system is composed by a 27.725km water network distributed in the four major basins (Magdalena, Amazonas, Atrato, Orinoco) along the country. 73.1% of the system can be considered as waterways. However instead of leveraging the potential for the exploitation of this natural resource and the promotion of a riverine transportation system which can impact neighboring populations in social aspects such as health and education, in Colombia the priority is focused to road transportations systems. The National Government, which has adopted the challenges for the sustainability of the planet, has accepted the Sustainable Development Goals (SDG) promulgated by the UN and at the same time has linked the iniciative to its 2014 – 2018 National Development Plan. The possibility of connecting the promotion of its riverine vocation and capabilities for solving known socioeconomic needs motivate the creation of this methodological proposal that is driven by the identification of an interregional project portfolio that can be the starting point for closing the diagnosed gaps. De esta red el 73.1% es clasificado como navegable. Sin embargo a pesar del potencial que se tiene para el aprovechamiento del recurso natural y el fomento de un sistema de transporte hidroviário que impacte positivamente a las poblaciones ribereñas en temas sociales como salud y educación, en Colombia la prioridad se encamina al modal de transporte terrestre. El Gobierno Nacional adoptando los nuevos desafíos para impulsar la sostenibilidad del planeta, se ha acogido a los Objetivos de Desarrollo Sostenible (ODS) promulgados por la ONU y a su vez los ha enlazado con sus estrategias contempladas en el Plan Nacional de Desarrollo 2014 – 2018. La posibilidad de enlazar el fomento de la vocación fluvial de los ríos del país y sus capacidades de solventar las necesidades socioeconómico conocidas, motivan la creación de una propuesta metodológica orientada a la identificación de un portafolio de proyectos interregional que se constituya en el punto de partida para la solución de las brechas diagnosticadas al tiempo que se cumplan con las metas establecidas por la Agenda 2030 para el Desarrollo Sostenible definidas por las Naciones Unidas.
