Intelligence, surveillance, and reconnaissance (ISR) provides an important basis for well-informed decisions in military and security-related domains. Due to the diversity of contributing partners and ISR assets as well as the amount of acquired information, a structured approach to managing and making best use of acquired information is required. Tackling these issues, a conceptual architecture for consistently integrating, presenting, and analyzing ISR products from heterogeneous sources is proposed. This architecture is based on a requirements analysis, the examination of standards and architectural frameworks especially from NATO, as well as the state of the art in the same and conceptually related domains. Following a modular and layer-based approach, the architecture is developed step-by-step, with this publication focusing on the aspects of interoperable source integration and management of the quality of information.

Krebserzeugende Stoffe finden sich überall, auch am Arbeitsplatz. Nicht immer lässt sich ein Kontakt mit ihnen erfolgreich vermeiden. Die in der Folge potenziell auftretenden Krebserkrankungen weisen in der Regel lange Latenzzeiten von durchschnittlich 40 Jahren auf. Ohne eine Dokumentation der Beschäftigungshistorie mit krebserzeugenden und keimzellmutagenen Stoffen lässt sich ein Zusammenhang zwischen einer Erkrankung und einer möglichen Belastung am Arbeitsplatz nach dieser Zeit kaum noch erkennen. In diesem Artikel wird auf die Verpflichtungen der Unternehmen, ein Expositionsverzeichnis nach der Gefahrstoffverordnung zu führen, eingegangen und auf die Möglichkeit, die Archivierungs- und Aushändigungspflicht an den zuständigen Unfallversicherungsträger abzutreten. Aktuelle Entwicklungen im Zusammenhang mit der Zentralen Expositionsdatenbank (ZED) werden beleuchtet und ein Einblick in deren Funktionsweise und zukünftige Weiterentwicklungen wird gegeben. Im Anschluss wird die Entwicklung der Nutzungszahlen der letzten 2,5 Jahre erörtert.

AbstractSubstantial changes in the composition of the vaginal microbiome occur following the end of pregnancy. To identify potential drivers of microbiome changes in individual women during the pregnancy to postpartum transition, we evaluated vaginal samples from 48 pregnant women during their first and third trimesters and postpartum. We determined the species composition of vaginal communities and the vaginal fluid levels of compounds involved in mediating changes in host physiology and the immune system at each time point. We used linear mixed-effects models to characterize associations. Consistent with previous reports, but with a larger sample size, a US population, and variations in the dominant bacteria, the vaginal microbiome was found to be more diverse during the postpartum period. There was a lower abundance of Lactobacillus and significantly higher proportions of Streptococcus anginosus and Prevotella bivia. Moreover, we uniquely demonstrated that postpartum vaginal secretions were also altered postpartum. There were elevated levels of hyaluronan and Hsp70 and decreased levels of the D- and L-lactic acid isomers. We posit that these variations are consequences of alterations in the vagina after delivery that profoundly alter the host environment and, thus, lead to changes in the capability of different bacterial species to survive and proliferate.

Background: The COVID-19 pandemic has disrupted routine hospital services globally. This study estimated the total number of adult elective operations that would be cancelled worldwide during the 12 weeks of peak disruption due to COVID-19. Methods: A global expert response study was conducted to elicit projections for the proportion of elective surgery that would be cancelled or postponed during the 12 weeks of peak disruption. A Bayesian β-regression model was used to estimate 12-week cancellation rates for 190 countries. Elective surgical case-mix data, stratified by specialty and indication (surgery for cancer versus benign disease), were determined. This case mix was applied to country-level surgical volumes. The 12-week cancellation rates were then applied to these figures to calculate the total number of cancelled operations. Results: The best estimate was that 28 404 603 operations would be cancelled or postponed during the peak 12 weeks of disruption due to COVID-19 (2 367 050 operations per week). Most would be operations for benign disease (90·2 per cent, 25 638 922 of 28 404 603). The overall 12-week cancellation rate would be 72·3 per cent. Globally, 81·7 per cent of operations for benign conditions (25 638 922 of 31 378 062), 37·7 per cent of cancer operations (2 324 070 of 6 162 311) and 25·4 per cent of elective caesarean sections (441 611 of 1 735 483) would be cancelled or postponed. If countries increased their normal surgical volume by 20 per cent after the pandemic, it would take a median of 45 weeks to clear the backlog of operations resulting from COVID-19 disruption. Conclusion: A very large number of operations will be cancelled or postponed owing to disruption caused by COVID-19. Governments should mitigate against this major burden on patients by developing recovery plans and implementing strategies to restore surgical activity safely.
