PURPOSE: We investigated the cumulative live birth rate (CLBR) in women receiving governmental subsidies for assisted reproductive technology (ART) in Saitama Prefecture, Japan. METHODS: Women who applied for subsidies from Saitama Prefectural Government for the first time in 2016 were enrolled and followed up until the end of 2017. Treatment information, including live birth, was obtained from the Japanese ART registry by linking it with unique identification numbers for treatment. Patients' factors associated with having a live birth were investigated. RESULTS: Of 1,072 women (2,513 applications), 495 (46.2%) had a live birth with 8 (1.6%) twin pregnancies. The CLBR over six subsidized cycles was 53.7% for women aged <40 years, and 17.2% over three subsidized cycles for women 40‐42 years; highest among women <35 years (58.4%), followed by those aged 35‐39 years (49.3%). Multivariate analysis revealed patient age as the only independent factor for having a live birth. CONCLUSIONS: The CLBR of women receiving subsidies for ART was greatest in women aged <35 years. Effective policies for promoting ART among younger couples who seek infertility treatment are essential.

AbstractAccurate knowledge of the live birth rate for cleavage stage embryos is essential to determine an appropriate number of embryos to transfer at once. Results from previous studies lack details needed for practical use. This is a mathematical analysis and model building study of day 3 cleavage stage embryo transfers. A total of 996 embryos were transferred in 274 fresh and 83 frozen embryo transfers. Embryo morphology was divided into 4 groups based on number of cells and fragmentation percentage. Each embryo transfer was modeled as an equation equating the sum of the live birth rates of the transferred embryos to the number of live births that resulted. The least squares solution to the system of embryo transfer equations was determined using linear algebra. This analysis was repeated for ages 35 to 42 years old at oocyte retrieval. The best fit live birth rates per embryo in the age group centered on 35 years old were 29%, 13%, 10%, and 9% for embryos in the 8-cell with ≤ 5% fragmentation, 8-cell with > 5% fragmentation, 9–12 cell, and 6–7 cell groups, respectively. Cleavage stage embryos with fewer than 6 cells on day 3 had very low best fit live birth rates close to 0% at age 39 years and were excluded from the primary analysis to prevent overfitting. These live birth rates can be used with a simple embryo transfer model to predict rates of single and multiple gestation prior to a planned cleavage stage embryo transfer.

While considerable research has been carried out on the experiences of birth mothers in adoption, birth fathers remain a relatively neglected group. As part of an ongoing project to redress the balance, Gary Clapton explores the life course of a group of 30 birth fathers ranging in age from 35 to late 60s. Beginning with the immediate post-adoption period, he traces the men's early feelings of grief and loss, and in a minority of cases, alleged indifference, through to a spectrum of emotions spanning curiosity, concern, regret and 'connectedness'. Clapton points to similarities with the reported experiences of birth mothers, including a continuing sense of parenthood, and highlights the need to rethink notions of fatherhood. He calls for a greater focus on birth fathers in adoption, not only for their sakes but in the direct interests of the adopted person seeking knowledge of their birth family history.

These global success rates may greatly depend on the proportion of couples who give up before having got a baby because their success probability is likely nor null, nor identical to the one of couple who did carry on the IVF program. Our objective is to investigate new statistical methods to estimate the cumulative success rates after 4 IVF attempts taking into account treatment interruptions.MATERIAL AND METHODS. We analysed data from 3,037 couples beginning an IVF treatment in two French IVF centres between 1998 and 2002, one in Paris and the other in Clermont-Ferrand (a mid-town in the centre of France). We defined IVF success rate as the percentage of couples beginning treatment at an IVF center who finally obtain a liveborn child, taking into account fresh and frozen transfers for one attempt and taking into account successive attempts of the couple in the center. Multiple imputations have been used to provide an estimated success rate among couples who gave up before getting a child. A global success rate after 4 IVF attempts for the whole population has then been computed.RESULTS. At the first attempt 21% of the couples obtained a child and 32% gave up. Taking into account the whole course in the centre, 37% (n=1117) of couples obtained a birth, 52% (n=1576) interrupted IVF before the 4th attempt and 11% (n=344) had completed 4 unsuccessful attempts. Teenage childbearing has been the subject of long-standing concern among the public and policy makers. Teenagers who give birth are much more likely to deliver a low birthweight or preterm infant than older women, and their babies are at elevated risk of dying in infancy. The annual public costs associated with teen childbearing have been estimated at $9.1 billion. The U.S. teen birth rate fell by more than one-third from 1991 through 2005, but then increased by 5 percent over two consecutive years. Data for 2008 and 2009, however, indicate that the long-term downward trend has resumed. Although the recent declines have been widespread by age, race and ethnicity, and state, large disparities nevertheless persist in these characteristics. This paper explores the relationship between birth rate and work load. More babies are born in the period when there is less work to be done but there are also other factors relevant to the number of conceptions and these include opportunities for conception, physical stamina, schedule of physical activity, leisure patterns, holidays, knowledge of contraception, climate and household temperatures, and differential fertility.
