Relapse to substance abuse is a global problem and is conceptualized as an integral component of the recovery process. Global statistics on rates of relapse after substance abuse treatment are disturbingly high, averaging about 75% within a 3- to 6-month duration after treatment. This study sought to gain full understanding of the factors that precipitate relapse among substance abusers in Ghana. Data were collected through in-depth interviews with 15 relapsed substance abusers who were previously treated for substance abuse, and three mental health professionals at a psychiatric rehabilitation unit in Ghana. Findings showed that seven factors, including positive/negative emotional reinforcements, sense of loss, interpersonal conflicts, peer influence, familial, religio-cultural, and treatment-based issues complot to instigate and maintain the relapse cycle. The findings provide valuable insights into the relapse phenomenon in Ghana. Clinicians should actively engage family members in the relapse prevention process, and provide insight into religio-cultural relapse precipitants.

<i>Objective:</i> The efficacy of pharmacological relapse prevention in alcoholism with acamprosate and naltrexone has been supported by several controlled trials. It remains uncertain whether any differential indication for treatment exists. <i>Methods:</i> We evaluated outcome data of a controlled trial on acamprosate and naltrexone in patients with low vs. high baseline somatic distress, depression and anxiety (Symptom Checklist-90, SCL-90), low vs. high baseline craving, and according to typological differentiation as proposed by Cloninger and Lesch. These variables have previously been suggested to be predictors of outcome. <i>Results:</i> Comparing the course of abstinence rates, acamprosate was mainly efficacious in patients with low baseline somatic distress, whereas naltrexone was effective especially in patients with high baseline depression. Baseline craving showed no predictive value. Pharmacological treatment was efficacious in type II alcoholics according to Cloninger. Applying Lesch's typological differentiation, acamprosate was shown to be mainly effective in type I, whereas naltrexone revealed best treatment effects in type III and IV. <i>Conclusion:</i> The study supports the hypothesis that different subgroups of alcohol dependent subjects might benefit from a differential treatment with either naltrexone or acamprosate. Baseline psychopathology and especially typological differentiation might be useful in matching patients to distinct pharmacotherapeutic interventions.

Substance abuse is a global problem that has generated considerable concern among patients, families, clinicians and researchers. Existing research has not explored the views of social workers as treatment professionals but has focused on the service users and their significant others. A qualitative study was conducted to explore the factors influencing relapse in individuals with substance use disorders (SUDs) from a social work perspective. The findings revealed that various factors play a role in relapse at the different levels described from the ecological perspective. The study contributes towards the development of strategies to combat relapse and help in formulating relapse prevention programmes.

Relapse prevention as a supervision strategy for sex offenders is discussed. In the authors' experience, risk factors associated with the abusive acts of sex offenders differ from those of the general criminal population. A review of the information essential to effective probation or parole case planning is provided. Specific probation conditions relevant to sex offenders are suggested Stress is placed on the importance of developing a community-based, collateral network of individuals who might assess the sex offender's self-management and su pervision. An alternative form of community notification, which places responsibility on the offender to inform selected members of his daily life, is discussed.
