This article discusses the two major groups of Alzheimer medications, which are hotly debated in the specialized literature because of their doubtful efficacy. Examining this issue under the rubric of an `anthropology of uncertainty,' this article seeks to address the question: how do doctors prescribe medications given tensions created by uncertainty? A partial answer is drawn from research conducted in Brazil with local psychogeriatricians, which has documented a high degree of certainty regarding Alzheimer drugs and their benefits. I argue that one reason for this certainty is that `efficacy' has become increasingly non-specific in Alzheimer's disease through the broadening of outcome measures in clinical trials. While such measures previously focused on cognitive symptoms, they now encompass concepts such as functionality, quality of life and activities of daily living. The certainty of the Brazilian psychogeriatricians is further buttressed by three interacting elements: (i) the influence of the pharmaceutical industry; (ii) long-standing arguments for including non-cognitive symptoms in dementia care and research; and (iii) a specific discourse found in geriatrics and gerontology, which recognizes `the person beyond cognition.'

El presente artículo desarrolla una sistematización y análisis sobre una intervención desarrollada en mi actividad profesional como trabajador social en un Centro de Recuperación estatal. Surge de las tensiones en el abordaje con una persona con diagnóstico de demencia y la inquietud por generar narrativas que posibiliten prácticas y discursos despatologizantes.El marco teórico consta de cuatro ejes, donde destacan: discursos/prácticas en claves de poder-saber, la herencia positivista y su reproducción acrítica; la categoría de enfermedad mental como construcción teórica; los aportes del análisis institucional; y la denuncia de lo natural desde la semiología.La metodología consiste en el enfoque biográfico-narrativo, entendiendo la identidad narrativa como permanente (re)significación en relación con los espacios transitados, generando la apertura a nuevos discursos y universos de sentido, a nuevas realidades.La reflexión se centra en la necesidad de generar discursos/prácticas por fuera de las hegemónicas biologicistas, conscientes de las lógicas de poder que las atraviesan, construyendo lugares más empáticos con el sufrimiento humano. AbstractThis article develops a systematization and analysis of an intervention made in my professional activity as a social worker in a state Recovery Center. It comes from the tensions in the approach of a person diagnosed with dementia and the need to create narratives that enable de-pathologizing practices and discourses.The theoretical framework consists of four main ideas, including: discourses/practices in the form of power-knowledge, the positivist heritage and its uncritical reproduction; the category of mental illness as a theoretical construction; the contributions of institutional analysis; and the criticism of what is natural from semiotics.The methodology consists of a biographical-narrative approach, understanding the narrative identity as a constant (re)signification in relation to the inhabited spaces, welcoming new discourses and universes of meaning, new realities.The reflection is focused on the need to create discourses/practices outside the hegemonic biologistic ones, being aware of the logics of power that go through them, building places which are more empathetic with human suffering.

AbstractThis paper offers the Eastern Mediterranean Region (EMR) viewpoint with Qatar as a case for lasting transformation of health systems. The Qatar case study illustrates the importance of research in the development of health policy. It provides description of a series of projects that have been undertaken in relevant national areas such as autism, dementia, genomics, palliative care and patient safety. The paper discourse draws attention to investment requirement in health research systems to respond to country national health priorities and to strengthen public health policies for improving health and social outcomes by narrowing the gap between research and politics. In short, the discussion highlights the following: i) health is a human right marching towards universal health care, with research underpinning every advance in health care and quality medical services; ii) evidence-based research is emerging as a critical tool to aid policy- and decision-makers; iii) investment necessity in healthcare research/systems to enable responding to a country's national health priorities and to strengthen public health policies; and iv) need for multi-sectoral involvement of stakeholders to bridge the gap between research and politics. Finally, atypical stakeholders' engagement and bond to politics is a prerequisite to achieve healthcare objectives and policy success so as to reap the benefits of public health results.

PurposeOld age, in its most intense and extreme aspects involving frailty, dementia, Alzheimer's and death, is more often ignored rather than discussed in contemporary anthropology, remaining largely inaudible and invisible. This paper explores the marginal position of the study of old age in contemporary anthropology against the backdrop of the prominence of the post‐colonial agenda. The purpose of this paper is to discuss the comparison between the neglected Third Age and the abundantly discussed Third World in the context of the anthropological discourse on others.Design/methodology/approachThis is a theoretical paper on ageing.FindingsThis paper explores the marginal position of the study of old age in contemporary anthropology against the backdrop of the prominence of the post‐colonial agenda. The comparison between the neglected Third Age and the abundantly discussed Third World is discussed in the context of the anthropological discourse on others. Studying the old‐as‐other reveals two types of alterity: that which is culturally constructed as different vs that which is essentially different. The others that dominate the agenda of contemporary anthropology are culturally constructed, while the old‐as‐other is an ontological essence. The condition of being old, it is argued, is essentially beyond culture, constituting an extra‐cultural materiality. As such, the old‐as‐other does not answer to the anthropological dictum of representing the "natives' point of view" and cannot fit the contemporary hermeneutics of anthropological relativism. Contemporary anthropology, which resists essential objects such as the savage and the old, thus ignores the raw materiality of old age while producing ethnographically‐informed commentaries on eldercare.Originality/valueThe paper is original in highlighting the juxtaposition between the savage and old age that is used to facilitate an understanding of the contemporary discipline of anthropology as a regime of social constructionism, which fails to confront and represent the bare materiality of old age.

Abstract
Citizen science is the new mantra both in academic circles and the public discourse. While the citizen science ideal is conceptually broad, If and how it can be realized in fields often depicted as value free/value neutral—such as applied AI—is controversial. The practical challenges in generating ethical AI encapsulating the citizen science ideal are addressed by targeting scientific practices underlying the participatory design of an AI-based tracking app aimed at enhancing the safety and wellbeing of vulnerable citizens with dementia of a Danish municipality through the engagement of the local community. The focus is on the process of social value construction and its citizen science rationale: how design values have been debated, traded-off, and selected via participatory-deliberative methods and by engaging experts and non-expert stakeholders as citizen scientists. An emphasis is on the import of dialogic interaction for negotiating values in open conversations within diverse groups of interest. Deliberative procedures are beneficial to produce ethical AI embodying vital citizen science desiderata since users'/citizens' values, needs, and expectations are fulfilled while technical-efficiency standards are also met. The result is a public methodology for designing ethical AI that better expresses the true spirit of citizen science in liberal democracies (value-laden, pluralistic, inter-disciplinary, inclusive, participatory, cooperative, solidarity-oriented). Hence, trust and acceptance of the AI generated, even of contentious "surveillance" technologies, is enhanced since the digital innovation can be perceived as truly citizens-/humans-centred and society-oriented.

Understanding why we produce labels for neuropsychiatric conditions, such as Alzheimer's disease (AD), and how we use those words to tell stories about our brain, as well as which groups control such diagnostic discourse, is important to a wise understanding of our cognitive abilities, their limitations, and even our very human nature. Here, we explore the history and current focus of a newly emerging field called neuroethics and explore its relationship (or lack thereof) to a newly created clinical syndrome called involuntary emotional expressive disorder (IEED). The main argument concerns the lack of neuroethical discussion of issues pertinent to social influences on disease and the construction of professional specialization. We are critical of the processes associated with the creation of both the field and the syndrome, and express concern about their eventual outcomes. The interaction of social, political, and business institutions, the inherent interests of the advancement of larger research projects (and the individuals that compose them), their potential for profit, and other incentives to enhance marketability and public attention toward certain research programs will be examined as we discuss the development of the field of neuroethics. Similarly, we argue that these social factors and forces are instrumental in the development of IEED as a recognizable category and condition. Our critique is guided by the hope that through such analyses we can improve our understanding of how we go about our academic activities in cognitive neuroscience and also improve our efforts to help people suffering from neuropsychiatric conditions, such as dementia.

Visual metaphors stand at the border between text and image, as they are linguistic figures that visualize the spoken. Based on this assumption and on the existing knowledge on the discourse analytical method of metaphor analysis, Part One of the article develops a method of visual metaphor analysis (VMA). Part Two uses this method to examine the metaphors of peace found in the acclaimed movie Mango Dreams, the winner of the Peace on Earth Film Festival 2017. The film by John Upchurch tells a story of Dr. Amit Singh, whose family was killed by Muslims during the partition of India and who is suffering from the onset of dementia. In order to confront his fading memories and in pursuit of peace, he commences on a journey to his childhood home in what today is Pakistan. On his travel, he is aided by Muslim rickshaw driver Salim, whose wife was raped and murdered by Hindus. During the long journey across India in a rickshaw, the two form a close friendship and help each other find the peace they have been searching for. Based on our visual metaphor analysis (VMA) of the film, as well as an interview with the director, the article demonstrates how metaphors are employed to visualize a positive concept of peace, particularly HOME, JOURNEY, and BRIDGE, which has a specific temporal, spatial, and moral dimension. In contrast to much of the international relations research on visualization of peace and conflict, the conceptualizations of peace in Mango Dreams metaphorically envision positive peace, instead of the more familiar conceptualization of negative peace, through a representation of PEACE not only in terms of a place (HOME) or process (JOURNEY) but as practice (BRIDGE).

When should a surveillance system that is used in preventive policing sacrifice the privacy of citizens to prevent criminality? What should be the impact of individual moral expectations when a social media platform designs an algorithm? To what degree can we use technology-driven deception in dementia care practices? And can we create a moral compass for a dashboard society? Over the last decade, the impact of technological innovation has been unprecedented. It has profoundly changed the way we participate and interact in society. It has also led to new moral challenges. Not only because of the technology itself, but also because this technology is used in the context of a globalised world with a more prominent role for the private sector. This can result in moral confusion: individuals who alternately assume the role of citizen and consumer feel unable to influence the design of technology that has a strong impact on their core values. Sustaining this moral confusion is in nobody's long-term interest. In this book, we propose to overcome this moral confusion by using a bottom-up design approach that incubates ethics when constructing new technologies. This book is composed of four parts. In the first part we focus on how to integrate moral decisions and morality in the design process of new technology. In the second part we assess how moral design relates to related discourse, including business ethics, law and policy. In the third part of this book various case studies are highlighted that focus on particular moral design issues at the crossroads of technological innovation in the public and private sector. In the last part we look ahead and discuss what the future might look like if we use moral design as a central approach in creating new technology. This book is relevant for IT and engineering professionals, business leaders and policymakers with innovation in their portfolios, and students of (applied) science who are interested in the moral design of technology. The chapters are written by experts and leading researchers in an attractive, accessible and practical writing style. Each chapter offers colourful examples and challenges the reader to critically think through moral decision-making and the design of innovation.
