Mobile health (mHealth) is rapidly being implemented and changing our ways of doing, understanding and organising healthcare. mHealth includes wearable devices as well as apps that track fitness, offer wellness programmes or provide tools to manage chronic conditions. According to industry and policy makers, these systems offer efficient and cost-effective solutions for disease prevention and self-management. While this development raises many ethically relevant questions, so far mHealth has received only little attention in medical ethics. This paper provides an overview of bioethical issues raised by mHealth and aims to draw scholarly attention to the ethical significance of its promises and challenges. We show that the overly positive promises of mHealth need to be nuanced and their desirability critically assessed. Finally, we offer suggestions to bioethicists to engage with this emerging trend in healthcare to develop mHealth to its best potential in a morally sound way.

AbstractThe political philosophy latent in Borges's works rests on the belief in a self-sufficient individual, the preeminence of liberty, a distrust of government, and nostalgia for anarchy understood as a self-organized order. Yet Borges also emphasizes the fallibility of individuals and warns against the civic indifference brought about by an isolated individualism. A paradox seems to emerge from these simultaneous convictions: would anarchy work if individuals are unable to do much in and by themselves? Can an individualistic disposition be conducive to a rich and orderly civic life? Borges's notion of fallibility is consistent with his defense of liberal anarchism because fallibility carries less pernicious effects under liberal anarchism than it does under alternative political arrangements. Thus, his notion of liberal anarchism is compatible with his concern for civic order if we look at the ethics of self-restraint that sustains Borges's simultaneous advocacy of a self-organized order and a stable civic life.

"Common morality has been the touchstone for addressing issues of medical ethics since the publication of Beauchamp and Childress's Principles of Biomedical Ethics in 1979. In my presentation, I will challenge that reigning view by presenting two arguments. The negative argument shows why common morality cannot be the ethics of medicine. The positive argument explains why medical professions require their own ethics. I will then explain medicine's distinctive ethics in terms of the trust that society allows to the profession. By distinguishing roles from professions, I will explicate sixteen specific duties that medical professionals undertake when they join the profession. My derivation of medicine's distinctive ethics begins with a thought experiment demonstrating that trust is at the core of medical practice. Society allows doctors to develop special knowledge and skills and allows them to employ special powers, privileges, and immunities that could be particularly dangerous to members of society. Society, therefore, has to be assured that professional's use of their remarkable powers and privileges will be constrained to their intended use. Professions' publically declared codes and oaths go a long way to engender public confidence in medical professionals. Medical education must complete the job by helping our trainees understand their professional obligations and become clinicians who uphold their profession's ethics. Medical educators therefor have to help our students comprehend and internalize their duty to "seek trust and be deserving of it," and uphold their fiduciary responsibility to "use medical knowledge, skills, powers and privileges for the benefit of patients and society." Despite the Supreme Court decision in 1973, Roe v Wade, making abortion legally available without impediment during the first two trimesters of pregnancy, neither the legal nor ethical problems of abortion have been solved. Congressional opposition to federal financial support of abortion & a wide disparity in availability of abortion indicate the unsettled state of public policy. Fetal research & the prospect of in vitro fertilization point to issues which recent court decisions have not clarified. Amniocentesis as a major tool of prenatal diagnosis highlights some of the new dilemmas. Many, perhaps most, MDs still appear morally opposed to using abortion for sex selection or in cases where simple postnatal treatment is available to correct genetic defects. What are the rights of MDs & the rights of women? There is a growing power within medicine to push back the time of viability of an infant earlier & earlier. Summarising the interrelationship between law and medical ethics, I would say that in cases which do not touch the patient's body or integrity, such as professional secrecy, statutory law may take precedence over rules of medical ethics. But in cases where the human subject becomes a victim because of domestic statutory laws which are in contradiction with medical ethics, the medical practitioners should insist on adhering to their professional standards in such a way that the legislators will have to adapt their legislations to the laws of humanity and public conscience. Legislators, as well as medical practitioners, should not forget that the term 'being' is preceded and qualified by 'human'.
