A food supply that delivers energy-dense products with high levels of salt, saturated fats and trans fats, in large portion sizes, is a major cause of non-communicable diseases (NCDs). The highly processed foods produced by large food corporations are primary drivers of increases in consumption of these adverse nutrients. The objective of this paper is to present an approach to monitoring food composition that can both document the extent of the problem and underpin novel actions to address it. The monitoring approach seeks to systematically collect information on high-level contextual factors influencing food composition and assess the energy density, salt, saturated fat, trans fats and portion sizes of highly processed foods for sale in retail outlets (with a focus on supermarkets and quick-service restaurants). Regular surveys of food composition are proposed across geographies and over time using a pragmatic, standardized methodology. Surveys have already been undertaken in several high- and middle-income countries, and the trends have been valuable in informing policy approaches. The purpose of collecting data is not to exhaustively document the composition of all foods in the food supply in each country, but rather to provide information to support governments, industry and communities to develop and enact strategies to curb food-related NCDs.

The ongoing Zika virus (ZIKV) outbreak in Latin America, the Caribbean, and the Pacific Islands has underlined the need for a coordinated research network across the whole region that can respond rapidly to address the current knowledge gaps in Zika and enhance research preparedness beyond Zika. The European Union under its Horizon 2020 Research and Innovation Programme awarded three research consortia to respond to this need. Here we present the ZikaPLAN (Zika Preparedness Latin American Network) consortium. ZikaPLAN combines the strengths of 25 partners in Latin America, North America, Africa, Asia, and various centers in Europe. We will conduct clinical studies to estimate the risk and further define the full spectrum and risk factors of congenital Zika virus syndrome (including neurodevelopmental milestones in the first 3 years of life), delineate neurological complications associated with ZIKV due to direct neuroinvasion and immune-mediated responses in older children and adults, and strengthen surveillance for birth defects and Guillain-Barré Syndrome. Laboratory-based research to unravel neurotropism and investigate the role of sexual transmission, determinants of severe disease, and viral fitness will underpin the clinical studies. Social messaging and engagement with affected communities, as well as development of wearable repellent technologies against Aedes mosquitoes will enhance the impact. Burden of disease studies, data-driven vector control, and vaccine modeling as well as risk assessments on geographic spread of ZIKV will form the foundation for evidence-informed policies. While addressing the research gaps around ZIKV, we will engage in capacity building in laboratory and clinical research, collaborate with existing and new networks to share knowledge, and work with international organizations to tackle regulatory and other bottlenecks and refine research priorities. In this way, we can leverage the ZIKV response toward building a long-term emerging infectious diseases response capacity in the region to address future challenges.

ObjectivesData linkage was conducted between the Office for National Statistics' Covid Infection Survey (CIS), the Department of Health and Social Care's Test and Trace (T&T) and NHS' Personal Demographics Service (PDS) datasets. Linked data was required to provide reliable estimates of rates of COVID-19 transmission and infection used to inform policy regarding the ongoing pandemic.
ApproachThe CIS was created to track infection rates in the UK population. Linking CIS participants to positive tests in T&T helped improve these estimates. Linkage to PDS was required to attach NHS number to these datasets to facilitate further linkages that could also be used to inform Government about the spread of the virus. Multiple approaches were used to link the data. Initially, T&T was linked to itself via a series of strict matchkeys to cluster records belonging to the same individual, to create a person level identifier. Subsequent linkage of CIS-PDS, T&T-PDS and CIS-T&T involved deterministic linkages with matchkeys designed and applied independently. A probabilistic (Fellegi-Sunter scoring) method was used to link CIS-PDS and CIS-T&T. Additional, associative links were created between CIS and T&T records that had matched to the same PDS record but had not matched to each other.
ResultsThe accuracy of CIS-PDS and CIS-T&T linkages was high (recall and precision >98%; all 95% lower confidence intervals >93%). A quality assessment of T&T-PDS is underway, as are relevant bias analyses.
ConclusionAs a result of this linkage, COVID-19 analysts have access to enriched datasets linked to compare previously separated variables, with confidence that the linkage method used was to required quality standards. The linked data has been used to provide crucial evidence to Government on infection and re-infection rates. Subsequent linkages have enabled analysts to explore risk factors associated with different variants of the virus, vaccination status and hospital episodes. Improvements continue to be made.

AbstractWe use a mix of qualitative and quantitative analyses to examine 1354 survey responses from members of the American Anthropological Association about their practice and teaching of cultural anthropology research methods. Latent profile analysis and an examination of responses to open‐ended survey questions reveal distinctive methodological clustering among anthropologists. However, two historical approaches to ethnography remain prominent: deep hanging out and a mixed methods toolkit, with the former remaining central to the practice and teaching of all forms of contemporary cultural anthropology. Further, many anthropologists are committed to advancing research methods that account for power imbalances in fieldwork, such as through community‐based and participatory approaches. And a substantial number also teach a wider array of methods and techniques that open new career pathways for anthropologists. Overall, our study reveals a core set of ethnographic practices—loosely, participant‐observation, informal interviews, and the experiential immersion of the ethnographer—while also highlighting the great breadth of cultural anthropological research practice and pedagogy. The findings presented here can help inform how current and future anthropological practitioners and educators position themselves to meet the ever‐changing demands of community members, funders, clients, collaborators, and students.

OBJECTIVES: Returning to work after an episode of acute coronary syndrome (ACS) is challenging for many patients, and has both personal and social impacts. There are limited data regarding the working status in the very long-term after ACS. METHODS: We retrospectively analyzed 1,632 patients who were working prior to hospitalization for ACS in a quaternary hospital and were followed-up for up to 17 years. Adjusted models were developed to analyze the variables independently associated with actively working at the last contact, and a prognostic predictive index for not working at follow-up was developed. RESULTS: The following variables were significantly and independently associated with actively working at the last contact: age4median (hazard-ratio [HR], 0.76, po0.001); male sex (HR, 1.52, po0.001); government health insurance (HR, 1.36, po0.001); history of angina (HR, 0.69, po0.001) or myocardial infarction (MI) (HR, 0.76, p=0.005); smoking (HR, 0.81, p=0.015); ST-elevation MI (HR, 0.81, p=0.021); anterior-wall MI (HR, 0.75, p=0.001); non-primary percutaneous coronary intervention (PCI) (HR, 0.77, p=0.002); fibrinolysis (HR, 0.61, po0.001); cardiogenic shock (HR, 0.60, p=0.023); statin (HR, 3.01, po0.001), beta-blocker (HR, 1.26, p=0.020), angiotensinconverting enzyme (ACE) inhibitor/angiotensin II receptor blocker (ARB) (HR, 1.37, p=0.001) at hospital discharge; and MI at follow-up (HR, 0.72, p=0.001). The probability of not working at the last contact ranged from 24.2% for patients with no variables, up to 80% for patients with six or more variables. CONCLUSIONS: In patients discharged after ACS, prior and in-hospital clinical variables, as well as the quality of care at discharge, have a great impact on the long-term probability of actively working.

This article's authors delve into, and comment on, some of the key provisions within law no. 219, passed in 2017, which came into full effect in 2018. The legislation presents several innovative aspects: (a) communication time is equated to care; (b) patients may turn down lifesaving treatments, yet doctors must put in place all suitable support processes, from a psychological standpoint as well, in order to make sure that patients make informed decisions in full awareness; (c) refusal to treatment may be expressed prior to the onset of the disease making the patient incapable, as long as the advance directive is laid out by a mentally capable adult who has been provided with all relevant medical information available as to the consequence of a refusal to undergo a given treatment; (d) artificial nutrition and hydration are tantamount to treatment; thus, they may not be carried out and kept in place in absence of valid consent; (e) patients may appoint a healthcare proxy holder, tasked with interacting with doctors and caregivers and expressing consent or refusal; (f) patient will, whether current or advance, must be complied with even under emergency or urgency conditions, provided that clinical conditions and circumstances make it possible to acquire it; (g) doctors may disregard advance directives only when specifically provided for by the law; (h) patients may not demand treatment deemed to be illegal or running counter to ethical codes or scientific evidence. The new legislation, therefore, is meant to uphold the right to exercise self-determination as well as the patient's quality of life, yet ensuring that doctors remain fully capable of making the decisions that they are best positioned to.
