En mi formación de posgrado a finales de los años ochenta, teníamos cerca de treinta camas hospitalarias en un pabellón llamado "sépticas" (1). En Colombia, donde el aborto estaba totalmente penalizado, allí estaban mayoritariamente mujeres con abortos inseguros complicados. El enfoque que recibíamos era técnico: manejo de cuidados intensivos; realizar histerectomías, colostomías, resecciones intestinales, etc. En esa época algunas enfermeras eran monjas, y se limitaban a interrogar a las pacientes para que "confesaran" qué se habían hecho para abortar. Siempre me inquietó que las mujeres que salían vivas se iban sin ninguna asesoría, ni con un método anticonceptivo. Al preguntar alguna vez a uno de mis docentes me contestó con desdén: "este es un hospital de tercer nivel, esas cosas las hacen las enfermeras en primer nivel". Al ver tanto dolor y muerte, decidí hablar con las pacientes del servicio y empecé a entender sus decisiones. Recuerdo aún con tristeza tantas muertes, pero un caso en particular aún me duele: era una mujer cercana a los cincuenta años que llegó con una perforación uterina en estado de sepsis avanzada. A pesar de la cirugía y los cuidados intensivos, falleció. Alcancé a hablar con ella y me contó que era viuda, tenía dos hijos mayores y había abortado por "vergüenza con ellos", pues se iban a dar cuenta de que tenía vida sexual activa. A los pocos días de su fallecimiento, me llamó el profesor de patología, extrañado, para decirme que el útero que habíamos enviado para examen patológico no tenía embarazo. Era una mujer en estado perimenopáusico con una prueba de embarazo falsamente positiva, debido a los altos niveles de FSH/LH típicos de su edad. ¡¡¡NO ESTABA EMBARAZADA!!! No tenía menstruación porque estaba en premenopausia y una prueba falsamente positiva la llevó a un aborto inseguro. Claro, las lesiones causadas en las maniobras abortivas la llevaron al desenlace fatal, pero la real causa subyacente fue el tabú social respecto a la sexualidad. Tuve que ver muchas adolescentes y mujeres jóvenes salir del hospital vivas, pero sin útero, a veces sin ovarios y con colostomías, para ser despreciadas por una sociedad que les recriminaba el haber decidido no ser madres. Tuve que ver situaciones de mujeres que llegaban con sus intestinos protruyendo a través de sus vaginas por abortos inseguros. Vi mujeres que en su desespero se autoinfligieron lesiones tratando de abortar con elementos como palos, ramas, gajos de cebolla, barras de alumbre, ganchos, entre otros. Eran tantas las muertes que era difícil no tener por lo menos una mujer diariamente en la morgue a consecuencia de un aborto inseguro. En esa época no se abordaba la salud desde lo biopsicosocial sino solamente desde lo técnico (2); sin embargo, en las evaluaciones académicas que nos hacían, ante la pregunta de definición de salud, había que recitar el texto de la Organización Mundial de la Salud que involucraba estos tres aspectos, ¡qué contrasentido! Para dar respuesta a las necesidades de salud de las mujeres y garantizar sus derechos, cuando ya era docente, inicié el servicio de anticoncepción posevento obstétrico en ese hospital de tercer nivel. Hubo resistencia de las directivas, pero afortunadamente logré donaciones internacionales para la institución y esto facilitó su aceptación. Decidí concursar para carrera docente con el ánimo de poder sensibilizar a profesionales de la salud hacia un enfoque integral de la salud y la enfermedad. Cuando en 1994 se realizó la Conferencia Internacional de Población y Desarrollo (CIPD) en El Cairo ya llevaba varios años en la docencia, y cuando leí su Programa de Acción, encontré nombre para lo que estaba trabajando: derechos sexuales y derechos reproductivos. Empecé a incorporar en mi vida profesional y docente las herramientas que este documento me daba. Pude sensibilizar personas del Ministerio de Salud de mi país y trabajamos en conjunto recorriéndolo con un abordaje de derechos humanos en materia de salud sexual y reproductiva (SSR). Esta nueva mirada buscaba además de ser integral, dar respuesta a viejos problemas como la mortalidad materna, el embarazo en la adolescencia, la baja prevalencia anticonceptiva, el embarazo no planeado o no deseado o la violencia contra la mujer. Con otras personas sensibilizadas empezamos a permear con estos temas de SSR la Sociedad Colombiana de Obstetricia y Ginecología, algunas universidades y hospitales universitarios. Todavía seguimos dando la lucha en un país que a pesar de tantas dificultades ha mejorado muchos indicadores de SSR. Con la experiencia de haber trajinado en todas las esferas con estos temas, logramos con un puñado de colegas y amigas de la Universidad El Bosque crear la Maestría en Salud Sexual y Reproductiva, abierta a todas las profesiones, en la que rompimos varios paradigmas. Se inició un programa en el que la investigación cualitativa y cuantitativa tenían el mismo peso y algunos de los egresados del programa están ahora en posiciones de liderazgo en los entes gubernamentales e internacionales replicando modelos integrales. En la Federación Latinoamericana de Obstetricia y Ginecología (FLASOG) y en la Federación Internacional de Obstetricia y Ginecología (FIGO), pude por varios años aportar mi experiencia en los comités de SSR de esas asociaciones para beneficio de las mujeres y las niñas en los ámbitos regional y global. Cuando pienso en quienes me han inspirado en esta lucha, debo resaltar las grandes feministas que me han enseñado y acompañado en tantas batallas. No puedo mencionarlas a todas, pero he admirado la historia de vida de Margaret Sanger con su persistencia y mirada visionaria. Ella luchó durante toda su vida para ayudar a las mujeres del siglo XX para que obtuvieran el derecho a decidir si querían o no tener hijos o hijas y cuándo (3). De las feministas actuales he tenido el privilegio de compartir experiencias con Carmen Barroso, Giselle Carino, Debora Diniz y Alejandra Meglioli, lideresas de la Federación Internacional de Planificación de la Familia, Región del Hemisferio Occidental (IPPF-RHO, por su nombre en inglés). De mi país quiero resaltar a mi compatriota Florence Thomas, psicóloga, columnista, escritora y activista feminista colombo-francesa. Es una de las voces más influyentes e importantes del movimiento por los derechos de la mujer en Colombia y en la región. Arribó procedente de Francia en la década de 1960, en los años de la contracultura, los Beatles, los hippies, Simone de Beauvoir y Jean-Paul Sartre, época en la que se empezó a criticar el capitalismo y la cultura del consumo (4). Fue entonces cuando se comenzó a hablar del cuerpo femenino, la sexualidad femenina y cuando llegó la píldora anticonceptiva como una revolución total para las mujeres. A su llegada en 1967, ella experimentó un choque porque acababa de asistir a toda una revolución y solo encontró un país de madres, no de mujeres (5). Ese era el único destino de una mujer, ser callada y sumisa. Entonces se dio cuenta de que no se podía seguir así, hablando de "vanguardias revolucionarias" en un ambiente tan patriarcal. En 1986 con las olas del feminismo norteamericano y europeo, y con su equipo académico crearon el grupo Mujer y Sociedad de la Universidad Nacional de Colombia, semillero de grandes iniciativas y logros para el país (6). Ella ha liderado grandes cambios con su valentía, la fuerza de sus argumentos, y un discurso apasionado y agradable a la vez. Dentro de sus múltiples libros resalto Conversaciones con Violeta (7), motivado por el desdén hacia el feminismo de algunas mujeres jóvenes. Lo escribe a manera de diálogo con una hija imaginaria en el que, de una manera íntima, reconstruye la historia de las mujeres a través de los siglos y da nuevas luces sobre el papel fundamental del feminismo en la vida de la mujer moderna. Otro libro muestra de su valentía es Había que decirlo (8), en el que narra la experiencia de su propio aborto a sus 22 años en la Francia de los años sesenta. Mi experiencia de trabajo en la IPPF-RHO me ha permitido conocer líderes y lideresas de todas las edades en diversos países de la región, quienes con gran mística y dedicación, de manera voluntaria, trabajan por lograr una sociedad más equitativa y justa. Particularmente me ha impresionado la apropiación del concepto de derechos sexuales y reproductivos por parte de las personas más jóvenes, y esto me ha dado gran esperanza en el futuro del planeta. Seguimos con una agenda incompleta del Plan de acción de la CIPD de El Cairo, pero ver cómo la juventud enfrenta con valentía los retos, me motiva a seguir adelante y aportar mis años de experiencia en un trabajo intergeneracional. La IPPF-RHO evidencia un gran compromiso por los derechos y la SSR de adolescentes en sus políticas y programas, que son consistentes con lo que la Organización promueve; por ejemplo, el 20% de los puestos de toma de decisión están en manos de jóvenes. Las organizaciones miembros, que basan su labor en el voluntariado, son verdaderas incubadoras de jóvenes que harán ese recambio generacional inexpugnable y necesario. A diferencia de lo que nos tocó a muchos de nosotros, trabajar en esta complicada agenda de salud sexual y reproductiva sin bases teóricas, hoy vemos personas comprometidas y con una sólida formación para reemplazarnos. En la Facultad de Medicina de la Universidad Nacional de Colombia y en la Facultad de Enfermería de la Universidad El Bosque, las nuevas generaciones están más motivadas y empoderadas, con grandes deseos de cambiar las rígidas estructuras subyacentes. Nuestra gran preocupación son los embates de ultraderecha que soportan grupos antiderechos, muchas veces mejor organizados que nosotros, que sí apoyamos los derechos y somos verdaderos provida (9). Ante este escenario, debemos organizarnos mejor y seguir dando batallas para garantizar los derechos de las mujeres en el ámbito local, regional y global, aunando esfuerzos de todas las organizaciones proderechos. Estamos ahora comprometidos con los Objetivos de Desarrollo Sostenible (10), entendidos como aquellos que satisfacen las necesidades de la generación presente sin comprometer la capacidad de las generaciones futuras para satisfacer sus propias necesidades. Esta nueva agenda se basa en: - El trabajo no finalizado de los Objetivos de Desarrollo del Milenio - Los compromisos pendientes (convenciones ambientales internacionales) - Los temas emergentes en las tres dimensiones del desarrollo sostenible: social, económica y ambiental. Tenemos ahora 17 Objetivos de Desarrollo Sostenible y 169 metas (11). Entre estos objetivos se menciona en varias ocasiones el "acceso universal a la salud reproductiva". En el Objetivo 3 de esa lista se incluye garantizar, de aquí al año 2030, "el acceso universal a los servicios de salud sexual y reproductiva, incluidos los de planificación familiar, información y educación". De igual manera, el Objetivo 5, "Lograr la igualdad de género y empoderar a todas las mujeres y las niñas", establece que se deberá "asegurar el acceso universal a la salud sexual y reproductiva y los derechos reproductivos según lo acordado de conformidad con el Programa de Acción de la Conferencia Internacional sobre la Población y el Desarrollo, la Plataforma de Acción de Beijing". No se puede olvidar que el término acceso universal a la salud sexual y reproductiva incluye el acceso universal al aborto y la anticoncepción. Actualmente 830 mujeres mueren cada día por causas maternas prevenibles; de estos decesos, el 99% ocurre en países en desarrollo, más de la mitad en entornos frágiles y en contextos humanitarios (12). 216 millones de mujeres no pueden acceder a métodos de anticoncepción moderna y la mayoría vive en los nueve países más pobres del mundo y en un ambiente cultural propio de la década de los sesenta (13). Este número solo incluye las mujeres de 15 a 49 años en cualquier tipo de unión, es decir el número total es mucho mayor. Cumplir con los objetivos marcados supondría prevenir 67 millones de embarazos no deseados y reducir a un tercio las muertes maternas. Actualmente tenemos una alta demanda insatisfecha de anticoncepción moderna, con un bajísimo uso de los métodos de larga duración reversible (dispositivos intrauterinos e implantes subdérmicos) que son los más efectivos y de mayor adherencia (14). No hay uno solo de los 17 Objetivos de Desarrollo Sostenible donde la anticoncepción no tenga un papel preponderante: desde el primero que se refiere al fin de la pobreza, pasando por el quinto de igualdad de género, el décimo de reducción de la desigualdad, entre los países y en el mismo país, hasta el decimosexto relacionado con paz y justicia. Si queremos cambiar el mundo, debemos procurar acceso universal a la anticoncepción sin mitos ni barreras. Tenemos la obligación moral de lograr la erradicación de la pobreza extrema y avanzar en la construcción de sociedades más igualitarias, justas y felices. En anticoncepción de urgencia (AU), estamos muy lejos de alcanzar lo que esperamos. Si en métodos de larga duración reversible tenemos una baja prevalencia, en la AU la situación empeora. No en todas las facultades de medicina de la región se aborda este tema, y donde sí se hace, no hay homogeneidad de contenidos, ni siquiera dentro del mismo país. Hay aún mitos sobre su verdadero mecanismo de acción. Hay países como Honduras donde está prohibida y no hay un medicamento dedicado, como tampoco lo hay en Haití. Donde está disponible el acceso es ínfimo, particularmente entre las niñas, adolescentes, jóvenes, migrantes, afrodescendientes e indígenas. Hay que derrumbar las múltiples barreras para el uso eficaz de la anticoncepción de emergencia, y para eso necesitamos trabajar en romper mitos y percepciones erróneas, tabúes y normas culturales; lograr cambios en las leyes y normas restrictivas de los países; lograr acceso sin barreras a la AU; trabajar intersectorialmente; capacitar al personal de salud y la comunidad. Es necesario transformar la actitud del personal de salud en una de servicio por encima de sus propias opiniones. Reflexionando acerca de lo que ha pasado después de la CIPD realizada en El Cairo, su Programa de Acción cambió cómo miramos las dinámicas de población de un énfasis en la demografía a un enfoque en los derechos humanos y las personas. Los gobiernos acordaron que, en este nuevo enfoque, el éxito era el empoderamiento de las mujeres y la posibilidad de elegir a través de expandir el acceso a la educación, la salud, los servicios y el empleo, entre otros. Sin embargo, ha habido avances desiguales y persiste la inequidad en nuestra región, no se cumplieron todas las metas, los derechos sexuales y reproductivos continúan fuera del alcance de muchas mujeres (15). Aún queda un largo camino para recorrer, hasta que mujeres y niñas del mundo puedan reclamar sus derechos y la libertad de decidir. Globalmente la mortalidad materna se ha reducido, hay mayor asistencia calificada del parto, mayor prevalencia anticonceptiva, la educación integral en sexualidad y el acceso a servicios de SSR para adolescentes ya son derechos reconocidos y con grandes avances, además ha habido ganancias concretas en materia de marcos legales más favorables en particular en nuestra región; sin embargo, si bien las condiciones de acceso han mejorado, las legislaciones restrictivas de la región exponen a las mujeres más vulnerables a abortos inseguros. Hay aún grandes desafíos para que los gobiernos reconozcan la SSR y los DSR como parte integral de los sistemas de salud, existe una amplia agenda contra las mujeres. En ese sentido, el acceso a SSR está bajo amenaza y opresión, se requiere movilización intersectorial y litigios estratégicos, investigación y apoyo a los derechos de las mujeres como agenda intersectorial. Hacia adelante hay que esforzarnos más en el trabajo con jóvenes, para avanzar no solo en el Programa de Acción de la CIPD, sino en todos los movimientos sociales. Son uno de los grupos más vulnerables, y de los mayores catalizadores para el cambio. La población joven aún enfrentan muchos desafíos, especialmente las mujeres y niñas; las jóvenes están especialmente en alto riesgo debido a la falta de servicios y salud sexual y reproductiva amigables y confidenciales, la presencia de violencia basada en género y la falta de acceso a los servicios. Además hay que mejorar el acceso al aborto; es responsabilidad de los estados garantizar la calidad y seguridad en el acceso. Aún en nuestra región existen países con marcos totalmente restrictivos. Las nuevas tecnologías facilitan el autocuidado (16), lo que permitirá ampliar el acceso universal, pero los gobiernos no pueden desvincularse de su responsabilidad. El autocuidado se está expandiendo en el mundo y puede ser estratégico para llegar a las poblaciones más vulnerables. Hay nuevos desafíos para los mismos problemas, que requieren una reinterpretación de las medidas necesarias para garantizar los DSR de todas las personas, en particular mujeres, niñas y en general las poblaciones marginadas y vulnerables. Es necesario tener en cuenta aspectos como las migraciones, el cambio climático, el impacto de medios digitales, el resurgimiento de discursos de odio, la opresión, la violencia, la xenofobia, la homo/transfobia y otros problemas emergentes, pues la SSR debe verse en un marco de justicia, y no aislado. Debemos exigir rendición de cuentas a los 179 gobiernos que participaron en la CIPD hace 25 años y a los 193 países que firmaron los Objetivos de Desarrollo Sostenible. Deben reafirmarse en sus compromisos y expandir la agenda a los temas no considerados en ese momento. Nuestra región ha dado ejemplo al mundo con el Consenso de Montevideo, que se convierte en una hoja de ruta para el cumplimiento del plan de acción de la CIPD y no debe permitirnos retroceder. Este Consenso pone en el centro a las personas, en especial a las mujeres, e incluye el tema de aborto invitando a los estados a que consideren la posibilidad de legalizarlo, lo que abre la puerta para que los gobiernos de todo el mundo reconozcan que las mujeres tienen el derecho a decidir sobre la maternidad. Este Consenso es mucho más inclusivo: Considerando que las brechas en salud continúan sobresalientes en la región y las estadísticas promedio suelen ocultar los altos niveles de mortalidad materna, de infecciones de transmisión sexual, de infección por VIH/SIDA y de demanda insatisfecha de anticoncepción entre la población que vive en la pobreza y en áreas rurales, entre los pueblos indígenas y las personas afrodescendientes y grupos en condición de vulnerabilidad como mujeres, adolescentes y jóvenes y personas con discapacidad, acuerdan: 33-Promover, proteger y garantizar la salud y los derechos sexuales y los derechos reproductivos para contribuir a la plena realización de las personas y a la justicia social en una sociedad libre de toda forma de discriminación y violencia. 37-Garantizar el acceso universal a servicios de salud sexual y salud reproductiva de calidad, tomando en consideración las necesidades específicas de hombres y mujeres, adolescentes y jóvenes, personas LGBT, personas mayores y personas con discapacidad, prestando particular atención a personas en condición de vulnerabilidad y personas que viven en zonas rurales y remotas y promoviendo la participación ciudadana en el seguimiento de los compromisos. 42-Asegurar, en los casos en que el aborto es legal o está despenalizado en la legislación nacional, la existencia de servicios de aborto seguros y de calidad para las mujeres que cursan embarazos no deseados y no aceptados e instar a los demás Estados a considerar la posibilidad de modificar las leyes, normativas, estrategias y políticas públicas sobre la interrupción voluntaria del embarazo para salvaguardar la vida y la salud de mujeres adolescentes, mejorando su calidad de vida y disminuyendo el número de abortos (17). ; In my postgraduate formation during the last years of the 80's, we had close to thirty hospital beds in a pavilion called "sépticas" (1). In Colombia, where abortion was completely penalized, the pavilion was mostly filled with women with insecure, complicated abortions. The focus we received was technical: management of intensive care; performance of hysterectomies, colostomies, bowel resection, etc. In those times, some nurses were nuns and limited themselves to interrogating the patients to get them to "confess" what they had done to themselves in order to abort. It always disturbed me that the women who left alive, left without any advice or contraceptive method. Having asked a professor of mine, he responded with disdain: "This is a third level hospital, those things are done by nurses of the first level". Seeing so much pain and death, I decided to talk to patients, and I began to understand their decision. I still remember so many deaths with sadness, but one case in particular pains me: it was a woman close to being fifty who arrived with a uterine perforation in a state of advanced sepsis. Despite the surgery and the intensive care, she passed away. I had talked to her, and she told me she was a widow, had two adult kids and had aborted because of "embarrassment towards them" because they were going to find out that she had an active sexual life. A few days after her passing, the pathology professor called me, surprised, to tell me that the uterus we had sent for pathological examination showed no pregnancy. She was a woman in a perimenopausal state with a pregnancy exam that gave a false positive due to the high levels of FSH/LH typical of her age. SHE WAS NOT PREGNANT!!! She didn't have menstruation because she was premenopausal and a false positive led her to an unsafe abortion. Of course, the injuries caused in the attempted abortion caused the fatal conclusion, but the real underlying cause was the social taboo in respect to sexuality. I had to watch many adolescents and young women leave the hospital alive, but without a uterus, sometime without ovaries and with colostomies, to be looked down on by a society that blamed them for deciding to not be mothers. I had to see situation of women that arrived with their intestines protruding from their vaginas because of unsafe abortions. I saw women, who in their despair, self-inflicted injuries attempting to abort with elements such as stick, branches, onion wedges, alum bars and clothing hooks among others. Among so many deaths, it was hard not having at least one woman per day in the morgue due to an unsafe abortion. During those time, healthcare was not handled from the biopsychosocial, but only from the technical (2); nonetheless, in the academic evaluations that were performed, when asked about the definition of health, we had to recite the text from the International Organization of Health that included these three aspects. How contradictory! To give response to the health need of women and guarantee their right when I was already a professor, I began an obstetric contraceptive service in that third level hospital. There was resistance from the directors, but fortunately I was able to acquire international donations for the institution, which facilitated its acceptance. I decided to undertake a teaching career with the hope of being able to sensitize health professionals towards an integral focus of health and illness. When the International Conference of Population and Development (ICPD) was held in Cairo in 1994, I had already spent various years in teaching, and when I read their Action Program, I found a name for what I was working on: Sexual and Reproductive Rights. I began to incorporate the tools given by this document into my professional and teaching life. I was able to sensitize people at my countries Health Ministry, and we worked together moving it to an approach of human rights in areas of sexual and reproductive health (SRH). This new viewpoint, in addition to being integral, sought to give answers to old problems like maternal mortality, adolescent pregnancy, low contraceptive prevalence, unplanned or unwanted pregnancy or violence against women. With other sensitized people, we began with these SRH issues to permeate the Colombian Society of Obstetrics and Gynecology, some universities, and university hospitals. We are still fighting in a country that despite many difficulties has improved its indicators of SRH. With the experience of having labored in all sphere of these topics, we manage to create, with a handful of colleagues and friend at the Universidad El Bosque, a Master's Program in Sexual and Reproductive Health, open to all professions, in which we broke several paradigms. A program was initiated in which the qualitative and quantitative investigation had the same weight, and some alumni of the program are now in positions of leadership in governmental and international institutions, replicating integral models. In the Latin American Federation of Obstetrics and Gynecology (FLASOG, English acronym) and in the International Federation of Obstetrics and Gynecology (FIGO), I was able to apply my experience for many years in the SRH committees of these association to benefit women and girls in the regional and global environments. When I think of who has inspired me in these fights, I should highlight the great feminist who have taught me and been with me in so many fights. I cannot mention them all, but I have admired the story of the life of Margaret Sanger with her persistence and visionary outlook. She fought throughout her whole life to help the women of the 20th century to be able to obtain the right to decide when and whether or not they wanted to have children (3). Of current feminist, I have had the privilege of sharing experiences with Carmen Barroso, Giselle Carino, Debora Diniz and Alejandra Meglioli, leaders of the International Planned Parenthood Federation – Western Hemisphere Region (IPPF-RHO). From my country, I want to mention my countrywoman Florence Thomas, psychologist, columnist, writer and Colombo-French feminist. She is one of the most influential and important voices in the movement for women rights in Colombia and the region. She arrived from France in the 1960's, in the years of counterculture, the Beatles, hippies, Simone de Beauvoir, and Jean-Paul Sartre, a time in which capitalism and consumer culture began to be criticized (4). It was then when they began to talk about the female body, female sexuality and when the contraceptive pill arrived like a total revolution for women. Upon its arrival in 1967, she experimented a shock because she had just assisted in a revolution and only found a country of mothers, not women (5). That was the only destiny for a woman, to be quiet and submissive. Then she realized that this could not continue, speaking of "revolutionary vanguards" in such a patriarchal environment. In 1986 with the North American and European feminism waves and with her academic team, they created the group "Mujer y Sociedad de la Universidad Nacional de Colombia", incubator of great initiatives and achievements for the country (6). She has led great changes with her courage, the strength of her arguments, and a simultaneously passionate and agreeable discourse. Among her multiple books, I highlight "Conversaciones con Violeta" (7), motivated by the disdain towards feminism of some young women. She writes it as a dialogue with an imaginary daughter in which, in an intimate manner, she reconstructs the history of women throughout the centuries and gives new light of the fundamental role of feminism in the life of modern women. Another book that shows her bravery is "Había que decirlo" (8), in which she narrates the experience of her own abortion at age twenty-two in sixty's France. My work experience in the IPPF-RHO has allowed me to meet leaders of all ages in diverse countries of the region, who with great mysticism and dedication, voluntarily, work to achieve a more equal and just society. I have been particularly impressed by the appropriation of the concept of sexual and reproductive rights by young people, and this has given me great hope for the future of the planet. We continue to have an incomplete agenda of the action plan of the ICPD of Cairo but seeing how the youth bravely confront the challenges motivates me to continue ahead and give my years of experience in an intergenerational work. In their policies and programs, the IPPF-RHO evidences great commitment for the rights and the SRH of adolescent, that are consistent with what the organization promotes, for example, 20% of the places for decision making are in hands of the young. Member organizations, that base their labor on volunteers, are true incubators of youth that will make that unassailable and necessary change of generations. In contrast to what many of us experienced, working in this complicated agenda of sexual and reproductive health without theoretical bases, today we see committed people with a solid formation to replace us. In the college of medicine at the Universidad Nacional de Colombia and the College of Nursing at the Universidad El Bosque, the new generations are more motivated and empowered, with great desire to change the strict underlying structures. Our great worry is the onslaught of the ultra-right, a lot of times better organized than us who do support rights, that supports anti-rights group and are truly pro-life (9). Faced with this scenario, we should organize ourselves better, giving battle to guarantee the rights of women in the local, regional, and global level, aggregating the efforts of all pro-right organizations. We are now committed to the Objectives of Sustainable Development (10), understood as those that satisfy the necessities of the current generation without jeopardizing the capacity of future generations to satisfy their own necessities. This new agenda is based on: - The unfinished work of the Millennium Development Goals - Pending commitments (international environmental conventions) - The emergent topics of the three dimensions of sustainable development: social, economic, and environmental. We now have 17 objectives of sustainable development and 169 goals (11). These goals mention "universal access to reproductive health" many times. In objective 3 of this list is included guaranteeing, before the year 2030, "universal access to sexual and reproductive health services, including those of family planning, information, and education." Likewise, objective 5, "obtain gender equality and empower all women and girls", establishes the goal of "assuring the universal access to sexual and reproductive health and reproductive rights in conformity with the action program of the International Conference on Population and Development, the Action Platform of Beijing". It cannot be forgotten that the term universal access to sexual and reproductive health includes universal access to abortion and contraception. Currently, 830 women die every day through preventable maternal causes; of these deaths, 99% occur in developing countries, more than half in fragile environments and in humanitarian contexts (12). 216 million women cannot access modern contraception methods and the majority live in the nine poorest countries in the world and in a cultural environment proper to the decades of the seventies (13). This number only includes women from 15 to 49 years in any marital state, that is to say, the number that takes all women into account is much greater. Achieving the proposed objectives would entail preventing 67 million unwanted pregnancies and reducing maternal deaths by two thirds. We currently have a high, unsatisfied demand for modern contraceptives, with extremely low use of reversible, long term methods (intrauterine devices and subdermal implants) which are the most effect ones with best adherence (14). There is not a single objective among the 17 Objectives of Sustainable Development where contraception does not have a prominent role: from the first one that refers to ending poverty, going through the fifth one about gender equality, the tenth of inequality reduction among countries and within the same country, until the sixteenth related with peace and justice. If we want to change the world, we should procure universal access to contraception without myths or barriers. We have the moral obligation of achieving the irradiation of extreme poverty and advancing the construction of more equal, just, and happy societies. In emergency contraception (EC), we are very far from reaching expectations. If in reversible, long-term methods we have low prevalence, in EC the situation gets worse. Not all faculties in the region look at this topic, and where it is looked at, there is no homogeneity in content, not even within the same country. There are still myths about their real action mechanisms. There are countries, like Honduras, where it is prohibited and there is no specific medicine, the same case as in Haiti. Where it is available, access is dismal, particularly among girls, adolescents, youth, migrants, afro-descendent, and indigenous. The multiple barriers for the effective use of emergency contraceptives must be knocked down, and to work toward that we have to destroy myths and erroneous perceptions, taboos and cultural norms; achieve changes in laws and restrictive rules within countries, achieve access without barriers to the EC; work in union with other sectors; train health personnel and the community. It is necessary to transform the attitude of health personal to a service above personal opinion. Reflecting on what has occurred after the ICPD in Cairo, their Action Program changed how we look at the dynamics of population from an emphasis on demographics to a focus on the people and human rights. The governments agreed that, in this new focus, success was the empowerment of women and the possibility of choice through expanded access to education, health, services, and employment among others. Nonetheless, there have been unequal advances and inequality persists in our region, all the goals were not met, the sexual and reproductive goals continue beyond the reach of many women (15). There is a long road ahead until women and girls of the world can claim their rights and liberty of deciding. Globally, maternal deaths have been reduced, there is more qualified assistance of births, more contraception prevalence, integral sexuality education, and access to SRH services for adolescents are now recognized rights with great advances, and additionally there have been concrete gains in terms of more favorable legal frameworks, particularly in our region; nonetheless, although it's true that the access condition have improved, the restrictive laws of the region expose the most vulnerable women to insecure abortions. There are great challenges for governments to recognize SRH and the DSR as integral parts of health systems, there is an ample agenda against women. In that sense, access to SRH is threatened and oppressed, it requires multi-sector mobilization and litigation strategies, investigation and support for the support of women's rights as a multi-sector agenda. Looking forward, we must make an effort to work more with youth to advance not only the Action Program of the ICPD, but also all social movements. They are one of the most vulnerable groups, and the biggest catalyzers for change. The young population still faces many challenges, especially women and girls; young girls are in particularly high risk due to lack of friendly and confidential services related with sexual and reproductive health, gender violence, and lack of access to services. In addition, access to abortion must be improved; it is the responsibility of states to guarantee the quality and security of this access. In our region there still exist countries with completely restrictive frameworks. New technologies facilitate self-care (16), which will allow expansion of universal access, but governments cannot detach themselves from their responsibility. Self-care is expanding in the world and can be strategic for reaching the most vulnerable populations. There are new challenges for the same problems, that require a re-interpretation of the measures necessary to guaranty the DSR of all people, in particular women, girls, and in general, marginalized and vulnerable populations. It is necessary to take into account migrations, climate change, the impact of digital media, the resurgence of hate discourse, oppression, violence, xenophobia, homo/transphobia, and other emergent problems, as SRH should be seen within a framework of justice, not isolated. We should demand accountability of the 179 governments that participate in the ICPD 25 years ago and the 193 countries that signed the Sustainable Development Objectives. They should reaffirm their commitments and expand their agenda to topics not considered at that time. Our region has given the world an example with the Agreement of Montevideo, that becomes a blueprint for achieving the action plan of the CIPD and we should not allow retreat. This agreement puts people at the center, especially women, and includes the topic of abortion, inviting the state to consider the possibility of legalizing it, which opens the doors for all governments of the world to recognize that women have the right to choose on maternity. This agreement is much more inclusive: Considering that the gaps in health continue to abound in the region and the average statistics hide the high levels of maternal mortality, of sexually transmitted diseases, of infection by HIV/AIDS, and the unsatisfied demand for contraception in the population that lives in poverty and rural areas, among indigenous communities, and afro-descendants and groups in conditions of vulnerability like women, adolescents and incapacitated people, it is agreed: 33- To promote, protect, and guarantee the health and the sexual and reproductive rights that contribute to the complete fulfillment of people and social justice in a society free of any form of discrimination and violence. 37- Guarantee universal access to quality sexual and reproductive health services, taking into consideration the specific needs of men and women, adolescents and young, LGBT people, older people and people with incapacity, paying particular attention to people in a condition of vulnerability and people who live in rural and remote zone, promoting citizen participation in the completing of these commitments. 42- To guarantee, in cases in which abortion is legal or decriminalized in the national legislation, the existence of safe and quality abortion for non-desired or non-accepted pregnancies and instigate the other States to consider the possibility of modifying public laws, norms, strategies, and public policy on the voluntary interruption of pregnancy to save the life and health of pregnant adolescent women, improving their quality of life and decreasing the number of abortions (17).
[ES] Se ha buscado un diálogo de tres ramas del Derecho: Internacional, Constitucional y Procesal con la Economía, en especial con el Neo-Institucionalismo. Además, en algunos aspectos también dialoga con la sociología y con la filosofía. Es, por tanto, un trabajo INTERDICIPLINÁRIO. Contiene 4 Capitulos: i aspectos conceptuales de los derechos humanos de tercera generación; ii el derecho al desarrollo como derecho humano de tercera generación; iii consideraciones generales con respecto al proceso en la modernidad para la comprensión de su misión protectora de los derechos humanos de tercera generación; referencia especial a la protección del derecho al desarrollo; iv la protección judicial de los derechos humanos de tercera generación, en especial referencia al derecho al desarrollo (El Poder Judicial y el Derecho al Desarrollo). Descreve la responsabilidad de Naciones Unidas con el fin de que la humanidad disponga de unas mínimas condiciones de vida que garanticen el respeto de los hombres y ciudadanos en cada país, acciones, a fin de cuentas, dirigidas a la implantación de políticas públicas que favorezcan la dignidad del hombre. No es suficiente el mero reconocimiento positivo de los derechos humanos, sino que es necesario contar con instrumentos normativos procesales que, de forma efectiva, protejan y garanticen los derechos humanos, para así posibilitar al hombre el acceso a los tribunales, la reivindicación de sus derechos y el pleno conocimiento y entendimiento de éstos, por lo que el Poder Judicial juega un papel preponderante en la protección de los derechos humanos. En este tiempo una actuación judicial más humana, relacionada, evidentemente, con la implantación de un proceso más humano. Se subraya esta nueva misión del juez en la pos-modernidad, cuando se reflexiona sobre la visión de la economía neo-institucionalista, que destaca la importancia para el desarrollo socio-económico de las instituciones, de entre las cuales, se destaca, la Justicia. Por consecuente, estas afirmaciones económicas pueden se constituir en un antecedente racional para que se pueda llegar hasta la concepción de una finalidad económica que se reviste el proceso civil, esto es, un escupo económico para el proceso civil. Hay que observar, en la concepción de la Economía Neo-Institucionalista, el nuevo vigor que asume la Economía del Desarrollo. Dada la importancia que para el mercado representan los consumidores e incluso los efectos para la economía y para el desarrollo de un país - y no sólo para el propio mercado, ya que su equilibrio se refleja significativamente en la economía - es fundamental propiciar las condiciones favorables para que el mercado esté estable y en crecimiento. Esto dependerá sin duda, entre otras cuestiones, de la protección legal y judicial que se dé a los consumidores, especialmente la protección a través de las tutelas procesales (tanto individuales como colectivas). El Poder Judicial, con esas tutelas de protección a los consumidores y usuarios ofrecería, indirectamente, su contribución para el desarrollo socio-económico de una sociedad beneficiada con esas tutelas de protección.El proceso civil, en cuanto instrumento que el Estado dispone para hacer Justicia a los ciudadanos, más allá de su función redistributiva y de reintegración social al aplicar normas jurídicas a situaciones factuales, ha asumido, modernamente, una función de cooperador de la economía, ante justamente, el papel adyuvante del desarrollo socio-económico, que la seguridad jurídica puede proporcionar en el contexto social. Al Derecho Procesal Civil deben ser incorporados los más nuevos estudios de la Economía Neoinstitucionalista, en un esfuerzo científico interdisciplinario, para mejor reflexión y búsqueda de soluciones que tornen el sistema de justicia, ágil y eficiente. La justiciabilidad puede ser una vía para favorecer la efectividad de los derechos humanos. Asumimos posición de entre aquellos que defienden que el Derecho al Desarrollo, comprendidos en los derechos sociales, por ejemplo, de educación fundamental, salud básica, asistencia a los desamparados y el derecho de acceso a la Justicia, son integrantes indispensables y con posibilidades de constituirse en un derecho subjetivo individual definitivo. Defendemos, por consiguiente, que así como es perfectamente posible la justiciabilidad de los derechos económicos, sociales y culturales es igualmente posible la justiciabilidad del derecho al desarrollo en el estadio actual de los derechos humanos. Los tribunales y juzgados ya han protegido de forma bastante enfática y en numerosas ocasiones algunos de estos derechos de tercera generación, como son los de los consumidores y del usuario y los de cariz ambientalista, algo no muy diferente al derecho al desarrollo aunque es muy difícil verificar una jurisprudencia correspondiente. Hay que considerar que la defensa de la protección y justiciabilidad de los DESCs ha sido gradualmente admitida en muchos países democráticos, no sólo con sede doctrinaria pero también jurisprudencial. Y de la misma manera, se puede antever que el derecho al desarrollo, poco a poco, va siguiendo el mismo camino de aquellos otros derechos ya plenamente justiciables y protegidos por el Poder Judicial. Seguramente que en actualidad hay una nueva Justicia en Brasil, por los poderes constituidos, han firmado dos pactos republicanos de Estado por un Sistema de Justicia más accesible, ágil y efectivo, es decir, por una Justicia mejor, más democrática y más eficiente, que ha exigido la edición de 18 (dieciocho) nuevas leyes, de nuevas medidas tomadas por el Presidente de la República Brasileña y por el Presidente de la Corte de Justicia mayor, el Supremo Tribunal Federal STF. El Banco Mundial ha querido aprovechar el modelo de desarrollo del STJ para ayudar en la modernización de las cortes de América Latina, Caribe y África, dentro del eje de cooperación conocido como Sud-Sud. Estas cooperaciones con instituciones de países en desarrollo han sido valoradas por este importante organismo internacional de políticas de desarrollo. La creación del CNJ ha comportado en el seno nacional importantes cambios éticos y la modernización de carácter administrativo. Basta citar, como ejemplos éticos, los siguientes: 1º) el término del nepotismo en el Poder Judicial brasileño; 2º) establecimiento de límites en los salarios de jueces, desembargadores y ministros de tribunales superiores y la eliminación de salarios adicionales que aumentaban considerablemente los valores que el Estado pagava a los magistrados. Discrete choice experiments (DCEs) are a popular methodology for eliciting human preferences and willingness-to-pay (WTP) measures. However, there are a number of methodological challenges, such as choice task complexity, hypothetical bias and non-attendance to the choice attributes, which have been examined in the two surveys included in this dissertation. One survey was conducted at the School of Dental Medicine, University of Zagreb, to estimate patients' WTP for dental care delivery at the school clinic and investigate the validity of derived estimates. Among the attributes included in a DCE, treatment explanation was on average the most valued attribute, followed by dental staff behavior, dental care provider and waiting time in the office. As patients often have diverse expectations and preferences for health care interventions, it is important to consider heterogeneity in their preferences when estimating WTP for health care. Mixed logit and latent class analyses detected a large heterogeneity in patients' preferences. Four classes of patients with distinct preferences for dental care were identified. Older and/or more educated patients tended to give relatively less importance to treatment explanation, but were more attentive to interpersonal characteristics. Higher education was also associated with a higher propensity to substitute faculty dental care with private care providers. Different survey design features, such as the opt-out option and the selection of attributes and their levels, may affect the choice behavior of respondents and estimated utility weights. However, not giving an opt-out option to respondents did not affect the preferences for dental care delivery and response error variance. Furthermore, adding the cost attribute to a DCE task did not change the rank order of other attributes and their utility weights did not differ significantly, indicating that respondents were willing to trade off cost with other dental care attributes. However, adding the cost attribute increased the response error variance, which could be best explained by a higher cognitive burden from adding an extra attribute to the choice task. We did not find any strong evidence that the effect of including an extra cost attribute is any different from the expected effect of including any other choice attribute ; therefore, its influence may not be as relevant as some of the previous studies may have suggested. Choice studies have shown that respondents often ignore one or more attributes when making choices and may ignore the cost attribute itself, thus precluding the calculation of marginal WTP values. The contingent valuation (CV) method directly estimates respondents' WTP, thereby avoiding this issue. The DCE method yielded significantly higher welfare estimate for dental care than the CV method. While strategic behavior in direct WTP estimates was not evident, almost every second DCE respondent appears to have ignored the cost attribute, which may explain the discrepancy in welfare estimates between the two methods. The findings suggest that direct elicitation of WTP through the CV method produced more realistic estimates, and raise concerns about the derived WTP measures in DCE studies that did not consider non-attendance to the cost attribute during the estimation process. However, a typical CV study values a good as a whole and offers no information about the value of the individual attributes that comprise a good. To deal with this limitation, the CV task was accompanied by the best-worst choice (BWC) task which provided information used to derive the attribute-specific WTP values from the holistic WTP values estimated through the CV method. The proposed methodology could be a valuable alternative to traditional DCEs when the choice behavior of respondents raises concerns over the validity of estimated welfare measures. Choice contexts often cannot be easily described with a restricted number of attributes, which may lead to the omitted variable bias. On the other hand, asking respondents to evaluate large number of attributes imposes high cognitive burden and stimulates them to simplify the choice task, which may contribute to an increased error variance and affect the validity of utility estimates. Environmental valuation generally involves trade-offs between complex goods and services. How to describe a complex valuation context is an important consideration, and was investigated in a second survey that evaluated the preferences of trail users for different resource, social and managerial conditions in the Medvednica Nature Park. Two different choice experiments that differed in the provision of information about the recreational setting were designed. One DCE used multidimensional attributes to keep the number of choice attributes manageable for respondents, while considering all relevant aspects of visitors' experience. The other DCE used a greater number of more specific, unidimensional, attributes to describe the same recreational setting, but implemented a partial profile design in which only a subset of attributes appeared in each choice set. Perceived difficulty of the choice task, self-reported choice certainty and choice consistency were similar across the two experiments. Heterogeneity in preferences and scale was detected in both experiments. Indications of non-compensatory behavior, and greater error variance among less experienced trail users were found in the partial profile experiment with more specific trail attributes, but not in the experiment with multidimensional attributes. Environmental conditions are often inter-related and as such cannot be simultaneously included in a DCE. A linked structural choice model was designed to deal with the correlated or causally-related DCE attributes as well as with the ambiguity when using multidimensional choice attributes, since respondents might be focused on different aspects of those attributes when making choices. A structural choice model was used to link the two choice experiments and investigate the amount of variability in multidimensional attributes (crowding on trail and intensity of road traffic, evaluated within the DCE) explained by each of its sub-dimensions (i.e. specific crowding-related conditions, evaluated within object case best-worst scaling (BWS) experiment). The findings indicate that visitors' preferences for crowding were mostly influenced by perceptions of 'trail use level' and 'noise from road traffic'. Our approach offers richer information to policy makers than other solutions to the attribute correlation/causality issues in complex choice studies, namely the exclusion of some relevant attributes or simply their aggregation into a single composite attribute. ; Poskusi diskretne izbire so priljubljena metodologija za pridobivanje preferenc ljudi in mer pripravljenost na plačilo. Na Stomatološki fakulteti Univerze v Zagrebu je bila opravljena raziskava, da bi ocenili pacientovo pripravljenost na plačilo za bolnišnično zobozdravstveno oskrbo v šolski kliniki in raziskali veljavnost izpeljanih ocen. Ker imajo pacienti pogosto različna pričakovanja in preference za intervencije zdravstvenega varstva, je pomembno, da pri ocenjevanju PZP za zdravstveno varstvo upoštevamo heterogenost v njihovih preferencah. Starejši in/ali bolj izobraženi pacienti so dajali sorazmerno manjši pomen razlagi zdravljenja, vendar so bili bolj pozorni na medosebne značilnosti. Visokošolska izobrazba je bila povezana tudi z večjo nagnjenostjo k nadomestitvi fakultetne zobozdravstvene oskrbe z zasebnimi ponudniki. Različne značilnosti zasnove raziskave, kot so možnost izključitve in izbira atributov ter njihovih ravni, lahko vplivajo na način izbire anketirancev in ocene parametrov koristnosti. Vendar pa pri anketirancih, ki opcije možnosti izključitve niso imeli, to ni vplivalo na preference v ponudbi zobozdravstvene oskrbe in na nepojasnjeno varianco modela. Poleg tega dodajanje stroškovnega atributa v poskusu diskretne izbire ni spremenilo vrstnega reda drugih atributov in njihove uteži koristnosti se niso bistveno razlikovale, kar pomeni, da so bili anketiranci pripravljeni zamenjati stroške z drugimi atributi zobozdravstvene oskrbe. Vendar pa je dodajanje stroškovnega atributa povečalo nepojasnjeno varianco modela, kar bi lahko najbolje pojasnili z višjo kognitivno obremenitvijo zaradi prisotnosti dodatnega atributa k izbirni nalogi. Nismo našli trdnega dokaza, da je učinek vključitve dodatnega stroškovnega atributa drugačen od pričakovanega učinka pri vključitvi kateregakoli atributa izbire, zato ta vpliv morda ni tako pomemben, kot so predlagale nekatere prejšnje študije. Zasnovani sta bili dve različni izbirni nalogi, ki sta se razlikovali v zagotavljanju informacij o rekreativnem okolju. En poskus diskretne izbire je uporabil večrazsežne atribute, da bi število atributov za anketirance ostalo obvladljivo, ob upoštevanju vseh pomembnih vidikov izkušenj obiskovalcev. Drugi poskus diskretne izbire je uporabil več specifičnih, enorazsežnih atributov, ki opisujejo enako rekreativno okolje, vendar je vključil delno profilno zasnovo, v kateri se v vsakem izbirnem setu pojavlja le podskupina atributov. Opažena težavnost naloge izbire, samo-poročana gotovost izbire in doslednost izbire so bili podobni v obeh poskusih. Heterogenost v preferencah in obsegu je bila ugotovljena v obeh poskusih. Indikacije nekompenzacijskega vedenja in večja nepojasnjena varianca med manj izkušenimi uporabniki poti so bili najdeni v poskusu delnega profila z bolj specifičnimi atributi, ne pa v poskusu z večrazsežnimi atributi. Okoljski pogoji so pogosto medsebojno povezani in kot taki ne morejo biti istočasno vključeni v poskus diskretne izbire. Povezani model strukturne izbire je bil zasnovan tako, da se ukvarja s (vzročno) povezanimi atributi, kot tudi z večpomenskostjo pri uporabi večrazsežnih atributov izbire, saj se lahko anketiranci med izbiranjem osredotočajo na različne vidike teh atributov. Za povezavo dveh izbirnih poskusov smo uporabili model strukturne izbire in raziskali obseg spremenljivosti v večrazsežnih atributih (gneča na poti in intenzivnost cestnega prometa, ocenjeno v poskusu diskretne izbire), ki ga razlaga vsaka od njegovih razsežnosti (to so specifične razmere, povezane z gnečo, ocenjevane v poskusu najboljšega-najslabšega lestvičenja). Ugotovitve kažejo, da so bile preference obiskovalcev glede gneče pod vplivom percepcije "ravni uporabe poti" in "hrupa od prometne ceste". Naš pristop ponuja bolj bogate informacije oblikovalcem politike od drugih rešitev glede problematike korelacije/vzročnosti med atributi pri kompleksnih poskusih izbire, in sicer izključitev nekaterih relevantnih atributov ali preprosto njihovo združevanje v en večrazsežni atribut.
Como se había mencionado en la editorial anterior (López-López, 2015), el libro de Suber (2015) es una contribución significativa para aclarar los límites y alcances del acceso abierto (OA) y permite decantar confusiones e impresiones que prevalecen en nuestras comunidades que no están preparadas para los cambios tan acelerados que las nuevas dinámicas de producción y comunicación académica están generando (Gallegos, Berra, Benito, & López-López, 2014). Suber (2015) plantea como el acceso abierto surge bajo un conjunto de motivaciones que influyeron en su desarrollo. Estas motivaciones son: 1. El incremento desmedido de los precios de las revistas científicas con lo cual se ha dificultado el acceso al conocimiento, generando mayor inequidad y profundizando además paradojas perversas como que en países pobres se produce conocimiento al que luego no puede accederse por no poder pagar las revistas donde se publica. Este punto es crítico para las sociedades o las bibliotecas que cuentan con presupuestos limitados para acceso a bases de datos o países que en consorcio buscan comprar colecciones relevantes para sus infraestructuras de conocimiento; las brechas generadas por los costos afectan a todos los investigadores y más a los que tiene mayores restricciones económicas ampliando la inequidad y brechas, disminuyendo la posibilidad de construir redes de colaboración por las asimetrías generadas. Es bueno anotar que el incremento de precios suele ser mayor en forma considerable que los indicadores de inflación y los presupuestos de las instituciones. 2. Las grandes editoriales suelen vender paquetes de revistas (Big deals) de múltiples calidades impidiendo así que se negocie en relación con las revistas que son necesarias estratégicamente por una institución y se suele castigar con un precio las cancelaciones a títulos que son de baja circulación o calidad. Y peor aún, las casas editoriales suelen suscribir cláusulas de confidencialidad en relación con los precios y las condiciones de negocio lo que impide tener negociaciones con más transparencia sobre la compra de los mencionados paquetes incluso con instituciones cercanas. 3. Las restricciones por precio generan limitaciones de acceso pues se impide la reproducción de los contenidos a todos los niveles, desde la formación terminando en la investigación propiamente dicha. Además ni siquiera hoy las bibliotecas pueden ser dueñas de lo que han pagado en el pasado, hoy estas solo pueden "arrendar" la información y se impide la migración de contenidos anteriores así se haya pagado por ellos. 4. Un agravante adicional es que en el mundo entero los investigadores ceden los derechos de publicación a las editoriales y estas se quedan con la propiedad de los contenidos. Éstos deben ser adquiridos por altos costos por los propios investigadores, sus instituciones y bibliotecas e incluso por los organismos financiadores. La mayor parte de la financiación de la investigación se genera de entidades estatales con dinero público, incluso, en universidades privadas aunque éstas los financien con dinero privado es absurdo que ellas mismas no puedan pagar de nuevo las cifras desproporcionadas que cobran algunas de estas editoriales. 5. Estas empresas de intermediación de conocimiento provocan además escasez artificial al restringir los accesos debido a los costos. Es así como los modelos de acceso abierto evidentemente pueden disminuir los costos o por lo menos tratar de tenerlos bajo la inflación o bajo los costos reales razonables, incluida la utilidad de producción. Pero es claro como se dijo antes que, en cualquier forma, la investigación que se financia con recursos públicos debe tener una condición de acceso como bien público y es por esta intervención directa que no es razonable pretender que el acceso abierto opere "interfiriendo el libre mercado". 6. Las revistas de acceso abierto están compitiendo en primer lugar por autores y artículos de la mejor calidad y en segundo lugar están interesadas en que esta calidad se refleje en diversos usos y no pretende competir o interferir en los mercados de las revistas de pago. Es por esto que el OA busca en forma prioritaria que la evaluación por pares garantice la calidad de los contenidos. Incluso muchas revistas de pago pueden incrementar sus márgenes de ganancia con el incremento de las tasas de rechazo. 7. La falta de conocimiento sobre lo que es el acceso abierto e incluso los proceso de comunicación y circulación del conocimiento por parte de investigadores, bibliotecarios y diversos actores de las cadenas de producción y circulación de conocimiento, suele ser usado por algunas de estas empresas para incrementar costos e incluso en incurrir en asuntos éticos. 8. Todas las estimaciones de crecimiento de la producción científica muestran que al parecer el negocio de las editoriales y revistas privadas seguirá creciendo con la consecuente imposibilidad de acceder al contenido bajo los modelos de negocio y no solo es necesario sino conveniente que el acceso abierto se desarrolle y crezca. 9. Desde el 2004, anota Suber (2015), Thomson Scientific identificó que es sus diferentes temáticas entre las revistas de más impacto hay buen crecimiento constante de revistas de Acceso Abierto lo cual es muy relevante por cuanto estás apenas están arrancando y deben competir con revistas de muy larga trayectoria. Por último, es necesario diferenciar los repositorios de acceso abierto de las revistas y documentos que pasaron por revisión de pares. Es necesario aclarar que hoy los repositorios incluyen todo tipo de contenidos y algunos han denominado "depósitos oscuros" que no exigen la evaluación de pares. Debido a esto se puede diferenciar en OA oro y OA verde. El primero se refiere al acceso abierto de las revistas sin tener en cuenta el modelo de negocio y el OA verde al que es facilitado por los repositorios y se entiende como un "auto archivo a la práctica de depositar un trabajo propio en un repositorio de OA". Estos términos permiten diferenciar el tipo de acceso abierto y las consecuencias de cada uno y es probable que se introduzcan nuevos términos que permitan diferenciar y aclarar más el acceso abierto. En definitiva el libro de Suber (2015) será un recurso obligado de consulta para quienes sigamos comprometidos con el acceso abierto, porque entendemos las implicaciones políticas y económicas de inclusión y democratización de acceso al conocimiento que tienen en especial países como el nuestro en el que la inversión en ciencia y educación es escaso. ; As mentioned in the previous editorial (López-López, 2015), Suber's (2015) book is a significant contribution that we can use to clarify the limits and the scope of Open Access (OA), and it is also helpful in explaining confusions and inaccuracies that are widespread in our communities, which are not quite ready to face the accelerated changes that are being generated by the new dynamics of scientific output and communication (Gallegos, Berra, Benito, & López-López, 2014). Suber (2015) states that OA emerges in the context of motivating circumstances. These are: 1) The overwhelming increase in the fees of scientific journals, which has made access to knowledge more difficult. This has created more inequality and has deepened perverse paradoxes, such as the fact that poor countries produce knowledge that they cannot later access due to the inability to pay those exorbitant fees. This issue is critical for societies or libraries that have limited budgets to pay for access to databases, or countries looking to purchase relevant collections for their knowledge infrastructures, in association. The gaps created by high costs have an effect on the whole research community, and more so on those with worse economic restrictions. The resulting asymmetries make it harder to build collaboration networks. It is worth noting that the increase in those fees is often (much) higher than inflation indicators and the corresponding increases in institutional budgets. 2) The main publishing houses often sell journal packages (Big deals), consisting of journals of varying quality. This prevents negotiation of the journals that are strategically necessary for an institution, and cancelling subscriptions to low quality or low circulation journals is often punished with a premium. Even worse, publishers often subscribe confidentiality clauses with regards to prices and business conditions, which fosters a lack of transparency in the purchase of the aforementioned packages. 3) Price restrictions create access limitations, because the reproduction of contents is forbidden at every level – from basic training all the way to research proper. Besides, nowadays not even libraries own what they have paid for in the past. They can only "rent" the information, and migration of previous content is prevented, even if it has been purchased in the past. 4) An additional problem is that researchers transfer their publication rights to the publishing houses and these keep property rights over the contents. These contents must then be purchased by researchers, their institutions, libraries and even supporting entities, at a high cost. Most funding for research is provided by government/state institutions with public monies, and sometimes not even private universities are able to afford the disproportionate fees charged by some of these publishers. 5) These middlemen companies also create artificial scarcity by restricting access due to costs. As such, OA models can reduce those fees or at least they can try to keep them under inflation or real reasonable costs, including profit. But clearly, as said before, it should be possible to access research funded with public resources in the same way other public goods are accessed. For this reason, it is not reasonable to say that OA operates by "interfering with the free market". 6) OA journals are competing for top quality authors and papers, and they are interested in getting this quality to reflect on different usages – they do not intend to compete or interfere with the paid journal market. Therefore, OA seeks that peer-review processes can guarantee the quality of its content. Many paid journals can actually increase their profit margins by increasing rejection rates. 7) A lack of knowledge of OA and even communication and circulation processes on the part of researchers, librarians and other actors in the production and circulation chains is often used by some of these companies to increase costs and even to engage in unethical practices. 8) All estimations of scientific output growth seem to show that the publishing business will continue to grow, with the corresponding impossibility of accessing contents under the current business model. It is therefore convenient and necessary that OA continues to develop and grow. 9) Since 2004, according to Suber (2015), Thomson Scientific identified a continued growth of OA journals in terms of impact, which is relevant since they are still young and must compete with journals with longer lifespans. Finally, we need to differentiate OA and peer-reviewed document repositories. Nowadays, repositories cover all sorts of content, and some which do not require peer review are called "dark repositories". Gold OA, therefore, is Open Access with no regard for the business model, and Green OA is a self-archiving practice of placing one's work into an OA repository. These terms enable us to differentiate the types of OA and their associated consequences – more such terms may be introduced in the future to clarify different models. In summary, Suber's (2015) book will be a mandatory resource for those who are still committed to OA, because we understand the political and economical implications of inclusion and democratization of access to knowledge, especially in countries such as ours, where investment in science and education is scarce.
Le Québec connaît une augmentation importante des immigrants, qui est en croissance surtout en dehors de Montréal (ISQ, 2017; Bakhshaei, 2015). Les écoles se trouvent donc au centre des débats relatifs aux différents enjeux inhérents à la diversité. Ils demeurent en manque des « pratiques interculturelles » qui sont davantage enracinées dans le milieu scolaire montréalais qu'ailleurs dans la province. De plus, les écrits de régulation n'adoptent pas la question de la diversité scolaire telle qu'elle est représentée aujourd'hui dans les écoles. Par ailleurs, différentes recherches sur l'échelle international soulignent que les élèves immigrants sont susceptibles d'échouer ou d'obtenir de faibles résultats scolaires comparés aux jeunes non immigrants (Kamanzi et al., 2016). Ainsi, la direction d'école doit, plus que jamais, exercer son leadership pour soutenir une bonne collaboration entre les acteurs scolaires et sociaux. Il s'agit de garder le cap sur la réussite éducative de tous les élèves. L'ampleur de cette réalité exige une collaboration de la part de l'école, des familles immigrantes et de la communauté. L'objectif général de cette thèse consiste à comprendre et à analyser les pratiques de collaboration école-familles immigrantes-communauté (ÉFIC). Cet objectif général se décline en trois objectifs spécifiques qui font objet de trois publications. - Le premier objectif spécifique consiste à identifier et à décrire les pratiques de collaboration que les membres de l'organisme communautaire Centre Multiethnique de Québec (CMQ) mettent en place pour favoriser l'intégration des familles immigrantes ainsi que l'intégration et la réussite scolaire et éducative de leurs enfants. Cet objectif fait l'objet d'un chapitre de livre, publié dans l'ouvrage Immigration et nouvelles vies aux Presses de l'Université Laval. Deux concepts soutiennent cette étude qualitative, soit l'accompagnement et le leadership. Pour réaliser cette recherche, une étude de documents a été menée. Il s'agit de 13 documents publiés par ledit centre, s'étalant entre 2015 et 2018. L'analyse de ces documents a permis de dégager la présence des pratiques en lien avec l'accompagnement individuel et collectif des familles nouvellement arrivées et de leurs enfants. De plus, il s'est avéré que le CMQ ne cesse de fournir des efforts pour satisfaire leurs attentes et leur procurer un sentiment de confiance et de sécurité (formations, financements, nouveaux services, etc.). - Le deuxième objectif spécifique consiste à repérer et à analyser les pratiques de collaboration ÉFIC et les types de lien de cette collaboration pour favoriser l'intégration et la réussite scolaire et éducative des élèves immigrants. Cet objectif fait l'objet d'un article paru dans la revue Creative Education. Le cadre conceptuel de cette étude qualitative se base sur l'approche mésosystémique du modèle de Bronfenbrenner (1979) et sur les six dimensions de l'approche de l'influence partagée d'Epstein (2001). Le corpus analysé, dégagé d'entrevues semi-dirigées auprès d'acteurs scolaires (n=11), de membres communautaires (n=10) et de familles immigrantes (n=6), a permis d'identifier deux dimensions primordiales à prendre en compte, soit le leadership et l'enfantalité, un mot qui désigne la nécessité de prendre en compte l'expérience et les besoins de l'élève immigrant dans la dimension de collaboration. Nos analyses mettent également en évidence, d'une part, la présence de certaines pratiques telle que la communication entre l'ensemble des acteurs; et, d'autre part, l'absence d'autres pratiques telle que la prise des décisions des familles immigrantes. Nos résultats appuient donc la nécessité de fournir plus de ressources et d'investissements de la part de toutes les instances, surtout le gouvernement. - Le troisième objectif spécifique a pour objectif de comparer les pratiques de collaboration ÉFIC selon les acteurs scolaires et sociaux ayant répondu à un questionnaire (12 acteurs scolaires, 24 membres communautaires et 19 familles immigrantes). Cet objectif fait l'objet d'un article publié par la revue Canadian Journal of Educational Administration and Policy. Les six dimensions du modèle de l'influence partagée d'Epstein (2001) forment le cadre théorique de cette étude quantitative. Au terme de cette recherche, d'une part, les analyses de Kruskall Wallis ont montré une différence significative entre les trois groupes d'acteurs quant aux pratiques de prise de décision et d'apprentissage à la maison. D'autre part, les analyses de corrélation ont montré que 1) le nombre d'années d'expérience des acteurs scolaires à l'école est corrélé positivement avec les variables communication, bénévolat, parentalité et prise de décision et que 2) le niveau de classe du jeune est corrélé positivement avec la variable parentalité. Finalement, un livret pratico-pratique, intitulé École-familles immigrantes-communauté : outils de collaboration en 42 pratiques et 255 actions clés, publié par les Presses de l'Université Laval, est rédigé en vue de rendre disponibles les pratiques de collaboration ÉFIC. Sa réalisation est basée sur les écrits et les témoignages de différents acteurs scolaires et sociaux par le biais des entrevues semi-dirigées menées dans cette thèse (échantillon de l'article 2). En premier lieu, le livret expose, entre autres, quelques mythes de la collaboration ÉFIC, ses leviers et freins ainsi que différents moyens de communication permettant de faciliter l'échange entre les acteurs. En second lieu, le livret suggère 44 pratiques efficaces et 255 actions clés destinées à chacun des acteurs concernés, soit les acteurs scolaires, la famille immigrante et les membres de la communauté. ; Quebec is experiencing a significant increase in immigrants, especially outside the metropolis (ISQ, 2017; Bakhshaei, 2015). Schools are, therefore, at the center of debates relating to the various issues inherent in diversity. However, they still lack "intercultural practices," which are more deeply rooted in the Montreal school environment than elsewhere in the province. In addition, regulatory writings do not adopt the issue of school diversity as it is represented today in schools. In addition, various research on the international scale underlines that immigrant students are likely to fail or obtain poor academic results compared to non-immigrant youth (Kamanzi et al., 2016). Thus, more than ever, the school administration must exercise its leadership to support good collaboration between school and social actors. It is about staying focused on the educational success of all students. The magnitude of this reality requires collaboration on the school, immigrant families and the community. The general objective of this thesis is to analyze the practices of school-immigrant families-community (ÉFCI) collaboration. Three specific purposes form this general objective, subject to three publications. - The first specific objective consists of identifying and describing the collaborative practices that the members of the community organization Centre Multiethnique de Québec (CMQ) put in place to promote the integration of immigrant families and academic and educational success of their children. This goal is the subject of a book chapter published in Immigration et Nouvelles vies at Presses de l'Université Laval. Two concepts support this qualitative study, support and leadership. We realized a document study to carry out this research. These are 13 documents published by the said center, spread out between 2015 and 2018. The analysis of these documents made it possible to identify the presence of practices linked to the individual and collective support of newly arrived families and their children. In addition, it turned out that the CMQ never stops making efforts to meet their expectations and give them a feeling of confidence and security (training, funding, new services, etc.). - The second specific objective is to identify and analyze ÉFIC collaboration practices and the types of links of this collaboration to promote immigrant students' academic and educational success. The conceptual framework of this qualitative study is the approach of the bioecological model of Bronfenbrenner (1979) and the six dimensions of the shared influence approach of Epstein (2001). The analyzed corpus, drawn from semi structured interviews with school stakeholders (n = 11), community members (n = 10) and immigrant families (n = 6), identified two essential dimensions to be considered the leadership and the childishness. This word indicates the need to consider the immigrant student's experience and needs in the dimension of collaboration. On the one hand, our analyses highlight specific practices, such as communication between all stakeholders. But, on the other hand, the absence of other practices such as the decision-making of immigrant families. Therefore, our results support the need for more resources and investments from all levels, especially the government. - The third specific objective aims to compare the EFIC collaboration practices according to the school and social actors who answered a questionnaire (12 school actors, 24 community members and 19 immigrant families). This goal is the subject of an article published by the Canadian Journal of Educational Administration and Policy. The six dimensions of Epstein's (2001) shared influence model form the theoretical framework for this quantitative study. At the end of this research, on the one hand, the analyzes of Kruskall Wallis showed a significant difference between the three groups of actors in terms of decision-making and learning practices at home. However, on the other hand, correlation analyzes have shown that 1) the number of years of experience of school actors in school is positively correlated with the variables communication, volunteering, parenthood and decision-making and that 2) the level class of the young is positively correlated with the parenthood variable. Finally, a practical booklet, named École-familles immigrantes-communauté : outils de collaboration en 42 pratiques et 255 actions clés, published by the Presses de l'Université Laval, is written to make the EFIC collaborative practices available. The writings and testimonies of various educational and social actors through the semi structured interviews conducted in this thesis (sample of article 2) are the basis of its production. First, the booklet exposes, among other things, some myths of the ÉFIC collaboration, its levers and brakes, and various means of communication, making it possible to facilitate the exchange between the actors. Second, the booklet suggests 44 effective practices and 255 actions intended for each of the actors concerned: school actors, the immigrant family, and community members.
L'articolo non vuole affrontare il più vasto tema di "fascismo e biblioteche", la politica bibliotecaria e le realizzazioni del ventennio in questo campo, ma si propone lo scopo di iniziare una ricostruzione della presenza e delle posizioni dei bibliotecari in questa fase della storia della società italiana. Il periodo fascista è un periodo di modernizzazione tecnica delle biblioteche italiane ma anche di irrigidimento del sistema bibliotecario italiano. Nasce in questo periodo una rappresentanza professionale dei bibliotecari (l'Associazione dei bibliotecari italiani, nel 1930) e qualche anno prima era stato costituito un vertice amministrativo (la Direzione generale delle accademie e biblioteche, nata nel 1926 e rimasta sostanzialmente la stessa fino ad oggi) che agiva come filtro fra la politica e la professione.La presa del fascismo tra i bibliotecari italiani, negli anni intorno alla Marcia su Roma, è molto limitata. Molti liberali considerarono il fascismo come un "male minore" rispetto alle tensioni sociali del 1919-1920, ma sono pochissimi e di scarsa importanza i bibliotecari che aderirono al fascismo prima della Marcia su Roma, come il conte Giuseppe Lando Passerini (1858-1932), bibliotecario alla Nazionale di Firenze e alla Laurenziana, o Antonio Toschi, bibliotecario a Bologna. Nessuna personalità importante del mondo delle biblioteche aderì al Manifesto degli intellettuali del fascismo (1925) scritto da Giovanni Gentile; pochi sono anche gli aderenti alla risposta preparata da Benedetto Croce, ma fra questi troviamo Emidio Martini, direttore della Biblioteca nazionale di Napoli in pensione.Tra gli esponenti del Partito Fascista troviamo alcuni direttori di biblioteca, come Italo Lunelli (1891-1960) direttore della Biblioteca comunale di Trento e Leonardo D'Addabbo (1893-1958) direttore della Biblioteca consorziale di Bari, che però non ebbero un ruolo significativo nella professione. Il personaggio più interessante è Piero Zama (1886-1984), fondatore del Partito fascista a Faenza e direttore della Biblioteca comunale della città dal 1920 al 1957. Zama però si staccò dal fascismo per il suo carattere reazionario e venne poi perseguitato.Le biblioteche furono spesso, invece, dei rifugi relativamente tranquilli per le persone contrarie al Fascismo. Alla Biblioteca Vaticana lavorarono Gerardo Bruni (1896-1975) e Igino Giordani (1894-1980), che avevano collaborato con don Sturzo nel Partito popolare e che furono mandati dalla Biblioteca a studiare biblioteconomia in America, nel 1927. Più tardi lavorò alla Vaticana anche Alcide De Gasperi, presidente del Consiglio dopo la Liberazione. Nelle biblioteche statali venivano spesso destinati insegnanti di liceo e professoni universitari antifascisti che il Regime voleva togliere dall'insegnamento: per esempio Bianca Ceva ed Elena Valla alla Biblioteca nazionale di Milano, il filosofo Giuseppe Rensi alla Biblioteca universitaria di Genova e Pilo Albertelli (1907-1944), eroe della Resistenza, alla Biblioteca nazionale di Roma.Nel periodo fascista venne costituita, dopo il Congresso mondiale delle biblioteche e di bibliografia tenuto nel 1929 a Roma e Venezia, l'Associazione dei bibliotecari italiani (dal 1932 Associazione italiana per le biblioteche), controllata dal Ministero dell'educazione nazionale ma indipendente dal Partito. Il Partito fascista costituì una propria Sezione Bibliotecari nell'Associazione fascista del pubblico impiego e poi nell'Associazione fascista della scuola: queste associazioni ebbero larghe adesioni, per i vantaggi che offrivano, ma non svolsero attività significative nel campo delle biblioteche. La relativa autonomia dell'AIB dalla pressione del Fascismo fu resa possibile dal prestigio del presidente, l'uomo politico e professore Pier Silverio Leicht, e dalla Direzione generale delle accademie e biblioteche, che gestiva il settore delle biblioteche limitando per quanto possibile l'ingerenza politica e ideologica.I direttori delle biblioteche statali che non erano favorevoli al fascismo restarono di solito ai loro posti, ma negli anni Trenta la tessera del Partito nazionale fascista diventò necessaria per i funzionari dello Stato e alcuni bibliotecari antifascisti vennero destituiti, come Pietro Zorzanello dalla Biblioteca Palatina di Parma nel 1934 e Anita Mondolfo dalla Biblioteca nazionale di Firenze nel 1937. Nel 1938 vennero licenziati dallo Stato i bibliotecari ebrei. Parecchi bibliotecari antifascisti preferirono prendere la tessera del PNF e rimanere ai propri posti, dove potevano operare per le biblioteche e, dalla fine degli anni Trenta, per la loro protezione dai rischi e dai danni della guerra.Dal 1934 nei congressi dell'Associazione italiana biblioteche diventò obbligatorio portare la camicia nera, divisa del Partito fascista, ma le fotografie della sala del convegno nel 1934 e nel 1940 mostrano che solo pochi bibliotecari la indossavano, o indossavano l'uniforme degli impiegati dello Stato introdotta nel 1938, mentre la maggioranza continuava a indossare i propri abiti borghesi. La fascistizzazione del mondo delle biblioteche fu soprattutto burocratica e rituale, imposta dall'esterno ma limitata ai discorsi ufficiali nei congressi e sulla rivista del Ministero, non incise in maniera rilevante sulla cultura dei bibliotecari, che cercarono di contrastarla in maniera coperta o di ignorarla. ; The article does not try to deal with the more extensive theme of "fascism and libraries", library policy and the achievements of the fascist regime in this field, but aims at a understanding of the presence and positions of librarians in this stage of the history of Italian society. The fascist period is one of technical modernization of Italian libraries but also of fixation of the Italian library system. This period sees the birth, in 1930, of a professional representation of librarians (the Association of Italian librarians), and a few years earlier of a top government unit, the General Direction of Academies and Libraries (established in 1926 and still basically the same to this day), that acted as a filter between politics and the profession. The grasp of fascism among Italian librarians, in the years around the March on Rome (1922), was very limited. Many liberals considered fascism as a "lesser evil" with respect to the social tensions of 1919-1920, but the librarians who supported fascism before the March on Rome were few and of little importance. Among these were count Giuseppe Lando Passerini (1858-1932), librarian at the National Library of Florence and at the Laurenziana, and Antonio Toschi, librarian in Bologna. Not one important personality of the library world supported the Manifesto of the intellectuals of fascism (1925) written by Giovanni Gentile; few were also the supporters of the reply drafted by Benedetto Croce, but among these we find Emidio Martini, retired director of the National Library of Naples.Among the exponents of the Fascist Party we find some library administrators, such as Italo Lunelli (1891-1960) director of the Public Library of Trent and Leonardo D'Addabbo (1893-1958) director of the Consortium Library of Bari, who however did not have a significant role in the profession. The most interesting personality is Piero Zama (1886-1984), founder of the Fascist Party in Faenza and director of the Municipal Library of the city from 1920 to 1957. Zama, however, abandoned fascism because of his reactionary evolution and was subsequently persecuted.Libraries were often a sort of hideout for those contrary to fascism. Gerardo Bruni (1896-1975) and Igino Giordani (1894-1980), who had worked with don Sturzo in the Popular Party, were sent by the Vatican Library to study librarianship in America, in 1927, and later also Alcide De Gasperi, president of the Council of Ministers after the Liberation, worked in the Vatican Library. Anti-fascist high school teachers and university professors that the regime wanted to remove from teaching were often destined to state libraries: for example Bianca Ceva and Elena Valla to the National Library of Milan, the philosopher Giuseppe Rensi to the University Library of Genoa and Pilo Albertelli, Resistance hero, to the National Library of Rome.After the World Congress of Libraries and Bibliography held in Rome and Venice in 1929, the Association of Italian Librarians (from 1932 the Italian Association for Libraries, AIB) was founded, under the control of the Minister for National Education but independent of the Fascist Party. The Fascist Party formed its own Librarians' Section in the Fascist Association of Civil Servants and later in the Fascist School Association: these Associations were widely supported, due to the advantages that they offered, but they carried out no significant activities in the library field. The relative independence of the AIB from the pressure of Fascism was made possible through the prestige of its president, the politician and professor Pier Silverio Leicht, and through the General Direction of Academies and Libraries, that controlled the library sector and limited as much as possible any political and ideological interference.The directors of state libraries who were not in favour of fascism usually remained in their positions, but in the 1930s the membership card of the National Fascist party became necessary for civil servants and some anti-fascist librarians lost their posts. Among these were Pietro Zorzanello, director of the Palatine Library of Parma, in 1934 and Anita Mondolfo, director of the National Library of Florence, in 1937. Jewish librarians were dismissed by the State in 1938. Many anti-fascist librarians preferred to take out a membership card of the National Fascist Party and remain in their positions, where they were able to work for libraries and, from the end of the 1930s, for their protection from the risks and dangers of the war.From 1934 it became obligatory to wear a black shirt, the uniform of the Fascist Party, in the national conferences of the Italian Library Association, but photographs of the convention hall in 1934 and 1940 show that only a few librarians wore it. A number wore the uniform of the civil service, introduced in 1938, but the majority continued to wear their own civilian clothes. The fascistization of the library world was above all bureaucratic and ritual, imposed from the outside but limited to official speeches in congresses and on the Ministry journal. It did not leave much of a mark on the culture of the librarians, who sought to counter it in a veiled manner or at least to ignore it.
The article offers a historiographical consideration of the description of the Khotyn battle 1621with an emphasis on the historical value of M. Kostin's "Chronicle of the Land of Moldova" in the coverage of theevent. The relevance of the study is dictated by the 400th anniversary of the battle. The purpose of the study is to clarify the historical significance of thechronicle in revealing the theme of the Battle of Khotyn in 1621. Methodology describes the work of M. Kostin, its source base, comparison with other historical sources, review of its use. Conclusions. "Chronicle of the Land of Moldova" by Myron Kostin, being the only source for studying the history of Moldova in the early and mid seventeenth century. at the same time it is a true description of the events of the Khotyn War (1620–1621). Although the chronicler is not a contemporary of the events, his account of the Battle of Khotyn is based on serious Polish sources of the relevant era, oral information from contemporaries of the fighting, the traditions of the princely family of Movileshty, and so on. The work is positively characterized by a deep and detailed description of events, impartiality, clear analysis of military operations of a participant in many battles, education of the author and his broad outlook, which allowed him to understand the causal links of actions, actions of the parties, unconditional literary talent M. Kostin. His disregard for the events of the Khotyn War of 1621 by a number of Ukrainian historians shows a reluctance to consider the event comprehensively, using all sources. Moreover, we see a focus on showing only one fragment of this historical event, the participation of the Ukrainian side in it. The language barrier to the use of the Chronicle is no more a problem than a political motive. ; L'article propose une revue historiographique de la description de la guerre de Khotyn de 1621 en mettant l'accent surla valeur historique de la «Chronique de la terre moldave» de M. Kostin à la lumière des événements. Pertinencenla recherche est dictée par le 400e anniversaire de la bataille. Le but de l'étude est de déterminer le poids historique de la chronique pour la divulgation de la bataille de Khotyn de 1621 par ceux-ci.Méthodologie : analyse de l'œuvre de M. Kostin, ses sources, comparaison avec d'autres sources historiques, un aperçu de son utilisation. Conclusion. «Chronique de la terre moldave» de Miron Kostin, étant la seule source pour étudier l'histoire de la Moldavie au début et au milieu du XVIIe siècle. est en même temps une description fiable des événements de la guerre de Khotyn (1620-1621). Bien que le chroniqueur ne soit pas un contemporain des événements, sonl'histoire de la bataille de Khotyn est basée sur des sources polonaises sérieuses de l'époque correspondante, des informations orales de participants à la guerre, les traditions de la famille princière Movila, etc. Caractérise positivement le travail de profondeur et de détail description des événements, objectivité, une analyse claire des opérations militaires d'un participant à de nombreuses batailles, l'éducation de l'auteur et de son large perspective, qui lui permet de comprendre la relation causale des actions des parties, une inconditionnelle littéraire Le talent de M. Kostin. Son ignorance dans la divulgation du thème de la guerre de Khotyn de 1621 par un certain nombre d'historiens ukrainiens montre une réticence à considérer l'événement de manière exhaustive en utilisant toutes les sources. Il y a de fortes chances que nous voyions concentration à ne montrer qu'un fragment de cet événement historique, la participation de la partie ukrainienne à celui-ci. La barrière de la langue à l'utilisation de la Chronique n'est pas un problème plus important que le motif politique. ; В статье предлагается историографический обзор описания Хотинской войны 1621 г. с акцентом на исторической ценности "Летописи Земли Молдовской" М. Костина в ракурсе освещении событий. Актуальность исследования диктуется 400-летним юбилеем битвы. Целью исследования является определение исторического веса летописи для раскрытия теми Хотинской битвы 1621 г. Методология: анализ произведения М. Костина, его источников, сравнение с другими историческими источниками, обзор его использования. Выводы. "Летопись Земли Молдовской" Мирона Костина, будучи единственным источником для изучения истории Молдовы начала и середины ХVII в. одновременно является достоверным описанием событий Хотинской войны (1620–1621 гг.). Хотя летописец не современник событий, однако его рассказ о Хотинской битве основывается на серьезных польских источниках соответствующей эпохи, устной информации от участников войны, традициях княжеской семьи Мовилэ и др. Позитивно характеризует произведение глубокое и детальное описание событий, объективность, четкий анализ военных операций участника многих битв, образованность автора и его широкий кругозор, что позволяет ему понимать причинно-следственную связь поступков сторон, безусловный литературный талант М. Костина. Ее игнорирование в раскрытие темы Хотинской войны 1621 г. целым рядом украинских историков показывает нежелание рассмотреть событие комплексно с использованием всех источников. Скорее всего, мы видим концентрацию на показе только одного фрагмента этого исторического события, участия в нем украинской стороны. Языковый барьер для использования "Летописи" не является большей проблемой, чем политический мотив. ; У статті пропонується історіографічний огляд опису Хотинської війни 1621 року з акцентом на історичні цінності "Літопису Землі Молдовської" М. Костіна в ракурсі висвітленні подій. Актуальність дослідження диктується 400-річним ювілеєм битви. Метою дослідження є визначення історичної ваги літопису для розкриття теми Хотинської битви 1621 р. Хоча літописець сучасник подій, проте його розповідь про Хотинську битву грунтується на серйозних польських джерелах відповідної епохи, усної інформації від учасників війни, традиціях княжої сім'ї Мовіле і ін. Позитивно характеризує твір глибокий і детальний опис подій, об'єктивність, чіткий аналіз військових операцій учасника багатьох битв, освіченість автора і йогоширокий світогляд, що дозволяє йому розуміти причинно-наслідковий зв'язок вчинків сторін, безумовний літературний талант М. Костіна. Її ігнорування в розкритті теми Хотинської війни 1621 р. цілою низкою українських істориківпоказує небажання розглянути подію комплексно з використанням усіх джерел. Швидше за все, ми бачимо концентрацію на показі тільки одного фрагмента цієї історичної події, участі в ньому української сторони. Мовний бар'єр для використання "Літопису" не є більшою проблемою, ніж політичний мотив.
AbstractBackground and aim: Nursing is an stressful profession and nurses are often confronted with various stressors in their work environment. Job stress may have harmful effects on an individual's physical and mental health and decrease quality of nursing care. The purpose of this study was to determine the relationship between job stress and mental health of nurses.Material and Methods: A cross-sectional design was used for the study. The sample of the study consisted of 237 nurses who were selected randomly from teaching hospitals in Hamadan, Iran. Data were collected by the nurses stress scale ( NSS) and GHQ-28. The validity of instruments was assured by a panel of experts. Cronbach's alpha coefficient was computed to assess the reliability of instruments. The reliability coefficient was 0.93 for the nursing stress scale and 0.82 for the General Health Questionnaire. Data were analyzed using the SPSS-13. The relationship between job stress and mental health was investigated by calculating Spearman's correlation coefficients and χ2.Results The majority of nurses reported high levels of job stress. There was a significant relation between sex and number of night shifts with job stress. Psychological problems were seen in 42.9% of hospital nurses. There was a significant correlation between job stress and general health scores. Conclusion: This study showed more than one third of the participants experienced high levels of job stress and mental disorders.There was significant relationship between job stress and mental health. ABSTRACT THE INFLUENCE OF TRANSFORMATIONAL LEADERSHIP AND WORK CULTURE ON HUMAN RESOURCES PERFORMANCE(A Case Study of the State Civil Apparatus, the Department of Population and Regional Civil Registration, Melawi Regency, Nanga Pinoh District, West Kalimantan) By : Bella SafiraB1021161114 Good performance is optimal, efficient and effective performance, namely performance that is in accordance with organizational standards and supports the achievement of organizational goals. Employee performance is influenced by several factors, including the Transformational Leadership Style and Work Culture. Transformational leadership style is the act of a leader who is able to change the perceptions, attitudes and behavior of subordinates regardless of whether the changes occur or not and the Work Culture leads to the unity of the shared meaning system adopted by organizational members that differentiates the organization from other organizations. Work culture in general is a philosophical statement, a function as a binding demand on employees because it can be formally formulated.This study aims to examine and analyze the effect of transformational leadership style and work culture on the performance of human resources in the Department of Population and Civil Registration, Melawi Regency, Nanga Pinoh District. The population in this study were all employees at the Department of Population and Civil Registration of Melawi Regency, Nanga Pinoh District. The sample in this study were 59 respondents. The data analysis model used is Multiple Linear Regression Analysis. Based on the results of the study, it shows that transformational leadership styles and work culture can predict or explain their effects on human resource performance. The results of the t-test study of the transformational leadership style and work culture variables have a significant effect on the performance of human resources. It was highlighted the process and was analysed the reasons pf spreading Russian language and narrowing the sphere of the using Ukrainian language in Ukraine in the second part of the 1940s-1980-s. On the materials of Dnipropetrovs'k region, one of the most developed region of the republic in the agrarian and agricultural sphere, it was shown that it was caused by the politics of the highest state-party leadership of the URSR and the most powerful agricultural construction in the first afterwardecade in the region's towns/cities during that the wide-involvement of the significant mass of people happened from different territories of the Soviet state and their subsidence on the permanent residence in the cities that made the using of Russian language objectively acceptable as the language of the international communication. The combination of these two factors contributed the large-scale deterioration of the Russian language in all spheres of production , administrative and managerial, cultural, everyday life and in the production, was identified the russification of the high, secondary special and professional education, general schools, nursery schools. Russian language became the language of the everyday life for the majority of the urban population. It was detected that the same tendencies started to appear in the village environment. It was analysed the forms and methods of its russification. The attention is focused on the factors that was promoted the wide-spreading and the fixing of the Russian language in the region. The serious influence on this situation had the policy that began to do in the USRS after the ending of the World War II on the initiative of the secretary of the CPSU Zhdanov A. It was called «zhdanivshchyna». The new outbreak of repression was happened during this period that was directed against scientific and creative intelligence. It was happened under the slogan of the struggle with "crawl" in front of west with "the rootless cosmopolitanism", "nationalism", the departure from marxism-leninism doctrine in science, culture, art. It is indicated that, as a result of such policy of the state, the region evolved from monolingual to bilingual. It's known that the situation with permanent ringing of the using of Ukrainian language didn't wick the negative among the population of the region. This position was based on the socially-economic factors (the best security of the industrial and grocery goods, higher salary compared to non-industrial regions, and the level and quality of life). The narrowing of the language sphere of the most Ukrainian ethnic region testified about the inorganic nature of the processes of the language policy in the Republic and the great role of the party-stare leadership in a sphere of the region russification. It's assigned that the process of russification supervised by the manifestations of anger up to the national history, culture, the perception of everything Ukrainian as less valuable. It's shown the demographic intertance of the carried out policy. ; Освещен процесс и проанализированы причины распространения русского языка и сужения сферы применения украинского языка в Украине во второй половине 1940-х‒1980-х гг. На материалах Днепропетровской области, одной из самых аграрно и промышленно развитых областей Украины, показано, что вызвано это было политикой высшего государственно-партийного руководства СССР и мощным промышленным строительством в первые послевоенные десятилетия в городах области, в ходе которого происходило широкое привлечение значительных людских масс из разных территорий Советского государства и оседание их на постоянное место проживания в регіоне, что делало естественным использование русского языка как языка межнационального общения. Показано, что сочетание этих двух факторов способствовало масштабному распространению русского языка во всех сферах производственного, административно-управленческого, культурного, повседневной жизни области, межличностного общения в быту и на производстве, определило русификацию заведений высшего, среднего специального и профессионального образования, общеобразовательной школы, детских дошкольных учреждений. Русский стал для большинства городского населения области языком повседневного общения. Выявлено, что аналогичные тенденции начали проявляться и в сельской среде. Проанализированы формы и методы ее русификации. Внимание сосредоточено на факторах, способствовавших широкому распространению и закреплению в области русского языка. Серьезное влияние на такую ситуацию оказала политика, осуществлявшаяся в СССР по инициативе секретаря ЦК ВКП (б) А. Жданова. В ходе ее в республике произошел новый подъем репрессий, вследствие которых наиболее пострадала научная и творческая интеллигенция. Проводилась она под лозунгами борьбы с «поклонением» перед Западом, с «безродным космополитизмом», «национализмом», «уходом» от марксистско-ленинского учения в науке, культуре, искусстве. Отмечается, что в результате такой политики государства область постепенно эволюционировала от преимущественно монолингвистической к билингвистической. Показано, что ситуация с перманентным сужением использования украинского языка не вызывала негатива у населения области. В основе такой его позиции лежали социально-экономические факторы (лучше обеспеченность региона промышленными и продуктовыми товарами, выше, по сравнению с непромышленными регионами, заработная плата, уровень и качество жизни). Сужение языковой среды украинского этнического большинства области свидетельствовало о неорганичности процессов языковой политики в республике и значительной роли партийно-государственного руководства всех уровней в русификации региона. Отмечено, что процесс русификации сопровождался проявлениями пренебрежения к национальной истории, культуре, восприятием всего украинского как неполноценного. Показано демографические последствия проводимой политики. ; Висвітлено процес і проаналізовано причини поширення російської мови і звуження сфери вживання ‒ української в Україні у другій половині 1940-х–1980-х рр. На матеріалах Дніпропетровської області, однієї з найбільш аграрно і промислово розвинутих областей республіки, показано, що викликано це було політикою вищого державно-партійного керівництва СРСР та потужним промисловим будівництвом у перші повоєнні десятиліття у містах області, в ході якого відбувалося широке залучення значних людських мас з різних територій Радянської держави та осідання їх на постійне проживання у містах, що робило об'єктивно прийнятним використання російської мови як мови міжнаціонального спілкування. Поєднання цих двох факторів сприяло масштабному поширенню російської мови у всіх сферах виробничого, адміністративно-управлінського, культурного, повсякденного життя області, міжособистісного спілкування у побуті і на виробництві, визначило русифікацію закладів вищої, середньої спеціальної та професійної освіти, загальноосвітньої школи, дитячих дошкільних закладів. Російська стала для більшості міського населення області мовою повсякденного спілкування. Виявлено, що аналогічні тенденції почали проявлятися і в сільському середовищі. Проаналізовано форми і методи його русифікації. Зосереджено увагу на факторах, що сприяли впродовж зазначеного періоду широкому поширенню і закріпленню в області російської мови. Серйозний вплив на таку ситуацію мала політика, що почала здійснюватися в СССР після закінчення Другої світової війни за ініціативи секретаря ЦК ВКП(б) А. Жданова. У ході її у республіці відбувся новий спалах репресій, спрямований проти наукової і творчої інтелігенції. Відбувалася вона під гаслами боротьби з «плазуванням» перед Заходом, з «безрідним космополітизмом», «націоналізмом», «відходом» від марксистсько-ленінського вчення в науці, культурі, мистецтві. Зазначається, що внаслідок такої політики держави область поступово еволюціонувала від переважно монолінгвістичної до білінгвістичної. Показано, що ситуація з перманентним звуженням використання української мови не викликала негативу у населення області. В основі такої його позиції лежали соціально-економічні фактори (краща забезпеченість регіону промисловими і продуктовими товарами, вищі, порівняно з непромисловими регіонами заробітна плата та рівень і якість життя). Звуження мовного середовища української етнічної більшості області свідчило про неорганічність процесів мовної політики у республіці та велику роль партійно-державного керівництва у напрямку саме русифікації регіону. Зазначено, що процес русифікації супроводжувався проявами зневаги до національної історії, культури, сприйняттям всього українського як меншовартісного. [spa] El crowdfunding (financiación participativa), como una de las formas más novedosas de financiación, ha logrado una gran atención en España y en todo el mundo por sus características de pequeño tamaño y eficiencia. Este método de financiación alternativo, apoyado por la economía de plataforma, satisface tanto la necesidad de capital de los financiadores como las exigencias de rentabilidad de los inversores. Esta forma de financiación fue regulada en España por la Ley 5/2015, de 27 de abril, de fomento de la financiación empresarial, que ha formado un sistema de transacción y supervisión centrado en las plataformas de financiación participativa, si bien el desarrollo de este nuevo mercado de financiación de España es restringido. Comparando la normativa con la de los Estados Unidos, el Reino Unido, Francia y China, el mercado español de crowdfunding está, por un lado, regulado de forma demasiado estricta, lo que impide que un gran número de plataformas realicen negocios de intermediación; por otro lado, la supervisión no es eficiente, lo que hace que la protección de los intereses de los inversores sea también insuficiente. En 2020, la Unión Europea publicó el Reglamento 2020/1503 relativo a los proveedores europeos de servicios de financiación participativa para empresas, y su aplicación en los próximos años seguramente presionará aún más el mercado nacional existente, mientras que tecnologías como Blockchain y bitcoin también afectarán al desarrollo del crowdfunding debido a su característica descentralizada. Por lo tanto, para aprovechar la tendencia de desarrollo futuro del crowdfunding, se debe cambiar el modo regulatorio rígido: fortalecer la autogestión de las plataformas; hacer uso de las asociaciones para dar soporte a la certificación, la supervisión y la formación de las plataformas, mientras que la autoridad pública puede centrarse en la promoción de las actividades de crowdfunding, mediante la supervisión de las asociaciones y la emisión de orientación; y además, aprovechar la tecnología Blockchain en el futuro para mejorar la transparencia y el proceso de verificación de la información, así como garantizar el cumplimiento del contrato; y, por último, consolidar la educación en materia de la inversión y el riesgo para los inversores, establecer un mecanismo completo de confianza mutua, prestar atención a la protección de los derechos de los pequeños inversores en los proyectos de crowdfunding de capital, entre otros derechos de los inversores y del promotor del proyecto. ; [cat] El crowdfunding (finançament participatiu), com una de les formes més noves de finançament, ha aconseguit una gran atenció a Espanya i a tot el món per les seves característiques de petita mida i eficiència. Aquest mètode de finançament alternatiu, secundat per l'economia de plataforma, satisfà tant la necessitat de capital dels finançadors com les exigències de rendibilitat dels inversors. Aquesta forma de finançament va ser regulada a Espanya per la Llei 5/2015, de 27 d'abril, de foment del finançament empresarial, que ha crear un sistema de transacció i supervisió centrat en les plataformes de finançament participatiu, tot i que el desenvolupament d'aquest nou mercat de finançament d'Espanya és restringit. Si comparant la normativa amb la dels Estats Units, el Regne Unit, França i la Xina, el mercat espanyol de finançament participatiu està, d'una banda, regulat de forma massa estricta, el que impedeix que un gran nombre de plataformes realitzin negocis d'intermediació; d'altra banda, la supervisió no és eficient, el que fa que la protecció dels interessos dels inversors sigui també insuficient. L'any 2020, la Unió Europea va publicar el Reglament 2020/1503, relatiu als proveïdors europeus de serveis de finançament participatiu per a empreses, i la seva aplicació en els anys pròxims segurament pressionarà encara més el mercat nacional existent, mentre que tecnologies com Blockchain i bitcoin també afectaran el desenvolupament del crowdfunding per la seva característica descentralitzada. Per tant, per aprofitar la tendència de desenvolupament futur del finançament participatiu, s'ha de canviar el sistema de regulació rígida: enfortir l'autogestió de les plataformes; fer ús de les associacions per donar suport a la certificació, la supervisió i la formació de les plataformes, mentre que l'autoritat pública pot promoure les activitats de finançament participatiu, mitjançant la supervisió de les associacions i l'emissió d'orientació; i a més, aprofitar la tecnologia Blockchain, en el futur, per tal de millorar la transparència i el procés de verificació de la informació, així como garantir el compliment del contracte; i, finalment, consolidar l'educació en matèria de la inversió i el risc per als inversors, establir un mecanisme complet de confiança mútua, prestar atenció a la protecció dels drets dels petits inversors en els projectes de finançament participatiu d'inversió, entre altres drets dels inversors i el promotor del projecte. ; [eng] Crowdfunding, as one of the newest forms of financing, is gaining widespread attention in Spain and worldwide for its characteristic of small size and efficiency. This alternative financing method, supported by the platform economy, satisfies both financiers' need for capital and investors' requirements for return on investment. Crowdfunding was regulated by Law 5/2015 on the promotion of business financing in Spain, which has formed a crowdfunding platform-centered transaction and supervision system, but the development of this new financing market of Spain is restricted. By comparing the regulations with those of the United States, the United Kingdom, France and China, the Spanish crowdfunding market is, on the one hand, too strictly regulated, which prevents a large number of platforms from carrying out intermediary operations; on the other hand, the regulation is not put into practice, resulting in insufficient protection of investors' interests as well. The European Union released Regulation 2020/1503 on crowdfunding service providers for business in 2020, and its application in the following years will certainly further compress the existing domestic market, while technologies such as Blockchain and bitcoin will also affect the development of crowdfunding due to its decentralized characteristic. Therefore, in order to seize the future development trend of crowdfunding, the strong regulatory model should be changed, that is: strengthening the self-management of platforms; making use of industry associations to assist in the certification, supervision and training of platforms, while the public authority can promote crowdfunding activities by supervising industry associations and issuing guidance; and in addition, blockchain technology can be used in the future to improve the transparency and the process of information verification, as well as guarantee the contract fulfillment; finally, we should strengthen investment and risk education for investors, establish a complete mutual trust mechanism, pay attention to the protection of small investors' rights in equity crowdfunding projects and other rights of investors and project owner.
Preventive medicine is the basis for the development of health care systems in the European Union, and investment in prevention is recognized as the most effective investment in the industry of preserving and improving health. Prophylactic direction – one of the most important in modern dentistry.The aim of the study – analysis of treatment and prevention performance indicators of Municipal Institution Zhytomyr City Dental Hospital No.1 and defi nition of methods for improving the primary prevention.Materials and Methods. We analyzed the annual reports of internal medicine and surgical departments of Zhytomyr City Dental Hospital No. 1 for the period from 2015 till 2016; we held a poll of 115 patients of the establishment regarding the methods for teeth cleaning and using the accessories and means necessary for oral cavity care. The following methods were used: bibliosemantic, system analysis and logical concluding,interview, and mathematical processing of the collected data. Results and Discussion. Numeric materials refl ecting the work of Zhytomyr City Dental Hospital No.1 indicate that doctors employed in the establishment conduct numerous maneuvers related to oral cavity sanitation using modern fi lling materials and diagnostic methods. However (and predominantly), all the work is directed at secondary prevention. As a result, annually a lot of teeth are removed due to complex caries (23.370–23.898), and there are a lot of patients with signs of periodontal diseases (3.521–3.766). Dispensary for diseases includes 94 patients with pre-tumor pathology. Considering that public hygienic teaching and education is one of the main components of primary treatment, we held a poll involving 115 patients of Zhytomyr City Dental Hospital No. 1. Study results indicate that the patients of the dental care entity have insuffi cient knowledge on using the accessories and means necessary for hygienic procedures. All the respondents are more ready to invest into prevention care than internal medicine and surgical dental treatment. It is possible to reach an upward trend in dental health indicators only with primary caries and periodontal diseases prevention. Adding hygienist in the staffi ng list of dental care entities would allow not only for comprehensive primary prevention, but would also unload the high-class dentists enabling them to conduct more complex oral cavity operations.Conclusions. Zhytomyr City Dental Hospital No. 1 employs no hygienists, that is why its work is aimed at secondary treatment of oral cavity diseases. Low sanitary culture level of Zhytomyr residents is tightly connected with dental health indicators. Further development and reforming of dental care demands training of the new generation of highly qualifi ed junior medical staff (hygienists). Involving of hygienists in treatment care allows improving the quality of preventive dental services and decreasing dental diseases incidence rate. ; Профилактическая медицина положена в основу развития систем здравоохранения стран Европейского Союза, а вложенные средства в предупредительное направление признано наиболее эффективными инвестициями в индустрию сохранения и укрепления здоровья. Профилактическое направление – одно из важнейших в современной стоматологии.Цель исследования – проанализировать лечебно-профилактические показатели деятельности Коммунального учреждения «Житомирская городская стоматологическая поликлиника № 1» и определить пути улучшения первичной профилактики.Материалы и методы. Были проанализированы годовые отчеты терапевтического и хирургического отделений КУ «Житомирская городская стоматологическая поликлиника № 1» за период с 2015 по 2016 г.; проведен опрос 115 пациентов заведения относительно методики чистки зубов и использования предметов и средств, необходимых при уходе за полостью рта. В исследовании использованы следующие методы: библиосемантический, системного анализа и логического обобщения, опрос, математической обработки собранных данных.Результаты исследований и их обсуждение. Цифровой материал, воспроизводящий работу Житомирской городской стоматологической поликлиники № 1 (ЖГСП № 1), убеждает, что врачи учреждения выполняют значительный объем вмешательств, связанных с санацией полости рта, используя современные пломбировочные материалы и методы диагностики. Однако (в основном) вся работа направлена на вторичную профилактику. Как следствие, ежегодно фиксируется большое количество удаленных зубов по поводу осложненного кариеса (23 370–23 898), значительное количество пациентов с проявлениями заболеваний пародонта (3521–3766). На диспансерном учете находится 94 пациента с предопухолевой патологией. Учитывая, что гигиеническое обучение и воспитание населения является одной из главных составляющих первичной профилактики, мы провели опрос 115 пациентов с ЖГСП № 1. Результаты исследования показывают, что пациенты стоматологического учреждения недостаточно осведомлены по вопросам применения предметов и средств, необходимых для выполнения гигиенических процедур. Все респонденты готовы вкладывать средства в предупредительные стоматологические мероприятия, а не в терапевтическое и хирургическое лечение зубов. Достичь положительной динамики в показателях, которые воспроизводят стоматологическое здоровье, возможно только в случае первичного предупреждения кариеса и заболеваний пародонта. Введение в штатное расписание стоматологических учреждений должностей гигиениста зубного позволило бы не только полноценно проводить первичную профилактику, но и освободить время высококвалифицированных врачей-стоматологов для проведения сложных манипуляций в полости рта.Выводы. В Житомирской городской стоматологической поликлинике № 1 отсутствуют должности зубных гигиенистов, поэтому работа учреждения направлена на вторичную профилактику заболеваний полости рта. Низкий уровень санитарной культуры населения г. Житомира тесно взаимосвязан с показателями стоматологического здоровья. Дальнейшее развитие и реформирование стоматологической помощи требует подготовки нового поколения высококвалифицированных младших медицинских специалистов (гигиенистов зубных). Привлечение гигиенистов зубных к оказанию профилактической помощи позволит улучшить качество профилактических стоматологических услуг и добиться снижения стоматологической заболеваемости. ; Профілактична медицина покладена в основу розвитку систем охорони здоров'я країн Європейського Союзу, а вкладені кошти в попереджувальний напрямок визнано найефективнішими інестиціями в індустрію збереження та зміцнення здоров'я. Профілактичний напрямок – один з найважливіших у сучасній стоматології.Мета дослідження – проаналізувати лікувально-профілактичні показники діяльності Комунальної установи «Житомирська міська стоматологічна поліклініка № 1» та визначити шляхи поліпшення первинної профілактики.Матеріали і методи. Проаналізовано річні звіти терапевтичного та хірургічного відділень КУ «Житомирська міська стоматологічна поліклініка № 1» за період з 2015 до 2016 р.; проведено опитування 115 пацієнтів закладу щодо методики чищення зубів та використання предметів і засобів, необхідних для догляду за порожниною рота. У дослідженні використано такі методи: бібліосемантичний, системного аналізу та логічного узагальнення, опитування, математичної обробки зібраних даних.Результати досліджень та їх обговорення. Цифровий матеріал, що відтворює роботу Житомирської міської стоматологічної поліклініки № 1 (ЖМСП № 1), переконує, що лікарі закладу виконують значний обсяг втручань, пов'язаних із санацією порожнини рота, використовуючи сучасні пломбувальні матеріали та методи діагностики. Проте (в основному) вся робота спрямована на вторинну профілактику. Як наслідок, щорічно фіксують велику кількість видалених зубів із приводу ускладненого карієсу (23 370–23 898), значну кількість пацієнтів з проявами захворювань пародонта (3521–3766). На диспансерному обліку перебуває 94 пацієнти з передпухлинною патологією. Враховуючи, що гігієнічне навчання та виховання населення є однією з головних складових первинної профілактики, ми провели опитування 115 пацієнтів із ЖМСП № 1. Результати дослідження свідчать, що пацієнти стоматологічної установи недостатньо обізнані з питань застосування предметів та засобів, необхідних для виконання гігієнічних процедур. Усі респонденти готові вкладати кошти в запобіжні стоматологічні заходи, аніж у терапевтичне та хірургічне лікування зубів. Досягти позитивної динаміки в показниках, що відтворюють стоматологічне здоров'я, можливо лише у разі первинного попередження карієсу та захворювань пародонта. Введення в штатний розпис стоматологічних закладів посад гігієніста зубного дозволило б не тільки повноцінно проводити первинну профілактику, а й звільнити час високо кваліфікованих лікарів-стоматологів для проведення складних маніпуляцій у порожнині рота.Висновки. У Житомирській міській стоматологічній поліклініці № 1 відсутні посади зубних гігієністів, а тому і робота закладу спрямована на вторинну профілактику захворювань порожнини рота. Низький рівень санітарної культури населення м. Житомира тісно взаємопов'язаний із показниками стоматологічного здоров'я. Подальший розвиток та реформування стоматологічної допомоги потребує підготовки нового покоління висококваліфікованих молодших медичних спеціалістів (гігієністів зубних). Залучення гігієністів зубних до надання профілактичної допомоги дозволить поліпшити якість профілактичних стоматологічних послуг та досягти зниження стоматологічної захворюваності.
Purpose of the study. The article explores the socio-pedagogical prerequisites and the main stages in the development of student self-management as a socio-cultural phenomenon and as a special form of initiative, independent public activity of students aimed at solving important issues of life activity of the university, developing social activity, supporting civic initiatives of student youth. During the research a number of general scientific methods were used: historical, descriptive and comparative. With the help of the historical method, the main stages of the development of student self-government, the features of its transformation in the educational space, were analyzed. The descriptive method made it possible to single out and systematize the features of the functioning of bodies of student government at all stages of its development. The application of the comparative method helped to identify the main differences and general characteristics in the structures of student self-government and procedurally provided a comparison of the implementation of the rights and competencies of student self-government bodies. Scientific novelty. For the first time, the main stages of the formation and development of student self-government in the Higher State Educational Institution of Ukraine "Bukovina State Medical University" are systematically presented, the understanding of the essence and functions of student self-government in the higher educational institution was deepened, and ideas for attracting students as competent, active and constructive partners in creation and formation of the European Higher Education Area. Conclusions. It is proved that student self-government is a form of self-organization of students, a mechanism for representation and assertion of one's rights, the possibility of self-realization. It is student self-management that is called upon to protect the rights of students and be their representative in the administrations of universities. Student self-government is determined by the law and the real ability of the student community to independently resolve issues within the framework of the current legislation and the university statute. The authors analyzed the development of student self-government in the historical aspect, which made it possible to determine the direct relationship between the essential features of its development and socio-political tendencies in the state. ; But de l'étude L'article examine le contexte social et éducatif, et les principales étapes du développement de l'autogestion des étudiants en tant que phénomène social et culturel et comme une forme particulière d'initiative, les activités d'auto-sociale des étudiants pour faire face aux questions importantes de l'université de la vie, le développement de l'activité sociale, le soutien des initiatives civiles des étudiants. Au cours de la recherche, un certain nombre de méthodes scientifiques générales ont été utilisées: historique, descriptive et comparative. En utilisant la méthode historique ont été analysés par étapes principales étudiants de l'auto en particulier sa transformation en espace éducatif. méthode descriptive a permis d'identifier et de systématiser les caractéristiques de fonctionnement des organes d'autonomie gouvernementale des étudiants à tous les stades de son développement. L'utilisation de la méthode comparative a permis d'identifier les principales différences et caractéristiques communes dans la structure du gouvernement étudiant et la procédure a fourni une comparaison des droits et des compétences du gouvernement étudiant. Nouveauté scientifique. Pour la première fois présente systématiquement les principales étapes de la formation et le développement de l'autonomie des étudiants dans le Suprême « State Medical University Bucovine » établissements d'enseignement publics de l'Ukraine, en profondeur la compréhension de la nature et les fonctions du gouvernement étudiant à l'université, a été développé pour attirer les étudiants en tant que partenaires compétents, actifs et constructifs dans la création et la formation de l'espace européen de l'enseignement supérieur. Conclusions Il est prouvé que l'autonomie gouvernementale des étudiants est une forme d'auto-organisation des étudiants, un mécanisme de représentation et d'affirmation de leurs droits, la possibilité de réalisation de soi. Ce gouvernement étudiant destiné à protéger les droits des étudiants et d'être leur représentant dans les administrations des universités. Le gouvernement étudiant est déterminé par le droit et la communauté étudiante la capacité de résoudre des problèmes de façon indépendante dans le cadre de la législation en vigueur et le statut universitaire. Les auteurs ont analysé le développement de l'autonomie des élèves dans l'aspect historique, permettant de déterminer une corrélation directe entre les caractéristiques essentielles de son développement et les tendances socio-politiques du pays. ; Цель исследования. В статье исследованы социально-педагогические предпосылки и основные этапы развития студенческого самоуправления как социокультурного феномена и как особой формы инициативной, самостоятельной общественной деятельности студентов, направленной на решение важных вопросов жизнедеятельности вуза, развитие социальной активности, поддержку гражданских инициатив студенческой молодежи. При проведении исследования были использованы ряд общенаучных методов: исторический, описательный и сравнительный. С помощью исторического метода были проанализированы основные этапы развития студенческого самоуправления, особенности его трансформации в образовательном пространстве. Описательный метод дал возможность выделить и систематизировать особенности функционирования органов студенческого самоуправления на всех этапах его развития. Применение сравнительного метода способствовало выявлению основных различий и общих характеристик в структурах студенческого самоуправления и процедурно обеспечило сравнение реализации прав и компетенций органов студенческого самоуправления. Научная новизна. Впервые системно представлены основные этапы становления и развития студенческого самоуправления в Высшем государственном учебном заведении Украины «Буковинский государственный медицинский университет», углубленно понимания сущности и функций студенческого самоуправления в высшем учебном заведении, получили дальнейшее развитие идеи по привлечению студентов как компетентных, активных и конструктивных партнеров в создании и формировании Европейского пространства высшего образования. Выводы. Доказано, что студенческое самоуправление является формой самоорганизации студентов, механизмом представительства и отстаивания своих прав, возможностью самореализации. Именно студенческое самоуправление призвано защищать права студентов и быть их представителем в администрациях вузов. Студенческое самоуправление определяется правом и реальной способностью студенческой общины самостоятельно решать вопросы в рамках действующего законодательства и устава университета. Авторами проведен анализ развития студенческого самоуправления в историческом аспекте, что позволило определить прямую зависимость между сущностными особенностями его развития и общественно-политическими тенденциями в государстве. ; Мета дослідження. Застосування порівняльного методу сприяло виявленню основних відмінностей та спільних характеристик в структурах студентського самоврядування та процедурно забезпечило порівняння реалізації прав і компетенцій органів студентського самоврядування. Вперше системно представлені основні етапи становлення та розвитку студентського самоврядування у Вищому державному навчальному закладі України «Буковинський державний медичний університет», поглиблено розуміння сутності і функцій студентського самоврядування у вищому навчальному закладі, отримали подальший розвиток ідеї щодо залучення студентів як компетентних, активних і конструктивних партнерів у створенні і формуванні Європейського простору вищої освіти. Висновки. Доведено, що студентське самоврядування є формою самоорганізації студентів, механізмом представництва і відстоювання своїх прав, можливістю самореалізації. Саме студентське самоврядування покликане захищати права студентів і бути їхнім представником в адміністраціях ВНЗ. Студентське самоврядування визначається правом і реальною здатністю студентської громади самостійно вирішувати питання в рамках чинного законодавства і статуту університету. Авторами проведено аналіз розвитку студентського самоврядування в історичному аспекті, що дозволило визначити пряму залежність між сутнісними особливостями його розвитку і суспільно-політичними тенденціями в державі.
Preventive medicine is the basis for the development of health care systems in the European Union, and investment in prevention is recognized as the most effective investment in the industry of preserving and improving health. Prophylactic direction – one of the most important in modern dentistry.The aim of the study – analysis of treatment and prevention performance indicators of Municipal Institution Zhytomyr City Dental Hospital No.1 and defi nition of methods for improving the primary prevention.Materials and Methods. We analyzed the annual reports of internal medicine and surgical departments of Zhytomyr City Dental Hospital No. 1 for the period from 2015 till 2016; we held a poll of 115 patients of the establishment regarding the methods for teeth cleaning and using the accessories and means necessary for oral cavity care. The following methods were used: bibliosemantic, system analysis and logical concluding,interview, and mathematical processing of the collected data. Results and Discussion. Numeric materials refl ecting the work of Zhytomyr City Dental Hospital No.1 indicate that doctors employed in the establishment conduct numerous maneuvers related to oral cavity sanitation using modern fi lling materials and diagnostic methods. However (and predominantly), all the work is directed at secondary prevention. As a result, annually a lot of teeth are removed due to complex caries (23.370–23.898), and there are a lot of patients with signs of periodontal diseases (3.521–3.766). Dispensary for diseases includes 94 patients with pre-tumor pathology. Considering that public hygienic teaching and education is one of the main components of primary treatment, we held a poll involving 115 patients of Zhytomyr City Dental Hospital No. 1. Study results indicate that the patients of the dental care entity have insuffi cient knowledge on using the accessories and means necessary for hygienic procedures. All the respondents are more ready to invest into prevention care than internal medicine and surgical dental treatment. It is possible to reach an upward trend in dental health indicators only with primary caries and periodontal diseases prevention. Adding hygienist in the staffi ng list of dental care entities would allow not only for comprehensive primary prevention, but would also unload the high-class dentists enabling them to conduct more complex oral cavity operations.Conclusions. Zhytomyr City Dental Hospital No. 1 employs no hygienists, that is why its work is aimed at secondary treatment of oral cavity diseases. Low sanitary culture level of Zhytomyr residents is tightly connected with dental health indicators. Further development and reforming of dental care demands training of the new generation of highly qualifi ed junior medical staff (hygienists). Involving of hygienists in treatment care allows improving the quality of preventive dental services and decreasing dental diseases incidence rate. ; Профилактическая медицина положена в основу развития систем здравоохранения стран Европейского Союза, а вложенные средства в предупредительное направление признано наиболее эффективными инвестициями в индустрию сохранения и укрепления здоровья. Однако (в основном) вся работа направлена на вторичную профилактику. Как следствие, ежегодно фиксируется большое количество удаленных зубов по поводу осложненного кариеса (23 370–23 898), значительное количество пациентов с проявлениями заболеваний пародонта (3521–3766). Учитывая, что гигиеническое обучение и воспитание населения является одной из главных составляющих первичной профилактики, мы провели опрос 115 пациентов с ЖГСП № 1. Результаты исследования показывают, что пациенты стоматологического учреждения недостаточно осведомлены по вопросам применения предметов и средств, необходимых для выполнения гигиенических процедур. Все респонденты готовы вкладывать средства в предупредительные стоматологические мероприятия, а не в терапевтическое и хирургическое лечение зубов. Достичь положительной динамики в показателях, которые воспроизводят стоматологическое здоровье, возможно только в случае первичного предупреждения кариеса и заболеваний пародонта. Введение в штатное расписание стоматологических учреждений должностей гигиениста зубного позволило бы не только полноценно проводить первичную профилактику, но и освободить время высококвалифицированных врачей-стоматологов для проведения сложных манипуляций в полости рта.Выводы. В Житомирской городской стоматологической поликлинике № 1 отсутствуют должности зубных гигиенистов, поэтому работа учреждения направлена на вторичную профилактику заболеваний полости рта.
